International Evidence on Nursing Management of Chest Trains The purpose of this thesis is to contribute to the generalisation of the common nursing care practices of the chest drain patients in the Uusimaa Hospital District (HUS) and in the future in the whole Finland. The aim of this thesis is to create a literature review on the nursing management of chest tubes to Töölö hospital. The literature review discusses evidence-based knowledge of the subject. A chest tube is a catheter which is inserted in the area called pleura cavity which is between a lung and a chest wall. The chest tube drains air, fluid, blood and pus from the pleura cavity which normally should not be there. The chest tube is connected to a chest drainage system that performs the actual draining of pleura cavity. The chest tube insertion and the nursing care of chest drain patients require certain special skills and the staff should follow aseptic techniques. The conditions which require chest tube insertion can cause death if they are not treated. Also poorly treated conditions can lead to serious consequences. There is not enough evidence based knowledge of chest drain management. Many treatment decisions are based on expert opinions. In particular nursing care concerning chest tube patients is poorly studied. In Finland there are no national guidelines concerning the nursing care of chest drain patients. The lack of national nursing guidelines is seen in other countries too. Also in Uusimaa Hospital District there are no common nursing care practices of chest drain patients. The practices might differ in different units. This thesis is a traditional literature review and more precisely it is an integrative literature review. The search of information sources is done as systematically as possible. Also the selection of information sources has been done by using the principles of systematic review. The databases in information search are provided by Laurea University of Applied Sciences. The number of useful information sources is 27. Eleven of these sources are medical studies. There are also two medical guidelines and one medical consensus. Only one of these literature sources is a study in nursing science. One of the articles was a literature review and oth-er one was a clinical review that concentrated on nursing care. The majority of this thesis information is based on ten nursing care reviews that were found through systematic literature search. This literature review goes through the whole nursing process of a chest tube patient. The thesis focuses on the preparation of the patient and the preparations for the chest tube insertion performed by the nurse and the nurse’s duties during chest drain insertion. Other subjects are the nurse’s duties after the chest tube insertion. Some of these duties concentrate on monitoring chest drainage system and patient monitoring. The nurse’s responsibilities include also the removal of the chest tube. In the future other students have an opportunity to create guidelines of nursing management of chest drains using the results of this thesis and their suitability to practice conditions....

Health behavior of the elderly people over 75 years of age living at home in Korso The purpose of this thesis was to investigate the health behavior of the elderly people over 75 years of age living at home in Korso. The objective of the thesis was to provide the Vantaa senior health clinic with the thesis results for developing their function. The results of the thesis will help the Vantaa senior health clinic to direct their counseling efficiently in the health reception. The thesis answers to such questions as what the nutrition and exercise habits of the elderly people over 75 years of age living at home in Korso are, how much the elderly smoke cigarettes and use alcohol, what kind of social network they have, what kind of health risks elderly people over 75 years of age have and how the health behavior differs between men and women. The thesis was carried out by utilizing a quantitative research method. The material was collected as a mailed inquiry through 19 May 2011 to 22 June 2011 and it was sent to the elderly, who turned 75 years in 2011, living in Korso without help from the home care. The questionnaire was based to The Health Behavior and Health among the Finnish Elderly (EVTK) created by the Finnish National Institute for Health and Welfare and used with customizations to fit the purpose of the thesis. A total of 100 inquiry forms were sent, out of which 58 were returned before the due date. The material was analyzed with PASW statistics software application. The results of this thesis show that 41(71%) of the respondents eat 4-5 meals per day and three meals are eaten by 13 (22%) respondents. 25 (43%) respondents reported to have eaten vegetables or root vegetables, during the latest whole week, on 6-7 days. 24 (41%) respondents reported to have eaten meat daily. Half of the respondents used mostly vegetable fat. 38 (65%) reported to be using vitamin-D products. 23 (40%) respondents reported to be exercising at least half an hour per day. Alcohol had been used by 40 respondents during the last year. 15 respondents use alcohol 1-2 times a month, 12 (21%) do not use every day, but at least once a week and 11 (19%) use less than once a month. Smoking was very rare, because only one of the respondents smoke daily and three other smoke occasionally. Many had quitted smoking after many decades of active smoking. The respondents are mostly in touch with their friends or relatives by meeting them face-to-face or by speaking with them on the phone. The most popular hobby outside the house was visiting home region and relatives by 48 (83%) of the respondents. The second most common hobby was summer cottage and camping by 32 (55%) of the respondents. Most respondents to the inquiry have had their blood pressure and cholesterol measured during the last year. According to the results of the thesis, when an elderly person comes to the appointment, one should pay most attention to the patient’s nutrition, exercise habits, support and musculoskeletal ailments and feeling of loneliness and social relations....

Päiväkirurgisen potilaan ohjaaminen

Päiväkirurgisessa hoitotyössä korostuu potilaan sitoutuminen omaan hoitoonsa, koska sairaalassa oloaika on lyhyt. Tämän seurauksena potilasohjaus on tärkeänä kehittämis- ja tarkastelukohteena päiväkirurgiassa. Opinnäytetyön tarkoituksena on tuoda esille erilaisia ohjausmetodeja aikuisen päiväkirurgisen potilaan ohjauksessa sekä selvittää miten sairaanhoitaja varmistaa potilaan ymmärtäneen ohjauksen.
Teimme opinnäytetyön systemaattisen kirjallisuuskatsauksen mukaisesti käyttäen apuna Medic, Cinahl, Linda ja Nelli -tietokantoja tutkimuksien etsimisessä. Löytämistämme tutkimuksista valitsimme neljä tutkimusta, jotka käsittelivät päiväkirurgisen aikuisen potilaan ohjaamista. Opinnäytetyömme viitekehyksessä käytimme lähteinä monipuolisesti kirjoja, Internetiä sekä Hoitotyön/Hoitotieteen lehtien artikkeleita.
Käytimme tutkimuksien purkamisessa apunamme sisällönanalyysiä ja abstrahoimme ohjaukseen liittyviä käsitteitä taulukoinnin avulla. Tutkimustulokset osoittivat, että monipuoliset ohjausmetodit sekä niiden yhdistäminen ohjaustilanteessa mahdollistavat laadukkaan ohjauksen. Potilaan ja sairaanhoitajan välinen avoin vuorovaikutus edesauttoi muodostamaan miellyttävän ilmapiirin ohjaustilanteessa. Hyvä ilmapiiri tuki potilaan ja sairaanhoitajan välistä luottamuksellista ja tasavertaista hoitosuhdetta.
Opinnäytetyömme tuloksia voi hyödyntää potilasohjauksen kehittämisessä sekä sairaanhoitajan ammatillisen kasvun tukemisessa ohjaustilanteissa. Opinnäytetyömme tulokset korostavat ennen kaikkea sairaanhoitajan ja potilaan välistä vuorovaikutusta, yksilöllistä ohjausta sekä ohjausmetodien yhdistämistä. Lisäksi käytännön harjoittelu, asioiden yhteneväisyys ja selkeys olivat potilaan ohjausta tukevia metodeja. Opinnäytetyömme tuloksia voi hyödyntää myös muilla hoitotyön osaamisalueilla. Potilasohjauksella ja potilaan tukemisella on merkittävä rooli sairaanhoitajan työssä sekä potilaan sitoutumisessa omaan hoitoonsa....

Monitoring and recording of a neurosurgical patient's level of conciousness on the ward This thesis is part of the quality improvement project between Laurea University of Applied Sciences and Hospital district of Helsinki and Uusimaa (HUS), Helsinki Central Hospital (HUCH) for the years 2007-2012. An introduction guide was produced during this project for the development needs of Töölö Hospital neurosurgical ward, detailing and presenting solutions for monitoring the patients’ levels of consciousness. This project was carried out during the years 2010-2012. The participants in this project were two nursing students of Laurea University of Applied Sciences, an instructional teacher and staff members from Töölö hospital. Neurosurgery refers to all surgical operations and procedures performed on the brain or spine. The levels of consciousness are heavily tied to nursing and observing patients who have gone through neurosurgical surgery. The levels are measured with the internationally used Glasgow coma scale (GCS). The most typical illnesses treated with neurosurgical procedures are intra-cranial tumors, brain injuries, cerebrovascular diseases, compression chambers and tumors in the central canal of the spine cord, hydrocephalus, liquorrehea, states of intense pain, motion disorders and epilepsy. The context of this thesis includes following; neurosurgery, neurosurgical nursing, monitoring and recording level of consciousness. The thesis itself also includes the aforementioned subjects in a summarized form that can be used as guidelines in neurosurgical nursing. The guide developed for the neurosurgical ward includes information about the observation and recording of the patients’ level of consciousness in neurosurgical nursing. The emphasis in this project was on gathering information about the theoretical background trough questionnaires concerning to observation and recording of level of consciousness in practice. The information was assembled into suggestions for improvement and an instruction guide to be used in the HUCH hospitals. The function of this introduction guide is to unify and standardize the practice of observing and recording of level of consciousness in neurosurgical nursing. The questionnaires used in the thesis were semi-structured questionnaires and the sampling size was 7. To analyze the questionnaires fenografical analyses principles were used. The research results show that recording the level of consciousness in the ward was considered functional. The instruction guide about the minimum criteria concerning monitoring level of consciousness was built partly based on those results....
