This thesis is part of the Zet-project, that Laurea University of Applied Science has carried out together with the city of Lohja, Hanko and Valaa Oy (formerly known as Meanfish Oy). In the project, young people from Lohja and Hanko develop together with professionals from the game industry a beneficial game that could support young people when managing everyday life and planning their future. The purpose of the thesis is to sort out the youths ability to work and function and the experience of participation and self-efficacy both by the young people themselves and those working with the young people in the area of Hanko. The purpose was to produce information about the youths’ ability to work and function and factors affecting them, and these factors could be used in the future when planning and developing support and service functions for young people. The core themes of the theoretical framework were juvenile development challenges, management of everyday life, a young adult as a member of society, as well as self-efficacy and participatory experience. In addition, functional capabilities and its dimensions, factors affecting the ability to work, as well as maintaining and improving work and functional performance from the point of view of young adults. The thesis was a qualitative research and the material used was gathered from interviews as well as survey forms. The interviews and survey forms consisted of four themes: everyday life management, functional performance, work ability and from the matters that support these. The interviews were participated by six young adults and the surveys were answered by four employees that work with juvenile related work. Material-based content analysis was used to analyze the research results. According to the results, young adults reacted positively to themselves and to their future. In the field of everyday life management there were shortcomings in long-term planning of independent actions and commitment to goals. In terms of functional performance, young adults experienced lack of social skills and environmental reasons as their weakest areas. Work ability was also considered to be affected mostly from self-sufficient, environmental reasons such as lack of regional job opportunities and educational institutes. According to the research, both young adults and people working in the field considered that the most important factors in supporting work ability and functional performance are sufficient guidance, assistance and encouragement on every aspect of life and at an adequately early stage. Relating to the information gained from the research, it was found that there are a variety of services and supportive activities aimed at young adults, but more resources could be allocated to basic wellbeing promoting work and on developing the services of low-threshold, as well as on provision of information. There should be more uniformity amid all these services so that individual support and guidance would be possible to gain from a single service point without disunity....

Aivokasvainpotilaan kortikosteroidihoito - Kirjallisuuskatsaus hoitotyön näkökulmasta

Opinnäytetyön tarkoituksena on kuvata kortikosteroidihoidon vaikutusta aivokasvainpotilaaseen. Tavoitteena on tuottaa neurokirurgian klinikan sairaanhoitajille kortikosteroidihoidon toteutukseen liittyvää materiaalia. Opinnäytetyöprojekti tehtiin osana Helsingin ja Uudenmaan Sairaanhoitopiirin (HUS) ja Laurea-ammattikorkeakoulun laadunkehittämishanketta. Projektin lähtökohtana oli Helsingin yliopistollisen keskussairaalan (HYKS) Neurokirurgian klinikan tarve saada aivokeskukseen materiaalia, jota voidaan hyödyntää uusien työntekijöiden ja opiskelijoiden perehdyttämisessä sekä auttaa neurokirurgian klinikalla työskenteleviä sairaanhoitajia pitämään yllä osaamistaan kortikosteroidihoidosta aivokasvainpotilaalla ja kehittämään sitä edelleen.
Opinnäytetyö toteutettiin kuvailevan kirjallisuuskatsauksen metodologian mukaisesti. Aineistoa haettiin eri tietokannoista kuten Laurea-Finna, CINAHL, ProQuest, Medi, Pubmed Google ja Google scholar. Hyödynsimme myös Helsingin ja Uudenmaan sairaanhoitopiirin omaa materiaalia aiheesta sekä ammattikirjallisuutta. Aineiston keruuta varten oli sisäänotto- ja poissulkukriteerit. Tutkimusaineistoon kelpuutettiin aikavälillä 2006-2016 julkaistut artikkelit ja tieteelliset julkaisut. Teoreettisessa viitekehyksessä esitellään kortikosteroidihoidon käyttötarkoitus, vaikutusmekanismi ja haittavaikutukset aivokasvain potilaalla. Lisäksi käydään läpi aivojen rakenne ja toiminta, yleisimmät aivokasvaimet ja niiden hoito sekä pahanlaatuisuus-luokitus.
Sairaanhoitajan tehtävänä on seurata ja ohjata potilasta sekä läheisiä kortikosteroidihoidosta tulevien haittavaikutuksien varalta, jotta ne havaitaan ja niihin voidaan puuttua hoidon varhaisessa vaiheessa. Tämän vuoksi aivokasvainpotilaan kortikosteroidihoitoon osallistujilla tulee olla tietämys aivojen rakenteesta ja toiminnasta, aivokasvaimista, kortikosteroidien käyttötarkoituksesta, vaikutusmekanismista sekä haittavaikutuksista.
Hoitotyön näkökulmasta kehittämistä tulisi lisätä ohjauksen keinoihin ja ohjausmateriaaliin.
Jatkoehdotuksena onkin kortikosteroidihoidon tietämyksen lisääminen esimerkiksi tuottamalla info- tai opetuspaketti hoitoon osallistuville sekä julkinen potilasopas....

The purpose of this thesis was to plan, execute and evaluate a Teddy health clinic for children 3-5- years of age. The teddy health clinic was organized at Iso Omena shopping center as a part of an event for families with children. The collaborators of the functional part of the thesis were Iso Omena’s library and child health clinic. The thesis is a pilot between Laurea University of Applied Sciences and Iso Omena’s child health clinic in the Teddy health clinic project. The Teddy health clinic is meant to help children learn about the content of a child health clinic practice through play and to give them a positive experience of a health clinic visit. When a child can bring his/her own soft toy, and participate in the procedures the content of a child health clinic visit will be more concrete. In this way children can process something they might possibly be nervous about trough play. It is important that a public health nurse can guide and communicate in a positive and fluent way to reach the goals of the practice. The thesis deals with child health clinic’s fixed-term check-ups and their content and the stages of the development of children aged 3-5 years. It also reviews guidance as a part of a public health nurse´s duties and preparing a child for examinations and procedures trough playing. The practical side of the thesis is implemented in the Teddy health clinic. Because a child learns and processes new things trough play, this can be used in getting a child to know about a child health clinic. The Teddy health clinic follows the procedure in fixed-term check-ups, trough play. Therefore, a child can learn the content in a way that feels safe. For the purpose of observing the benefits of the Teddy health clinic, there were feedback forms for parents to fill in. These forms evaluate how well the goals of the project have had been fulfilled. The observation of the children in this thesis was based on the review that the children did with VAS -scale and of course their spoken feedback. The evaluation is based also on the children´s drawings made on their visit to the Teddy health clinic. Three children aged 4-5 were interviewed. The Teddy health clinic was a success. The children took an interest in it and they were pleased when they had a chance to participate. The feedback from the children and their parents was exclusively positive and there were wishes that the practice would continue in the future. It was a nice and meaningful way of getting the children to know about the content of a child health clinic visit and to help them have a positive experience. The Teddy health clinics could be organized periodically in the future, because there is a demand for them. They could also be carried out in day care centers and afternoon clubs to reach even more children....

Terveyden edistämisen haasteet lastensuojelun erityisyksikössä

Tämän toiminnallisen opinnäytetyön tarkoituksena oli järjestää terveyttä edistävä hyvinvointipäivä lastensuojelun erityisyksikössä asuville nuorille. Tavoitteena oli lisätä nuorten tietoutta omasta terveydestään sekä tietoutta siitä, kuinka he voivat vaikuttaa itse oman terveydentilansa edistämiseen. Kiljavan Majakassa asuvat nuoret käyvät koulua omien voimavarojensa ja psyykkisen vointinsa rajoissa, joten koulun puolesta annettava terveyskasvatus voi osalla nuorista olla puutteellista. Tästä syystä Kiljavan Majakassa koettiin tarvetta nuorten terveyskasvatukseen, jonka pohjalta ajatus tämän tyyppiseen opinnäytetyöhön syntyi. Opinnäytetyön tilaajana oli yksityinen lastensuojelun erityisyksikkö Kiljavan Majakka.
Lapsuudessa ja nuoruudessa opitut tavat ja terveystottumukset, sekä elinolot vaikuttavat myös myöhemmässä elämässä. Myös kehitysympäristössä jossa lapset ja nuoret arkeaan elää, on suuri ero joko erojen tasaajana tai lisääjänä. Vaikka suuri osa nuorista voi hyvin, on eriarvoisuus silti vakava ongelma. Nuorten hyvinvointi on yleisesti parantunut Suomessa 2000-luvulla. Suhteet vanhempiin ovat kehittyneet myönteisellä tavalla. Fyysisiin työoloihin sekä työilmapiiriin liittyvät tekijät ovat parantuneet kouluissa. Nuoret ovat itse kokeneet terveydentilansa paremmaksi. Monet terveystottumukset ovat parantuneet, esimerkiksi tupakointi ja humalajuominen ovat vähentyneet ja liikunnan harrastaminen on lisääntynyt.
Hyvinvointipäivä toteutettiin 6.4.2017 Kiljavan Majakassa Nurmijärvellä. Osallistujia oli yhteensä kahdeksan, iältään 15-17-vuotiaita. Nuoret jaettiin pienryhmiin ohjaamisen helpottamiseksi. Hyvinvointipäivän toimintapisteitä oli yhteensä neljä ja toimintapisteiden kesto oli 20 minuuttia. Jokainen ryhmä kiersi vuorollaan jokaisen toimintapisteen. Toimintapisteiden aiheina olivat liikunta, psyykkinen hyvinvointi, seksuaalisuus ja lisääntymisterveys sekä ravitsemus.
Hyvinvointipäivän onnistumista arvioin kyselylomakkeella, jotka täytettiin osallistujien kanssa hyvinvointipäivän päätteeksi. Myös osa henkilökunnasta vastasi kyselyyn. Arviointilomakkeet nuorille ja henkilökunnalle poikkesivat hieman toisistaan. Kyselylomake oli kvantitatiivinen eli määrällinen. Kyselytulosten perusteella hyvinvointipäivä oli onnistunut melko hyvin sekä ohjauksen että toimintapisteiden osalta saadun palautteen perusteella. Henkilökunnan suurimmaksi kehittämisehdotukseksi nousi henkilökunnan määrä vastaavanlaisissa tapahtumissa. Nuoret puolestaan toivoivat enemmän ohjausta, joka myös osaltaan viittasi henkilökunnan riittävyyteen. Sekä henkilökunnan, että nuorten toiveena oli, että hyvinvointipäiviä järjestettäisiin jatkossakin ja Kiljavan Majakassa luvattiin järjestää vastavanlaisia tapahtumia myös tulevaisuudessa vaihtuvilla aihealueilla....

The purpose of the thesis is the concept of establishing peer support group. The thesis is one example of doing ground work about establish a peer support group and ultimately establish peer support group. The purpose of the group is to establish peer support group for parents who have ADHD-children or adolescents but the same concept can be adapted to other peer support groups or groups which operate on volunteer people. The thesis is also a review of the fundamental issues and world of experience in which the adolescents live before becoming adults. My own experiences about parenthood of ADHD-adolecent and heard need and the fact that Riihimäki has not this kind of group, are the inspiration of this thesis. The theoretic context of this thesis is based on finnish Käypä hoito -recommendations, the expertise of ADHD-liitto, the professional articles and the purpose of service design. In the body of this thesis has been used adapted service design method and it suits many kinds of need and action. With service design method can be taylored the concept to meet clients needs and it can be modified at any stage. The thesis was actualized partly functional. All the practical things were done for starting peer support group, but the group didn’t start yet so my thesis is a literary concept. Creating the concept was done in cooperation with the subscriber so they were able to impact on the contence of the concept. The developing challenge in doing the thesis was minimal participent of authorities in cooperation with the third sector, in this case with parish. The advertising of the peer support group didn’t reach all potential participants because in schools and in social services parents were not informed in desired way inspite of the advertisements had been delivered in those offices to distribute or inform. To be sure that the advertising is effective and it reaches right persons before starting peer support group we must develop cooperation with authorities so that they instruct potential participants in to peer support group, witch is ”low threshold” form of service. Establishing peer support group must be taking care of many practical issues and must be able to be prepared of changing things with plan B and to be able to adapt dif-ferent situations.
