Laws and regulations in elderly people’s medical care - literature review Our thesis is based on literature review. The aim of our thesis is to produce information about Finnish laws and regulations that safeguard elderly people’s medical care. The information gathered can be helpful for nurses and nursing students. The findings of our study are based on laws and regulations in publications by the Ministry of Social Affairs and Health. Accessibility mapping in Porvoo : Nearly hundred objects mapped This thesis was conducted in collaboration with the Disability Council of the City of Porvoo. A total of 23 students from Laurea University of Applied Sciences were involved in doing the accessibility study. It was decided together with the Disability Council to narrow down the number of survey objects, and in the end these totaled nearly 100. All the accessibility study objects were located in downtown Porvoo or in major suburbs. The aim of the accessibility study was to obtain information on accessibility of the physical environment, i.e. how well the object or area served all users. A survey form was compiled for the accessibility study. The form was drawn up in accordance with a guide published by The Finnish Association of People with Physical Disabilities. By means of the survey form, information on each accessibility study object was obtained, e.g. front yards and entrances, lobbies and reception desks, toilets, elevators and stairs. In the form, the answer options were ‘Yes’, ‘No’ and in some cases the questions were open ended allowing the respondents to give their own comments. The results show that the Porvoo public services, including agencies, health and sports facilities, are generally accessible entrance. In particular, the accessibility of health care services is well catered for. None of the mapped pharmacies have free entrance access, and generally their interiors were cramped and difficult to access with assistive devices. As a rule, banks and insurance companies have barrier-free entrances and interiors. Studied hotel is easily accessible. In the restaurants, some of which are in the old town, entrance accessibility is ensured with portable ramps. Most of the restaurants’ interior is roomy enough to move around with assistive devices. Located in the old town of churches and museums are historical buildings, where accessibility is difficult to carry out, so they are not accessible. Studied neighborhoods were Gammelbacka, Kevätkumpu, Näsi, and Tarmola and Huhtinen area. The regions of shops, and shopping centers are generally accessible. Some stores have a high threshold, which complicates the entrance. Induction loop is found only in a couple of mapped objects. The identified sites are reported in a separate attachment. We asked the Disability Council for feedback on our accessibility report. Their view was that we had managed to compile the information clearly in the report. From the very start of the project it was obvious that many interesting potential accessibility study objects had to be excluded. Simple and inexpensive development objective would be to obtain an induction loop, in particular for public services....

The post-operative psychiatric rehabilitation of a hip fracture patient at the medical center bed ward The goal of this thesis was to develop the evaluation of psychiatric rehabilitation after a hip fracture operation for wards 1 and 4 in the Lohja Medical Center. The thesis is part of a project on rehabilitation nursing “the treatment chain for hip fractures in the LOST area” and concerns the patient’s psychiatric rehabilitation. During the project students formed a work group with the representatives of the Lohja Medical Center. The purpose of the work group was to develop a new operating model to help nurses to evaluate patients’ psychiatric rehabilitation after a hip fracture operation. The work is activity-oriented and based on the wishes of the working life. The method used for the development was dissemination, in which the new information is produced for the use by the work community. Theme interviews were used to describe the current situation and to collect data on the evaluation of the new operating method. As a result of the work of the work group, Cornell depression scale and facial expression meter were suggested to be implemented. These tests can be used to help evaluate the mental well-being of the patient and are performed five days after the patient has been assigned to the ward. The results of the tests are then summarized and, if there is any decline in the patient’s psychiatric condition, the doctor decides on further treatment. The usability of the new operating model in the work community was evaluated through interviews and by conducting a SWOT analysis. The new methods for mental evaluation were considered challenging. With the new development method, hip fracture patient’s mental recovery can be evaluated by using these meters. The work community will continue to evaluate the patient’s psychiatric well-being during the rehabilitation. The patient’s psychiatric status will be entered in greater detail into the treatment plan. In the future, research can be continued by testing of different depression meters in order to find the most suitable method for their use....

Perheen merkitys vastasyntyneen hoidossa teho-osastolla

Perheen hoitoon osallistumista vastasyntyneiden teho-osastolla ei ole aina pidetty tärkeänä. Nykypäivänä perheen läsnäolon merkitys teho-osastoilla on tiedossa. Perheiden läsnäoloa koskevat rajoitukset ovat hiljalleen poistumassa. Opinnäytetyön tarkoituksena on selvittää perheen läsnäolon merkitys vastasyntyneiden teho-osastolla sekä selvittää erilaisia perhehoitotyön käytänteitä vastasyntyneiden teho-osastoilla Euroopassa. Tavoitteena on koota yhteen tutkittua tietoa.
Opinnäytetyö toteutettiin systemaattista kirjallisuuskatsausta mukaillen. Tutkimuksia haettiin eri tieteellisistä tietokannoista rajaten aineisto Euroopassa tehtyihin tutkimuksiin tai artikkeleihin. Lopulliseen analyysiin valittiin 9 artikkelia ja 4 kokonaista tutkimusta, joista suurin osa oli englanninkielisiä. Tutkimukset käsittelivät vastasyntyneiden teho-osastoympäristöä, varhaista vuorovaikutusta sekä teho-osastoilla käytettäviä perhehoitotyön malleja. Aineiston analyysimenetelmänä käytettiin sisällönanalyysiä.
Opinnäytetyössä tulokset jakaantuivat kahden tutkimuskysymyksen alle. Toisella kysymyksellä haluttiin selvittää perheen läsnäolon merkitystä teho-osastolla ja toisella kysymyksellä pyrittiin löytämään käytänteitä vastasyntyneiden teho-osastolla toteutettavasta perhehoitotyöstä. Perheen läsnäolo vaikuttaa positiivisesti hoidon laatuun, lapsen vointiin ja kehitykseen sekä edistää vanhemmuuteen kasvua. Vastasyntyneiden teho-osastolla toteutettavasta perhehoitotyöstä on olemassa seuraavanlaisia käytänteitä: perheystävällinen ympäristö, vanhemmat hoitajana, perheystävällinen ympäristö, jossa vanhemmat hoitajana sekä vanhemmat vierailijana osastolla.
Perheen läsnäolon positiivisen merkityksen johdosta perhekeskeisen toimintamallin käyttö on perusteltua. Vanhempien jatkuvan läsnäolon mahdollistaminen on suositeltavaa. Suomessa olisi hyvä lisätä perheystävällisen ympäristön kehittämistä. Jatkossa perhekeskeisen mallin yleistyessä olisi hyvä tutkia esimerkiksi hoitohenkilökunnan kokemuksia perheen jatkuvasta läsnäolosta vastasyntyneiden teho-osastolla sekä miten perheen jatkuva läsnäolo vaikuttaa lapseen ja hänen perheeseensä meillä Suomessa....

Nurses’ experiences of the patient holding experiment at the anesthesia and operating unit of Women’s Hospital This Bachelor’s thesis is a collaboration project between Laurea University of Applied Sciences, the hospital district of Helsinki and Uusimaa and Women's and Children's unit at HUCH (Helsinki University Central Hospital). The thesis is a part of the nursing care quality development project, which aims to develop perioperative nursing and patient holding at the anesthesia and operating unit of Women’s Hospital. The thesis examines the nurses' experiences of the patient holding experiment at the anesthesia and operating unit of Women’s Hospital. In patient holding, anesthetic preparation is performed in a holding area to a patient undergoing a surgery, while another patient is in the operating theatre. The purpose of the thesis is to study how nurses experienced patient holding and how they experienced it in terms of the perioperative nursing care. The target group of the survey is 50 nurses working at the anesthesia and operating unit of Women's Hospital during the patient holding experiment. The research method used was qualitative. The data was gathered through a semi-structured questionnaire. The survey was formed based on the knowledge gained through research and work experience. The data was analyzed using inductive content analysis. The response rate to the survey was 78%. The results show that the majority of the respondents felt that patient holding affected the smoothness of the function of the theatre and speeded up beginning the surgery. Matters that affected these were; that the nurses did not have to wait for the patient in the theatre, patient changes in the theatre were speeded up and anesthetic preparation was not needed to be performed in the theatre. About half of the respondents felt that patient holding reduced anesthetic preparation in the theatre, but also about 50 % felt that holding did not have an impact on the anesthetic preparation to be performed in the theatre. About half of the respondents felt that the flow of information was smooth, but just as many respondents felt that it was not. The process and function of the patient holding brought up confusion and the risk concerning making mistakes in reporting was found increased. The negative things that emerged in patient holding were; the lack of anesthetic preparation performed, inconsistency in anesthetic preparation, poor facilities and short duration of the experiment. In terms of perioperative nursing care the majority of the respondents felt that patient holding does smoothen the transition of patient care, requiring that the anesthetic preparation is performed in the holding area. An important matter was also the need for additional staff, a separate holding area apart from the recovery room and consistency in patient holding. In terms of patient care, patient holding was regarded as a good thing if anesthetic preparations are actually performed in the holding area. Implementation of the exercise events in vocational college, NOPS 2010 -2012, Promotion of work ability and functional capability among young people in vocational colleges This bachelor’s thesis is part of the NOPS project (Promotion of work ability and functional capability among young people in vocational colleges) which has been carried out between 2010-2012. This project is helping to developed methods of promoting health among students within their chosen vocation. NOPS- project’s main organizer is Laurea University of Applied Sciences. The project partners are Hyria and Keuda College in Helsinki, Järvenpää, Nurmijärvi and Mäntsälä. The target group of the project consists of students at the vocational colleges. This project is funded by Ministry of Social Affairs and Health and Laurea Hyvinkää. The purpose of this thesis was to find out that, how NOPS project Laurea’s students organized sports events have affected the vocational college students’ health. The thesis examined also, how health events practical implementation works. The theoretical basis of the thesis formed by: information, motivation and skill - Health Promotion (IBM) model, customer-based guidance and health exercise. NOPS’S project’s questioners were used to obtain and collect data at the beginning and the end of each sports events. The initial questioner was designed to obtain information on the student’s current knowledge on exercise performance, physical activity, health effect and the effects of exercise to the public health/ diseases. The second questioner looked at how the event changed student’s knowledge about physical activity-related information, as well as, how they perceived the health care skills and their motivation after the event. It also explored the effectiveness of the implementation of the sports events. There were seven events which were attended by a total of 70 students’ questioners (Total of 140 questioners’ inc. initial and the final questioners). All the answers were analyzed with SPSS- statistic program and the results were described as frequencies and as a percentage. During the health events, student’s information of sports and exercise were increased. The greatest increase was about information of breast cancer diseases (percentage change +23,4 %), and the information of smoking and physical commonality hazards (percentage change +16,5 %) as well as the information of physical hard work health effects (percentage change +13,5 %). During the events student’s ability chose healther options increased 67,5 %. 70,0 % of students took something positive from the events. 77,1 % of students involved in the experiment were of the opinion that, the events carried out were clearly understood and correct. 63,0 % of students were of the opinion that, the events were sufficiently operational. There were several challenges for the development of the events. Which are; there should be better methods for promoting health and how there can affect the student’s in the college and help them increase their exercise. There must be also some developments in supporting students to take care of ergonomics in their workplace. Students should be educated in the public health/ diseases and their prevention during their studies. The greatest increase was about information of breast cancer diseases (percentage change +23,4 %), and the information of smoking and physical commonality hazards (percentage change +16,5 %) as well as the information of physical hard work health effects (percentage change +13,5 %). During the events student’s ability chose healther options increased 67,5 %. 70,0 % of students took something positive from the events. 77,1 % of students involved in the experiment were of the opinion that, the events carried out were clearly understood and correct. 63,0 % of students were of the opinion that, the events were sufficiently operational. There were several challenges for the development of the events. Which are; there should be better methods for promoting health and how there can affect the student’s in the college and help them increase their exercise. There must be also some developments in supporting students to take care of ergonomics in their workplace. Students should be educated in the public health/ diseases and their prevention during their studies.
