Criteria of good care for the premature infant family The purpose of the thesis was to define the criteria of good care for premature infants’ families through a literature review. The goal of the thesis was to describe what premature infants’ good care is, so that the care could be harmonised and it could be developed to become better. Good care was examined from the premature infant’s and family’s point of view. The concept ‘premature infant’, which means a baby born too early, is used in the thesis. The thesis was executed in co-operation with the HUCH Neonatal intensive care unit K7. Co-operation was also made with the neonatal ward L2 in Jorvi and with the antenatal ward and children’s ward of the Women’s Hospital. The thesis was carried out as a literature review in which systematic literature review’s principles were applied. The information was searched from the school’s library, Nelli-portal and Laurus-database. The search was limited to the good care of a premature infant and family. Index words were premature infant, newborn, family, care, neonatal intensive care unit and neonatology. The index words were cut, so that the search results would be diverse. The search results, whose title, abstract or the whole text did not match the research questions, were excluded. In analysing the data an inductive, in other words a data-based, content analysis was used. The inductive content analysis was directed by the purpose of the thesis and the research questions. The analysis process proceeded phase by phase from simplifying the original statements to classifying them, from which it proceeded to upper and lower classification and eventually to the main divisions. The six criteria of the premature infant's good care formed through the analysis: 1) The premature infant's nursing environment and daily care, 2) Support of mental development, 3) The significance of the family in premature infant's care, 4) The significance of interaction and skin contact, 5) The responsibilities of the nursing staff in premature infant's care and 6) The ethicality of premature infant’s care and transition to the follow-up care. The results show that good care is not restricted just to the physical good nursing care of the premature infant, but the care has to be comprehensive. It also is seen from the results how big significance the parents’ presence and including them in the care has in the growth to parenthood and, above all, in the premature infant's development. The professional skills of the nursing staff and multiprofessional co-operation play also a significant part in good care. The results of the thesis can be used in the premature infant’s care at the neonatal intensive care unit. The criteria of good care can be used to guarantee the best possible care and nursing environment to the premature infant and his or her family. Laurea University of Applied Sciences Abstract Laurea Lohja Health Care Nursing Päivi Lokkila and Memarisara Shawbo Drug counsellors’ perception of substance abusers social exclusion Year 2013 Pages 60 The purpose of this thesis was to survey drug counsellors’ perception of substance abusers social exclusion in their own work. It was also pointed out how different municipalities’ substance abuse counselling services differ from each other. The municipality of Lohja substance abuse counselling services was compared to other municipalities nearby and also how the substance abusers exclusion occurs in them.The objective was to clarify substance abusers social exclusion in the society and the difference of the services offered in different municipalities. This thesis is associated with interregional Pumppu project which aims to find ways to support the path of welfare. Laurea University of Applied Sciences, Southeast Finland’s Social sector area Centre Oy Socom, the city of Hämeenlinna, Turku University of Applied sciences and University of Turku carry out this project. A qualitative method was used in the thesis. The data was collecte abuse workers at one private substance rehabilitation in Länsi-Uusimaa. Interviewers were mainly nurses and practical nurses. The interviews were carried out between October to January and they were carried out as single and pair interviews. Theme interviews were chosen as the interview method. The interview results were analyzed by themes by content analysis. The results showed that the substance abusers belonged to a group of people who were socially excluded due to substance abuse which diminishes relationships and the persons exclude themselves from the activities of the society. The social exclusion and risk factors for substance abuse were the same, which are situated in different times within one’s life cycle. Methods to prevent substance abusers social exclusion from society were to intervene in early childhood problems, to organize cafes for young people, to have proactive substance abuse services, to organize services easy to access and to add study and work opportunities. The interviewees saw that in the bigger municipalities there were more organized substance abuse counseling services than in the smaller municipalities. The Lohja Municipality organized substance abuse councelling services which were considered limited compared to its population and to the population of the nearby municipalities. In Lohja it was experienced that substance abuse counselling services were missing a recovery house and third sector services that caused wishes for more substance abuse counseling services. In order to get into rehabilitation depended on the following factors: the municipality’s budget, the financial situation of the municipality, the way of using money, the staffs professional way of handling workers, the staffs attitude, the patient’s own motivation to have treatment, long queues to substance abuse counseling services, the substance abusers intoxicant use history and the background of the family. The results of this thesis are similar to other studies which were carried out earlier and related to the same subject and theory. On the basis of the views of the interviewees, it can be said that the municipality of Lohja should arrange more substance abuse counseling services. In the future the subject of this thesis could be developed by studying other Municipalities substance abuse services and how the services differ from each other. Syöpää sairastavien potilaiden elämänlaatu virushoitokokeilun aikana

Opinnäytetyön tarkoituksena on kuvata syöpää sairastavien potilaiden kokemaa elämänlaatua ja sen muuttumista virushoitokokeilun aikana. Opinnäytetyön tavoitteena on pohtia virushoidon merkitystä potilaiden kokemaan elämänlaatuun ja tuottaa tietoa potilaiden kokemasta elämänlaadusta kokeellisista virushoidoista vastaavien Oncos Therapeutics Ltd:n ja Docrates-sairaalan käyttöön.
Syöpä on nopeasti yleistyvä sairaus. Suomessa elää yli 200 000 ihmistä, jotka ovat saaneet syöpädiagnoosin. Vuosittain syöpään kuolee noin 10 000 ihmistä. Miesten yleisin syöpä on eturauhassyöpä ja naisten rintasyöpä. Syöpään on olemassa monia erilaisia hoitomuotoja. Onkolyyttiset virushoidot ovat uusi, kokeellinen hoitomuoto, jossa virukset tuhoavat jakaantuessaan pelkkiä syöpäsoluja, jolloin terveet solut eivät vahingoitu.
Elämänlaatu on moniulotteinen käsite, jolle ei ole olemassa yhtä määritelmää. Yksilön kokemukseen omasta elämänlaadustaan vaikuttavat monet eri tekijät. Syöpään sairastuminen ja syöpähoidot vaikuttavat elämänlaadun kaikkiin osa-alueisiin monella tapaa.
Opinnäytetyön aineisto kerättiin Helsingissä yksityisen syöpäsairaalan, Docratesin 33 potilaalta. Aineisto kerättiin kyselylomakkeella, joka perustuu kansainväliseen EORTC QLQ-C30 – elämänlaatumittariin. Mittari on kehitetty syöpäpotilaiden elämänlaadun mittaamiseen ja elämänlaadun osa-alueina ovat yleinen elämänlaatu, toimintakyky ja oireiden esiintyminen. Potilaat, jotka osallistuivat kokeellisiin virushoitoihin, sairastivat pitkälle edennyttä syöpää. Hoitojakson aikana potilaat vastasivat jokaisen käyntikerran yhteydessä elämänlaatua koskevan kyselylomakkeen kysymyksiin. Potilaiden anonymiteetin takasi se, että sairaalan henkilöstö oli koodannut potilaiden vastaukset kyselylomakkeesta havaintomatriisiksi opinnäytetyötä varten. Täten potilaiden henkilöllisyys pysyi koko opinnäytetyön prosessin ajan salassa opinnäytetyön tekijöiltä.
Kaikkien potilaiden elämänlaatu keskimäärin koheni virushoitokokeilun aikana. Potilaiden kokemus omasta toimintakyvystään vaihteli suuresti. Potilaiden kokemista oireista uupumus väheni hoitojakson aikana. Syöpään liittyviä yleisiä oireita, kipua ja pahoinvointia, potilailla esiintyi melko vähän koko virushoitokokeilun ajan....

The aim of this thesis is to describe children’s experiences of everyday life in a family support center in the capital area. The thesis is a part of a wider local development project. The purpose of this thesis is to provide new information for the employees on how children experience everyday life in a family support center. This thesis also aims to show the children’s participation and activity. The family support center is also interested in the experiences of parents/guardians and employees. This thesis only includes the children’s experiences. It is possible to utilize this thesis later on, for comparing experiences between parents/guardians, employees and children. This thesis covers only one part of the big picture in family support center’s aspirations to acquire knowledge of everyday life in the center. The purpose of a family support center is to rehabilitate families and children. The starting point is to support the children’s growth and development together with their parents. The family support center offers support and help in everyday life in families’ homes, in the family support center or in both of them. The family support center organizes, for example, occupational therapy, group meetings, consultations and personal meetings. The family support center is a part of community care so I have discussed the family support center´s services and action in the theoretical part. I have also discussed the help to families. One section of the theory deals with children’s participation because the aim of this thesis is to increase it. In this thesis I used qualitative research method. The interviews were carried out with semi-structured theme interviews individually. A semi-structured theme interview is a very flexible research method. I also used the “My life’s important thing” –cards by Pesäpuu ry. Using cards during the interview was voluntary. The analysis method used was inductive content analysis. The interviewees were all boys, ages varied from 9 to 12 years. There were 4 interviewees. All the invited individuals participated in the interviews. According to the findings, the meaningful things to the interviewees in the everyday life were playtime, social activities, spending time together, conversations with employees, family and friends. Even though the interviewees experienced that being in the family support center is sometimes boring and there is nothing pleasant in it, they still named pleasant things when asked. All answers in the interviews were similar....

Patient guides in plain language for patients with type 2 diabetes Our thesis is a part of Pumppu project, which had perusturvakuntayhtymä Karviainen as a cooperation partner. Interregional Pumppu project is financed by the Regional Council of Päijät- Häme and the EU unit of the regions of Southern Finland. Laurea University of Applied Sciences is one of the co-organisers of Pumppu project. One target group for the Laurea subproject are diabetes patients, which this thesis relates to. The objective of the Pumppu project is to develop services in a way that customers and service providers meet. The aim is to support patients in their care path to new services, thus resulting in a seamless welfare path. This thesis was assigned by perusturvakuntayhtymä Karviainen, which is a primary health and social care consortium in the town of Karkkila and the municipality of Vihti in the western Uusimaa. The intent of this thesis was to develop plain language patient information for diabetes patients. The information was developed in cooperation with the locality’s diabetes nurses. The target group became more precise during the interviews – middle-aged and older type 2 diabetes patients. The diabetes nurses had written material in place for group counselling sessions, but did not have handouts from the counselling topics to share with the patients. The objective for the development work was to enable the diabetes nurses to provide better diabetes guidance to their patients using the two patient information guides we produced. The requirements set for the patient information guides were plain language and functionality, in view that they would be easy to introduce. Dissemination was used as the development method. This multi-method assessment process enabled us to process tacit knowledge and to create new knowledge. The first phase of the thesis surveyed the requirements of diabetes nurses in connection with the patient counselling they gave. Based on the survey, we began working on patient information guides for two of the most important subjects. As a result of the development process, we created instructions on important issues that should be taken into consideration with diabetes, as well as the importance of diet and exercise in diabetes management. It can be concluded that the development work has genuinely managed to improve the work life and the final results have been seen as beneficial for the work. The patient guides will in future be used to support diabetes related group counselling. Implementing hand hygiene in a medication room on the ward This thesis consists of two separate parts: a theoretical framework and an observational study. Hand hygiene now requires greater attention, since the single most significant way to prevent the spread of infection is through a proper implementation of hand hygiene. When handling medicines it is essential to carry out aseptic precautions in order to avoid exposure to drug ingredients and to prevent contaminating the pharmaceuticals. This study was carried out as a project in cooperation with KiiltoClean Ltd, Laurea University of Applied Sciences, and a cooperation partner ward in the Pirkanmaa Hospital District. The purpose of the project report was to describe the aseptic procedures carried out by the nursing staff in the medication room of the cooperation partner ward. The aim was to use the evidence gained to provide additional information on hand hygiene and its implementation in the medication room on the ward. The project report covered 22.5 hours of observing the aseptic procedures carried out by the nursing staff in the medication room. The data was collected through observations. To analyse the data the authors applied a structured observation matrix, combining research-based and empirical evidence, covering 20 functions related to aseptic procedures. Correct working procedures promote good implementation of hand hygiene. Good hand hygiene, standard precautions, appropriate tools for the administration of medication, and professional development, help in the implementation of aseptic procedures. Implementing hand hygiene in a medication room on the ward This thesis consists of two separate parts: a theoretical framework and an observational study. Hand hygiene now requires greater attention, since the single most significant way to prevent the spread of infection is through a proper implementation of hand hygiene. When handling medicines it is essential to carry out aseptic precautions in order to avoid exposure to drug ingredients and to prevent contaminating the pharmaceuticals. This study was carried out as a project in cooperation with KiiltoClean Ltd, Laurea University of Applied Sciences, and a cooperation partner ward in the Pirkanmaa Hospital District. The purpose of the project report was to describe the aseptic procedures carried out by the nursing staff in the medication room of the cooperation partner ward. The aim was to use the evidence gained to provide additional information on hand hygiene and its implementation in the medication room on the ward. The project report covered 22.5 hours of observing the aseptic procedures carried out by the nursing staff in the medication room. The data was collected through observations. To analyse the data the authors applied a structured observation matrix, combining research-based and empirical evidence, covering 20 functions related to aseptic procedures. NOPS 2010-2012, promotion of work ability and functional capability among young people in vocational collages : Hazard analysis for Hyrias precision mechanics This thesis was implemented as part of NOPS 2010-2012 project (works for young ability to work and function). NOPS 2010-2012 the purpose of which was to encourage the young to work and function during their study period. The project main implementer was Laurea University of applied sciences. The project partners were HYRIA education Oy, keski-uudenmaan education Keuda, Hyvinkää, the City of Järvenpää, the municipality of mäntsälä and nurmijärvi Study of health maintenance. NOPS 2010-2012 project focused on the high school students. Project was funded by the Ministry of sociality affairs and health. The purpose of this study was to determine, how well the Hyria precision mechanic students recognized risk factors in their work environment or their studying environment. The method used was quantitative. The data was collected by using a structured questionnaire. In addition, nine students participated in a thematic interview followed by a reflection discussion. The questionnaires were analyzed by using the SPSS -statistic program. The analysis of the interviews and reflection was based on themes. Key results showed that the precision-mechanic students recognized the risks factors at their working environment and in their working methods. Most identified working environment hazards were such as, slipping, tripping, and lack of protective equipment, as well their own operational personal issues such as, substance abuse and inappropriate treatment. The students experienced that the main cause of the accidents was their own actions and failures. The school taught the students to avoid accidents at work.
