Syftet med detta examensarbete är att ta reda på aktuell fakta om ESBL och Clostridium difficile samt att få fram hur man som vårdare kan förhindra smittspridning av dessa infektioner. Respondenten vill förmedla enhetliga vårdföreskrifter vid ESBL och Clostridium difficile-infektioner till en boendeenhet för äldre personer i Finland. Examensarbetet gjordes för att öka vårdpersonalens kunskap om dessa infektioner samt för att få kunskap om hur man undviker smittspridning av infektionerna. Forskningsfrågan är: Hur förhindra smittspridning av ESBL och Clostridium difficile på ett äldreboende.
Respondentens teoretiska utgångspunkter är vårdlidande utgående från Eriksson (1993, 1994) samt god vård av Eriksson (2/1990) och av Norberg (1994). Studien är kvalitativ. Datainsamlingen är en litteraturstudie som består av vetenskapliga artiklar. De valda artiklarna har sammanställts i en resumé (bilaga 1). Resumén analyserades med hjälp av innehållsanalys. Därefter tolkades materialet mot den teoretiska referensramen och mot den teoretiska bakgrunden.
Ur resultatet framgår tre områden som behöver beaktas på ett äldreboende: kunskap och information om ESBL och Clostridium difficile, snabb och säker diagnos med observationer och riskfaktorer samt utvecklingen av vårdföreskrifter. Utgående från resultatet har gjorts vårdföreskrifter om ESBL (bilaga 2) och Clostridium difficile (bilaga 3) för all personal, patienter och besökare. Vårdföreskrifterna har gjorts i form av plastade scheman som vid en aktiv infektion sättas upp i boenderummet. The purpose of this thesis is to find out the actual facts of ESBL and Clostridium difficile, and find out how caregivers can prevent the spread of these infections. The respondent wants to convey uniform health regulations concerning ESBL and Clostridium difficile infections at a housing unit for the elderly in Finland. The work was done to improve health professionals' knowledge about these infections and to gain knowledge on how to avoid the spread of infections. The research question is: How to prevent the spread of ESBL and Clostridium difficile in a nursing home.
Respondent's theoretical premises are health suffering starting from Eriksson (1993, 1994) as well as good care of Eriksson (2/1990) and Norberg (1994). The study is qualitative. The data collection has been carried out as a literature study consisting of scientific articles. The selected articles have been compiled into a summary (Appendix 1). Summary has been analyzed using content analysis. The material was then interpreted against the theoretical framework and background.
The results show that the nursing home needs to concider three areas: knowledge and information about ESBL and Clostridium difficile, the rapid and safe diagnosis of observations and risk factors, and the development of health regulations. Based on these results, the respondent has made care regulations for ESBL (Annex 2) and for Clostridium difficile (Appendix 3) for all staff, patients and visitors. The health regulations have been carried out in the form of laminated schedules that should be put up in the patient´s room in case of active infection.
