Kipustimulaatio neuropaattisen kivun hoidossa : kirjallisuuskatsaus

The study was conducted using a descriptive literature review and its purpose was to collect research information about chronic pain and especially focus on spinal cord stimulation as a pain management method. The main objective of this literature review was to provide research information about spinal cord stimulation to nursing staff to enhance their competence and to patients considering this treatment for pain management. This literature review focused on neuropathic pain and its drug-free management through spinal cord stimulation. Neuropathic pain is a condition caused by damage or disease affecting the somatosensory system. In Finland, pain stimulation is mostly used for pain management for neuropathic pain, for example, pain after peripheral nerve damage, complex regional pain syndrome, CRPS, or nerve root damage. Spinal cord stimulation is based on electrodes through which stimulation is provided from a pulse generator. The electrodes are surgically placed in the epidural space so they are in the area that can be stimulated to respond to pain relief- Multiple research studies support the use of spinal cord stimulation in managing neuropathic pain. As a result, five subcategories were identified: spinal cord stimulation helps with pain, no serious harm from the stimulation treatment, good monitoring of the patient is important in stimulation therapy, patient selection is important in stimulation therapy, and there is a significant amount of researched information about spinal cord stimulation. It was found that patient selection is an important factor in the success of the treatment, as is proper follow up after the implantation of the stimulation device. The results suggest that spinal cord stimulation is an effective method for managing chronic pain, reducing the need for drugtreatment. The purpose of this thesis was to increase the information of pain assessment among postsurgical patients and their opportunity to be a part of their pain assessment. The aim of this thesis was to produce online material for patients being treated in a postsurgical ward and add patientorientation. The thesis was carried out in cooperation with Hyvinkää hospital’s preoperative unit. The preoperative unit needed a patient material, which explains pain assessment and the pain scales used to assess pain. Postoperative pain assessment is an everyday task for those in the ward and the idea was to emphasize the importance of pain assessment for patients. That is why in this thesis the material created is about pain assessment tools used among patients. The knowledge base of the thesis consists of postoperative pain, good pain management and assessment and the principles of a good guidebook. The thesis was carried out as a narrative review and a deductive content analysis. The research articles were found in Medic- and ProQuest data bases. Seven articles were chosen from those data bases, which were used in a deductive content analysis. As a result of the narrative review, postoperative pain can be assessed with a numeric rating scale (NRS), verbal rating scale (VRS) and with a visual analog scale (VAS). The most used pain scale for pain measurement is NRS. The pain scale NRS should be used on a scale of zero to ten, so the patient is able to express ‘’no pain’’. The use of pain scales in pain assessment is important because pain is a subjective and multidimensional experience and pain scales are made to help pain assessment become more systematic. The assessment of pain is important in preventing prolonged or chronic pain. Recognizing and assessment of pain is a great part of pain management. Assessment of pain should be considered as the fifth vital sign. Based on the results of the narrative review a guidebook, about pain assessment and pain scales was developed. The guidebook considers pain evaluation and different kinds of pain scales, as well as basic knowledge about postoperative pain.
