The primary purpose of this study is to describe the way relatives of elderly memory disorder patients experienced a creative process that their loved ones took part in. The storycomposing project was part of a project called Art to Care Homes (“Taidetta hoivalaitoksiin”) and it was funded by the Finnish Cultural Foundation. It was organized in a care home for elderly in Southern Finland. The memory disorder patients attended storycomposing sessions supervised by a trained expert and later performed their compositions in a private concert. The aim of this thesis was to examine what kinds of emotions the relatives and loved ones experienced during the concert and what kind of meaning they gave to those emotions. The study was based on qualitative methods and the theoretical background lies in the previous research on memory disorder patients’ loved ones and also in the study of using creative methods, especially music, in rehabilitating memory disorder patients. Methodologically it is based on phenomenology. The method of data collection was theme conversation. After describing the data, the analysis aims at forming structures of meaning from the multiplicity and diversity of the data. After that, a synthesis is created based on all the structures of meaning that had something in common. The results showed seven different structures of meaning: gratefulness, relieve, curiosity, astonishment, joyfulness, gladness, affection and forgiveness. The study showed that not only did this non-medical treatment cheer up the memory disorder patient herself, but also it awakened very strong emotions and experiences in the loved ones, and these emotions seemed to be positive. To imbed creative rehabilitation methods in the everyday routines of elderly care still requires lots of work and changes in attitudes. This thesis hopefully encourages nurses and nurse practitioners to try even small creative moments as a part of everyday routines with their patients, because the effects can be surprisingly positive. To listen and highlight the experience of relatives is one way to increase the health care professionals’ awareness when it comes to creative rehabilitation methods. Relatives and loved ones are important companions to memory disorder patients and a big resource for the staff. Supporting their strength indirectly, by providing the memory disease patient creative activities, can be very fruitful. It would therefore be interesting to study in more depth how relatives experience other creative activities that are surely provided increasingly in care homes around Finland....

The purpose of this thesis was to produce two simulations of type 1 diabetes to the nursing students at Laurea Otaniemi. The aim was to improve the nursing education at Laurea University of Applied Sciences. The purpose of the simulations was to deepen students' knowledge about this topic. Thesis was made to Laurea University of Applied Sciences. The theoretical framework consisted of type 1 diabetes and its treatment, guidance in nursing, family nursing, and simulation-based learning. This thesis was carried out as a functional thesis. The product was two simulation exercises to the nursing students at Laurea Otaniemi, arranged on two days. The first simulation exercise consisted of injecting insulin, assessing the amount of the insulin and counting carbohydrates. The second simulation was about supporting families and guidance in nursing. After the first simulation day, feedback was collected from the students using an evaluation form created for this purpose. Simulation exercises were improved based on the received feedback, for the next teaching day. According to the feedback given by the students, the simulations were useful and deepened their expertise. In the future, the diabetes related simulations could be expanded to a bigger entity to be a part of the education. For example a diabetes day could be organized where students would be given different kinds of exercises about hypoglycemia and first aid situations....

Simulaatio-opinnot moniammatillisessa koulutuksessa terveydenhuollossa - kirjallisuuskatsaus

Tämän opinnäytetyön tavoitteena oli selvittää, onko moniammatillisista simulaatioista hyötyä sosiaali- ja terveydenhuollon opinnoissa. Opinnäytetyön tilaaja on Laurea Hyvinkään ammattikorkeakoulun Simulaatio-oppiminen ja –opetus kasvuun hanke. Menetelmänä käytetään kuvailevaa kirjallisuuskatsausta. Tutkimusaineistoa on haettu kansallisista ja kansainvälisistä tutkimuksista Medicin, Cinahlin sekä PubMedin tietokantahauilla.
Moniammatillinen yhteistyö on tärkeässä osassa asiakaslähtöisessä ja turvallisessa hoitotyössä. Suomessa meneillään oleva sosiaali- ja terveydenhuollon integraatio tekee aiheesta ajankohtaisen. Kuitenkin nykyiset sosiaali- ja terveysalan tutkinto-opinnot ovat hyvin erillään toisistaan. Onkin syytä pohtia tapoja, jolla moniammatillisuutta päästäisiin kehittämään jo opintojen aikana. Hyvin suunnitellut moniammatilliset simulaatiot voisivat olla yksi keino tähän. Todellisen tilanteen jäljitelmänä, simulaatiot pääsevät lähelle reaalimaailmaa ja opiskelijat voivat harjoitella yhteistyötaitoja turvallisesti ja potilaita vaarantamatta.
Kirjallisuuskatsaus osoitti, että moniammatillista simulaatioista voi olla todellista hyötyä sosiaali- ja terveydenhuollon opinnoissa. Ne kehittivät opiskelijoiden kommunikaatio ja yhteistyötaitoja ja toivat varmuutta ja kokemusta toimia moniammatillisessa tiimissä. Opiskelijat kokivat simulaatiot positiivisina oppimiskokemuksina ja he arvostivat mahdollisuutta moniammatillisuuden kehittämiseen jo opiskeluaikana. Moniammatilliset opinnot kiteytettiin usein lauseeseen ”It is a matter of learning with, from and about each other”. Voidaankin todeta, että moniammatilliset simulaatiot ovat varteenotettava keino edistää moniammatillisuuden kehittymistä sosiaali- ja terveydenhuollon opinnoissa....

The city of Hyvinkää provided us with the subject of our thesis. This thesis was produced in co-operation with the Hyvinkää city home care. The thesis considered,on a wider scale, a nurse’s competence regarding mobile technology, as well as the skills needed by a nurse, when using mobile technology. In the conceptual context, invocation of digital services and mobile technology in health care was viewed on a wider scale. The goal of this thesis was to describe how well nurses can utilise mobile technology in their work at the home care of Hyvinkää. The aim was to produce information of the know-how of mobile technology as well as the benefit given to nurses by mobile devices, and also how this new operation model could be developed further. This thesis was implemented as qualitative research, and the objectives were the nurses’ own experiences about mobile technology. The research was done by using themed interviews and the analysis was made as a content analysis. Four Hyvinkää home care nurses with experience of mobile technology were interviewed. This is a work-based thesis. The key results of the thesis were that the nurses’ experiences of mobile technology were positive, the planning of the care is customer-based and that the nurses are required to have a vast knowledge of mobile technology. The planning of one’s own work was related to mobile technology, as well as knowing how to use the Hilkka-information system. The compatibility of different information systems and the Hilkka-system were observed as development objects. The conclusion of this thesis was that the planning of customer care was somewhat different between different information systems. It was also stated that mobile technology could be used more in home care, than what is done today. The thesis suggested that the information systems Hilkka and Pegasos should be more compatible in order to make the nurse’s work easier to organise, and customer-based nursing more smoother...

This thesis was carried out as a commissioned study for the Southern home care unit of the City of Helsinki. The thesis is concerned with the tasks nurses have and how the practical nurses are completing medical procedures and ensuring the medical competence in home care, provided by the City of Helsinki. Nurses’ tasks in home care include the monitoring of overall competence in medication administration and the responsibility for providing orientation on medication for practical nurses, nursing assistants, nursing students and home care workers. The nursing procedures are bound by a statutory licensing practice, and the City of Helsinki has created a system for the process of demonstrating competence, which aims at ensuring the medication and nursing procedures competence of nursing staff. The purpose of this thesis was to gather knowledge of the appraisal of practical nurses in nursing and medical skills after competence demonstration, by a nurse manager. The key concepts used in this thesis include professional competence, evidence based nursing, patient safety, counselling competence and clinical nursing. Other essential concepts include the legislation steering the work as well as working in home care. A quantitative approach was selected as the research method. An electronic questionnaire survey was conducted among 44 nurses and public health nurses working in the Southern home care unit of the City of Helsinki in the autumn of 2017 (Oct 9, 2017 to Oct 29, 2017). 11 nurses responded to the questionnaire. The questions were formulated as statements and divided into themes according to the research questions. A 5-point Likert scale was created for the statements of the questionnaire, and the respondents gave their responses by choosing between multiple choice question alternatives. The form also included opened questions where freely worded responses could be written. To analyze the results, a table presenting the obtained responses in numbers was created. Some of the responses were analyzed as graphs where the numbers of respondents were given as percentages. The results were interpreted by comparing the percentages with the nurses’ experiences regarding the competence of the persons demonstrating their competence after the demonstration process. Based on the responses, the nurses of the Southern home care unit of the City of Helsinki are not fully aware of the tasks related to the competence demonstration process, and nurses evaluated that their counselling competence was quite good. The responses also indicate that nurses need strong skills in counselling and organization, as well as clinical nursing in their work. The criteria for demonstrating competence in nursing procedures and medication stimulate discussion in working communities, particularly owing to the uncertainty related to the process and different styles of receiving demonstrations. The results of this study can be used by the home care of the City of Helsinki to develop the competence demonstration process intended for practical nurses to become clearer for everyone involved. In connection with the development of the demonstration process, providing orientation for nurses on the reception of the competence demonstrations rose to the forefront. When nurses are familiar with their tasks, developing activities realistically and utilizing expertise will also come more naturally for them. Putting effort in orientation and clarifying the competence the demonstration process will come to further enhance the quality of nursing and the implementation of patient safety....

Diabeteksen hoidon opas Attendo Tapalankallion henkilökunnalle

Opinnäytetyöni aiheena oli diabeteksen hoito. Aihetta on tarkasteltu ikäihmisen diabeteksen hoidon näkökulmasta. Työn teoreettinen osuus koostuu ajankohtaisesta ja tutkitusta tiedosta. Teorian lähdeaineistona olen käyttänyt painettuja ja sähköisiä lähteitä.Käytetyt lähteet ovat olleet suomalaisia sekä kansainvälisiä.
Teorian pohjalta olen laatinut oppaan Attendo Tapalankallion hoitohenkilökunnalle. Oppaan tavoitteena on lisätä tietoutta diabeteksen hoidosta ja toimia tietolähteenä hoivakodin perus-, lähi- ja sairaanhoitajille sekä hoitoapulaisille. Oppaassa on avattu minkäläinen sairaus diabetes on, hoidon tavoitteet sekä diabeteksen hoito kokonaisuudessaan. Oppaassa tulee esiin lisäksi diabeteksen hoidon seuranta ja diabeteskomplikaatioiden ennaltaehkäisy käytännön hoitotyötä toteutettaessa. Opas sisältää myös ikäihmisen diabeteksen hoidossa huomioon otettavia erityispiirteitä.
Jokaiselle osastolle annettiin oppaan kirjallinen versio käytettäväksi. Opinnäytetyön yhteistyöhenkilöt antoivat palautteen oppaasta. Opas koettiin Attendo Tapalankallion tarpeita palvelevana, joka tavoitteen mukaisesti toimii hoitohenkilökunnan apuna diabeetikoiden hoitotyössä. Opas koettiin myös monipuoliseksi.
Opinnäytetyön jatkotutkimusaiheena voisi toimia oppaan hyödyllisyyden arviointi. Onko oppaan käyttö ja siihen perehtyminen lisännyt hoitohenkilökunnan tiedollista osaamista diabeteksen hoidosta. Hyödyllisyyden arvioinnissa olisi hyvä pohtia myös onko oppaan teoria yhdistynyt hoitohenkilökunnan toteuttamaan diabeteksen hoitoon käytännössä. Aihetta voisi tarkastella esimerkiksi tutkimuksen muodossa....

This thesis was a part of MORFEUS project. The aim of thesis was produce knowledge to the project about how congregations support child welfare and what kind of cooperation there is between congregation and child welfare. The purpose of MORFEUS project is to survey, among other things, cooperation network between different operators such as public sector and others social and healthcare operators. The aim is to develop form of cooperation between these operators. The thesis is comprised of a theory and literature review. The key notions in the thesis were multi-professional cooperation, church, welfare work, child welfare and child welfare work. On this thesis, the church means Evangelical Lutheran Congregation. Descriptive literature review was used as the research method. Information retrieval was conduct systematically using several databases. Five Master´s theses were selected for the thesis. Information retrieval showed that there is only little research made on this topic. The results showed that cooperation between congregation and child welfare has not been researched directly. The achieved information shows that cooperation between congregation and child welfare does exist. As a development proposal, it would be valuable for further research to survey the actual cooperation between congregation and child welfare....

Ergonomiset potilassiirrot-koulutuskokonaisuus sairaanhoitajaopiskelijoille

Tämän opinnäytetyön tavoitteena oli kehittää ergonomian opetusta hoitotyön oppimisympäristössä Laurea-ammattikorkeakoulussa. Opinnäytetyön tarkoituksena oli suunnitella ja toteuttaa potilassiirtoergonomiakoulutus ensimmäisen vuoden sairaanhoitajaopiskelijoille.
Teoriaperustassa käsiteltiin muun muassa ergonomiaa, ergonomisia potilassiirtoja ja ergonomian opetusta. Opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä, jossa hyödynnettiin laadullista tutkimusmenetelmää. Aineistonkeruumenetelmänä käytettiin palaute-kyselylomaketta, johon pyydettiin vastausta 15 opiskelijalta.
Opinnäytetyön tuotoksena tehtiin kuvasarja ergonomisista työasennoista, potilassiirroista ja apuvälineistä hoitotyön oppimisympäristöön postereiden muodossa. Postereiden tarkoituksena oli selkeyttää ergonomian oppimista ja mahdollistaa aiheen itseopiskelun. Postereita tehtiin yhteensä 15 kappaletta ja niitä käytettiin potilassiirtoergonomiakoulutuksessa yhtenä opetusmenetelmänä. Potilassiirtoergonomiakoulutukseen tehtiin lisäksi luentomateriaali tämän opinnäytetyön teoriaperustan pohjalta.
Palautekyselylomakkeiden tulosten perusteella potilassiirtoergonomiakoulutus koettiin erittäin hyödyllisenä opiskelijoiden näkökulmasta. Potilassiirtoergonomiakoulutus toi opiskelijoille paljon uutta tietoa siirtomenetelmistä, apuvälineistä ja ergonomian tärkeydestä hoitotyössä. Ensi kertaa psykiatriseen erikoissairaanhoitoon tulevan psykoosipotilaan ennakkotiedot sairaudesta ja hoidosta sekä odotukset hoitoa kohtaan.

Tässä opinnäytetyössä tarkoituksena oli selvittää ensi kertaa psykiatriseen erikoissairaanhoitoon tulevan psykoosipotilaan ennakkotietoja sairaudesta ja hoidosta sekä odotuksista hoitoa kohtaan. Tutkimuksesta saatuja tietoja hyödynnetään JMT kuntoutuspoliklinikalla. Teoreettinen viitekehys perustuu tutkittuun tietoon potilaslähtöisen hoidon merkityksestä. Saamalla tietoa uusien potilaiden ennakkokäsityksistä ja odotuksista, voidaan heidän hoitoaan muokata potilaslähtöisemmäksi.
Opinnäytetyö toteutettiin laadullisella tutkimusmenetelmällä ja tiedonkeruussa käytettiin teemahaastattelua, joka jaettiin kolmeen eri teemaan. Tutkimuksessa haastateltiin kolmea psykiatrisessa erikoissairaanhoidossa olevaa potilasta. Potilaat jotka valikoituivat haastatteluun, olivat hoidettavana JMT kuntoutuspoliklinikoilla. Tutkimus oli tarkoitus rajata koskemaan uusia psykoosipotilaita, jotka eivät ole ennen olleen erikoissairaanhoidon piirissä, mutta rajallisen ajan ja vähien potilasmäärien takia, haastatteluun valikoitui jo pidemmän aikaan hoidossa olevia potilaita.
Haastatteluista saadut tulokset analysoitiin laadulliselle tutkimukselle sopivalla lähestymistavalla. Haastattelusta saatuja tietoja vertailtiin keskenään ja tutkittiin, kuinka paljon vaikutusta taustatekijöillä on potilaiden ennakkotietoihin psykiatrisista sairauksista. Vertailussa ovat potilaan ikä, koulutustausta ja perheen mahdollinen aiempi psykiatrinen sairastelu.
Tuloksista perusteella selvisi, että haastateltavilla oli vähän ennakkotietoja psykiatrista sairauksista. Haastateltavat pitivät tärkeänä tiedon saamista sairaudestaan ja sen hoidosta. Koulutuksen tuomaa muutosta tiedon määrään ei pystynyt tutkimuksissa selvittämään, koska kaikki potilaat olivat käyneet saman asteisen koulutuksen. Iäkkäämmillä haastateltavilla oli enemmän ennakkotietoa psykiatrisista sairauksista elämän tuoman kokemuksen myötä. Myös suvussa olleella psykiatrisella sairastelulla oli vaikutusta potilaiden ennakkotietoihin ja odo-tuksiin hoitoa kohtaan. Nämä suhtautuivat positiivisemmin hoitoon ja heillä oli paremmat odotukset hoitoa kohtaa.
Psykiatristen potilaiden tietoa koskevia tutkimuksia on toteutettu vähän, ja ala tarvitsisikin enemmän tietoa psykiatristen potilaiden ennakkotiedoista ja odotuksista. Jatkotutkimuksissa, voisi käyttää laajempaa potilas aineistoa. Tällöin myös tulosten vertailu iän, koulutuksen ja perhetaustojen mukaan olisi laadukkaampaa....
