The purpose of this thesis was to compare the use of time nurses spend on taking the blood pressure, using three different measurement methods. The thesis was a project involving Laurea University of Applied Sciences and Hospital District of Helsinki and Uusimaa and Meilahti Triangle Hospital. The thesis was made on the initiative of Meilahti Triangle Hospital ward 6B. The thesis objective was to develop nurse’s use of methods when taking the blood pressure. The study was carried out as a time research. The research method used was quantitative and the observation method was systematic observation. Ward 6B was preliminarily testing a new electronic recording system. This thesis compared the new electronic registration system with the two methods already in use in the ward. Observation was conducted by a registered nurse working in the ward. She took the blood pressure using three different methods and recorded the results on an observation form. The measurements were divided into indirect and direct nursing care, which were 1) time used for preparation 2) time used for measurement 3) time used for recording. The results were analyzed using SPSS 16.0 program. The study results revealed that the electronic recording system for the measurement of time that was piloted was the fastest method of taking the blood pressure. The method of taking the blood pressure was an automatic gauge and recording the result with a portable electronic recording system. The second fastest method was taking the blood pressure with an automatic gauge and recording the results in the ward office. The slowest method was taking the blood pressure with an automatic gauge and recording the result using a laptop. When comparing the distribution of time spent on direct and indirect nursing between these three blood pressure taking methods, the amount of time spent on direct nursing was significantly less using the first method than with the other two methods. Instead, the share of direct nursing care was almost the same for all measurement practices. The study raised issues for further research. Utilizing wireless network technology in a nurse's work has not really been investigated. In addition, nursing staff’s attitudes to the use of technology in nursing have not been studied. Based on this thesis and its conclusions ward 6B can develop nurses’ use of time to a more efficient direction, because the recording system previously used was not the most effective in terms of the use of time....

Family Nursing Care on Neonatal Wards, a Survey for Parents The purpose of this thesis is to study the family care experiences of parents whose newborn babies have been nursed on neonatal wards and find out what proposals for development these parents have for family nursing care. This thesis is part of a wider project of family nursing development in neonatology during the period of the years 2011-2012. Development of family nursing care is part of the essential strategy in neonatology, and, if necessary, a new kind of family nursing model may be developed through it. The aim of this study is to get an idea of the realization of the family nursing care on neonatal wards described by parents. A further aim is to find out how to increase the satisfaction of the families with the care and how to make families feel themselves more secure. The material of this thesis consists of four different neonatal wards (Children's hospital Lastenklinikka department K7, Gynaecology hospitals Naistenklinikka department N7 and Kätilöopisto department V37 and the hospital of Jorvi department L2). The data was collected through self made questionnaires addressed to the parents whose babies were taken care of on those wards in January 2012. The total of 33 parents of newborns participated in the survey. This thesis is a quantitative study. The data were analyzed using the SPSS software. The results indicate that the parents have successfully been able to take part in the child care; such as feeding, diaper changing , and kangaroo care. The majority of the parents experienced that they had been able to participate in the baby care sufficiently enough. On the other hand many respondents experienced that they had been able to participate in the planning of the care only a little....

Keskustelutuokio HyvinvointiTV®:ssä teemana itsetunto

Tässä toiminnallisessa opinnäytetyössä, joka on osa Turvallinen Koti -hanketta, pidettiin kaksi tapaamista mielenterveyskuntoutujien kanssa nettiyhteyden välityksellä. Teemana molemmille tapaamisille toimivat hyvä itsetunto sekä sen parantaminen itsetuntemusta kehittämällä. Näiden tekijöiden katsotaan myös tutkimusten valossa vaikuttavan merkittävällä tavalla kuntoutumisprosessiin.
Molempiin tapaamisiin osallistui seitsemän molempia sukupuolia edustavaa noin 30-vuotiasta kuntoutujaa. Ensimmäisen tapaamisen aikana kuntoutujien oma-aloitteisesti tuomat aiheet olivat vähäiset ja tapaamista leimasi lievä oppituntimaisuus. Toiseen tapaamiseen mennessä keskinäinen suhde kuntoutujiin oli kehittynyt niin, että vapautuneempi tunnelma sai aikaan spontaanimpia reaktioita, mikä auttoi tuomaan esille uusia näkökulmia käsiteltäviä aiheita kohtaan.
Pidetyt tapaamiset toivat esille, kuinka tärkeää yhteisön tuki kuntoutujille on ja kuinka pitkäkantoiset seuraukset, varsinkin koulukiusaamisella, itsetunnon kehittymiseen voi olla. Yleisesti ottaen voi sanoa, että kuntoutujat pystyivät hyvin reflektoimaan tärkeäksi kokemiaan asioita. Myös palaute keskusteluista oli positiivinen....

Lähimmäisapu ry:n hoivapalveluasiakkaiden asiakastyytyväisyys

Opinnäytetyön tarkoituksena oli mitata Lähimmäisapu ry:n asiakkaiden asiakastyytyväisyyttä heidän saamiinsa hoivapalveluihin. Opinnäytetyön tutkimuskysymykset etsivät vastauksia seuraaviin kysymyksiin: ovatko Lähimmäisapu ry:n asiakkaat tyytyväisiä saamiinsa palveluihin ja miten Lähimmäisapu ry:n palveluita olisi syytä kehittää tulevaisuudessa.
Asiakastyytyväisyyskyselyn otanta oli suhteellisen pieni (20 vastaajaa). Työ toteutettiin mää-rällisenä tutkimuksena. Tiedonkeruumenetelmänä käytettiin Lähimmäisapu ry:n kanssa yhteistyössä laadittua kyselylomaketta, joka sisälsi sekä monivalinta- että avoimia kysymyksiä. Työssä käytetty aineisto on kerätty 1.11.2011- 31.1.2012. Aineiston keruu toteutettiin kysely-lomakkeen avulla. Opinnäytetyön aineistoa analysoitiin hyödyntäen Microsoft Excel 2007 _taulukkolaskentaohjelmaa.
Opinnäytetyön tulokset osoittavat, että Lähimmäisapu ry:n asiakkaat olivat pääosin tyytyväisiä saamiinsa palveluihin. Kehittämisehdotuksia saatiin vain muutamia. Asiakkaat olivat tyytyväisiä hoitajien ystävällisyyteen ja huomaavaisuuteen. Kehittämiskohteina mainittiin muun muassa se, että hoitajat perehtyisivät enemmän etukäteen asiakkaan taustoihin....

Clinical audit of electronic documentation in a home care unit The aim of the thesis was to analyze the present state of electronic structured documentation with evaluation at one home care unit in southern Finland. The home care unit used Pegasos as their software to document all patient data, as well as the Finnish Care Classification (FinCC) model. The first subject of the evaluation was to find out how nursing processes are realized through the electronic documentation. The second subject of the evaluation was use of the components of the Finnish nursing need (SHTaL) and function (SHToL) classifications. The evaluation of the systematic recording of the nursing formed the frame of reference. The home care unit’s 19 regular customers´ N = 166 (n = 19) nursing plan and the items of the daily documentation were used as the material of the thesis. These documentations were made in the autumn of 2011. The research method was a quantitative study. This suited the study method because in the thesis one wanted exact digital information about the documentation status of the home care unit. The measuring tool used in the research was developed by Kaila and Kuivalainen (2008). Their assessment form had been created in connection with HoiDok project. In the results of the nursing process need of care, objective of care, planned nursing procedures and function of care had been documented on all the customers. The nursing objective had been documented as the customer´s objective in about 32 % of the documentation cases. Outcomes of care had not been documented at all in the right place, but assessment of care had been carried out in 11 % of the cases. The nursing diagnosis had been used in about 20% of nursing documentation’s needs of care. There were no nursing summaries in the evaluated documents during three months. Nursing procedures of daily nursing documentation were focused on describing nurses’ activities. There was a lack of customer´s own subjective health description in the nursing diagnosis of the documentation. The share of the physical nursing needs in the documentation was 98 %. The components of the FinCC classifications had been used as follows: In the Finnish nursing need classification (SHTaL) the needs of the medical treatment had been documented in nearly 34 % of all the entries, nursing needs which are related to the daily nursing functions nearly 21 % and nursing needs which are related to the secretion about 12 % of entries. In the Finnish nursing function classification (SHToL) the needs of medical treatment had been documented in nearly 55 % of all the recordings, nutrition nearly 13 % and secretion nearly 12 %. There were also components which had not been used at all. The results of the evaluation can function as possible subjects of development to the organization and unit of the home care. Use of the nursing diagnosis, systematic documentation of the nursing evaluation and the documentation of nursing summaries formed the central subjects of development. Furthermore, the customer´s subjective health and mood should be seen in the daily nursing documentation. Documentation of the Finnish nursing need classifications and the Finnish nursing function classification includes the psychological and social components, the use of which should be increased by the home care unit. This formed into the second subject of development. HyvinvointiTV® konseptin kehitysmahdollisuudet ikääntyneille Katsaus opiskelijoiden opinnäytetöihin

Suomen väestö ikääntyy kiihtyvällä tahdilla. Samaan aikaan avuntarvitsijoiden määrä lisääntyy ja hoitajien määrä vähenee. Väestön ikärakenteen muutos luo paineen palvelurakenteen ja palvelujen muutokselle. HyvinvointiTV® on teknologinen ja interaktiivinen innovaatio, jonka avulla tähän muuttuvaan palvelun tarpeeseen voidaan vastata. HyvinvointiTV®:n avulla voidaan edistää ikääntyneen fyysistä ja psyykkistä terveyttä.
Tämän opinnäytetyön tarkoituksena oli kartoittaa Ikääntyneille suunnatun HyvinvointiTV®- konseptin tulevaisuuden haasteita ja kehitysmahdollisuuksia KOTIIN- ja Turvallinen Koti –hankkeisiin tehtyjen opinnäytetöiden pohjalta. Opinnäytetöitä oli yhteensä 32 kappaletta, niitä tarkasteltiin sisällönanalyysin keinoin.
Tutkimuksessa nousi esiin neljä eri keinoa, joilla HyvinvointiTV®:tä voi kehittää: tuotteistaminen, sisällön kehittäminen, teknologian kehittäminen, henkilökunnan osaamisen kehittäminen ja asiakasprosessien sujuvuus. Jotta HyvinvointiTV® voisi olla kiinteä osa suomalaisia sosiaali- ja terveyspalveluja, tulee sitä kehittää nyt ja tulevaisuudessa....

A process description of post-operative guidance for a breast cancer patient The thesis was carried out in a collaboration project between the hospital district of Helsinki and Uusimaa, Operative unit at HUCH (Helsinki University Central Hospital) and Laurea University of Applied Sciences. The actual collaboration partner in the thesis was the breast surgical ward in Women's hospital. The purpose of the thesis was to produce a process description of post-operative guidance for breast cancer patients for the breast surgical ward in Women's hospital. The new process description can be utilized not only to develop guidance, but to orientate new staff. Qualitative method was used as a research method. The data was gathered by theme interviews, which were carried out by interviewing a breast cancer nurse and physiotherapists in individual interviews and two ward nurses in a group interview. The interview themes were formed based on the thesis research questions which were: what are the contents of post-operative guidance for breast cancer patients, which are the professionals giving post-operative guidance for breast cancer patients and what guidance methods are used in breast cancer patients' guidance. The data was analyzed by using material-based content analysis. On the basis of the analysis the process description was created using the QPR-Process Guide programme. Based on the analysis of the thesis, breast cancer patients' guidance can be divided into three different stages: determining and preparing the guidance, implementing the guidance and the continuity of the guidance. The professionals that participate in patient guidance are a doctor, a nurse, a breast cancer nurse, a physical therapist, and if necessary, a social worker. The main guidance method used in patients' guidance is oral guidance. Oral guidance is supported by written guidance material, demonstrations and telephone guidance....
