The purpose of the thesis, commissioned by the Emergency Medical Services of the North Karelia Rescue Department, was to develop a quality indicator for the care of patients with ST-elevation myocardial infarction. With the help of this indicator, the quality of pre-hospital care of patients with ST-segment elevation myocardial infarction can be developed and improved. The study was conducted using a three-staged Delphi method. For the purposes of this study, the author of the thesis summoned a development group consisting of eight (n=8) specialists with relevant expertise in the treatment of patients with ST-elevation myocardial infarction. Five of the specialists (n=5) participated in the first two rounds of questionnaires of the Delphi process and four of them (n=4) participated in the last stage, i.e. the expert panel, of the Delphi process. The expert panel reached a consensus on the factors that measure the quality of pre-hospital care of patients with ST-segment elevation myocardial infarct. As a result, this thesis developed a quality indicator for the care of patients with ST-elevation myocardial infarction to be used by the Emergency Medical Services of the North Karelia Rescue Department. More research is needed to pilot the indicator and to develop it further....

The aim of this thesis was to develop the multi-sectoral network cooperation in youth ser-vices in the municipality of Liperi. The youth guidance and service network Pave is acting in Liperi according to youth act. It was examined what is the multi-sectoral network coop-eration like in the youth services in Liperi and how it should be developed. The develop-ment group of the network Veke was established to support the development work. The project was carried out by the principles of developmental work research where ap-propriate. The activities of the guidance and service network are analyzed with the devel-opment group of the network using the human activity system model. The development process of the network is represented after the cycle of expansive learning. Methods of this thesis were learning café and focused interview (n=3). The outcome of the development work was a network program which included the tasks and aims of the network and also a year clock. On the basis of this research, it can be noticed that the multi-sectoral network cooperation in the youth services in Liperi is cus-tomer-oriented and positive for development. A solid operational culture determines the activities of the network. The development of multi-sectoral network cooperation includes not only the development of procedures and management and strengthening of infor-mation production but also elements from political committing and impacting. Developing of employment-enhancing activities is in a transitional stage. New methods and models, as well as more multi-professional cooperation are needed for promoting employment. In Finland development work for employment-enhancing is being implemented between 1 September 2012 and 31 December 2015 in the Municipal Trial for the Reduction of Long-term Unemployment Project among other projects. These projects were launched as key projects by Ministry of Employment and the Economy in the current government programme. They focused on development and the experiments in cutting down on the long-term unemployment. The objective of this theses was to develop a new operating model to help the long-term unemployed find a way out of the unemployment vortex. Developmental work research was used in compiling the thesis. The development work was carried out in the Project Municipal Trial for the Reduction of Long-term Unemployment in Lieksa and the material was collected by interviewing the work coaches of the Project. A new customer-oriented tool to be used in the work carried out with the long-term unemployed, was created as a result of this development work. It helps the unemployed out of the spiral of unemployment. Many good customer-oriented benefits were achieved with the tool, such as the improvement of the customer’s functional ability, his/her quality of life that the way out of the unemployment. This tool will be introduced as part of promoting employment in the town of Lieksa. The further development of the tool will continue in practice....

Äitiysneuvolan sähköiset palvelut

Tiivistelmä
Terveydenhuollon sähköisten palveluiden kehittäminen on ajankohtainen asia Suomes-sa. Pohjois-Karjalassa käytössä olevien Medinet-palvelun ja Mediatri-tietojärjestelmän yhteensopivuus on toteutunut. Yhteistoiminnallisuus monimuotoistuu Assi-hankkeen myötä, jonka yksi osa tämä kehittämistehtävä on.
Kehittämistyön lähtökohtana on ollut terveydenhoitajatyön sisällön kehittäminen. Äitiys-neuvolan ja perhesuunnitteluneuvolan esitietietolomakkeen kehittäminen ja sähköistä-minen toteutui osana Assi-hankketta. Tietoteknisestä osaamisesta vastasivat Pohjois-Karjalan tietotekniikkakeskus Oy ja Mediconsult Oy. Opinnäytetyön tekijä vastasi lo-makkeiden sisällöstä.
Medinet- ja Mediatri-tietojärjestelmien välistä tietojen siirron yhteistoiminnallisuutta hyödynnetään esitietolomakkeissa. Asiakas tuottaa itse tietoa terveydestään ja hyvinvoinnistaan. Tieto siirtyy ohjelmien keskinäisen toiminnallisuuden avulla terveystietojärjes-telmään osaksi asiakkaan omaa terveyskertomusta olemassa oleville äitiys- ja perhe-suunnitteluneuvolan lomakkeille.
Asiakaskyselyssä selvitettiin asiakkaiden näkemyksiä Medinet-palvelusta ja neuvolan sähköisistä palveluista yleensä sekä näkemyksiä uudistetun lomakkeen sisällöstä. Medinet-palvelu koettiin hyvänä lisänä, palvelun käyttömahdollisuuksien toivottiin laajene-van. Äitiysneuvolan esitietolomakkeen käyttäminen koettiin helpoksi. Kysymykset olivat ymmärrettäviä ja tarpeellisia. Lomakkeen ulkoasu oli selkeä ja kysymysten ryhmittely looginen. Elintapoihin ja terveystottumuksiin liittyvien kysymysten ei koettu herättävän pohdintoja omaan terveyteen vaikuttavista asioista....

In social work, development work is often done in projects. In project work, evaluation is important. This thesis describes how process evaluation can be put into practice and how it can be used in development projects as a tool for supporting and guiding the work. The thesis concentrates on ongoing process evaluation in a project. Evaluation supports the execution of the development process in a goal-oriented, mindful, apt and efficient way. The project of the Federation of Mother and Child Homes and Shelters, a family with a baby as an asylum seeker (2012-2014), strives to increase the welfare of the families with a baby or small children who have applied for an asylum. As a commission from the project, a process evaluation was carried out in the middle of the project in 2013. Data for the evaluation was gathered by means of a group interview, a query for the team that was responsible for the development of the project and by using the learning café method for the same team. The gathered data was analyzed thematically. The results from the process evaluation were compiled in a process report. The project uses this report when demonstrating the project for the stakeholders and for others interested in the subject. Executing a process evaluation is a demanding task for the person carrying it out, but it pays off for the development team. Data-gathering was demanding and time-consuming and so was the data processing and analysis. On the other hand, the results from the evaluation process were available for the project team on a first-hand basis, which enabled quick adjustments in the project coordination. At the time of the writing, the project still goes on. Therefore all the benefits from the project evaluation are yet to be seen....

Työterveyshuolto esimiestyön psykososiaalisen työhyvinvoinnin tukena

Hyvinvoiva esimies on keskeinen tekijä työntekijöiden ja työyhteisöjen hyvinvoinnille sekä työn tuloksellisuudelle. Muuttuvassa työelämässä esimiestyön vaatimukset kasvavat ja henkinen kuormittavuus lisääntyy. Esimiestyön merkitystä työntekijöiden työhyvinvointiin on tutkittu lukuisissa tutkimuksissa. Hoitoalan esimiesten työhyvinvointiin liittyvää tutkimus- ja kehittämistoimintaa on tehty kuitenkin suhteellisen vähän.
Opinnäytetyön tarkoituksena oli kehittää työterveyshuollon palveluja esimiestyön psykososiaalista työhyvinvointia aikaisempaa tukevammaksi ja ennakoida tuen tarpeita seuraavien 5–10 vuoden aikana. Toimintatutkimukselliseen kehittämisprosessiin osallistui hoitotyön esimiehiä ja henkilöstöhallinnon edustaja Pohjois-Karjalan sairaanhoito- ja sosiaalipalveluiden kuntayhtymästä sekä työterveyshuollon ammattilaisia Joensuun Työterveydestä ja Tietotaito Group Oy:stä. Kehittämistoiminnan tapahtumista kerätty aineisto analysoitiin aineistolähtöistä analyysiä soveltaen. Tuloksena syntyi esimiestyön työterveyshuollon toimintamallin kehikko siirrettäväksi kohdeorganisaation esimiesten sähköiseen työkalupakkiin ja kehitettäväksi edelleen esimiesten, kohdeorganisaation edustajien ja työterveyshuollon yhteistyössä.
Esimiestyön työterveyshuollon toimintamalli on jaettavissa henkilökohtaiseen tukeen ja työterveysyhteistyöhön. Henkilökohtainen tuki jakaantuu edelleen ennaltaehkäisevään sekä työ- ja toimintakykyä palauttavaan tukeen ja esimiestyön työhyvinvointia tukevaan ryhmätoimintaan. Työterveysyhteistyö on jaettavissa työyhteisöön ja työhön sekä yksittäisen työntekijän asioissa tehtävään yhteistyöhön. Toimintamalli tukee ja ohjaa asiakasesimiehiä psykososiaaliseen työhyvinvointiin liittyvissä asioissa toimien samalla työterveyshuollon moniammatillisen toiminnan työvälineenä. Opinnäytetyön tuloksia voidaan hyödyntää esimiestyön työhyvinvointia kehitettäessä soveltaen eri aloille. Tuloksia voidaan hyödyntää myös työterveyshuollon ja asiakasorganisaatioiden työterveysyhteistyötä kehitettäessä. Esimiestyön työhyvinvointia tutkivaa ja kehittävää toimintaa tarvitaan lisää, koska työkykyjohtaminen ja esimiesten siinä tukeminen korostuvat tulevaisuudessa. Esimiestyöhön suunnatun työhyvinvointitapaamisen mallin luominen on jatkossa tarpeen....
