Determining the Effectiveness of Senior Clinics in Sipoo The thesis was designed to determine the effectiveness of a senior health clinic in Sipoo. The thesis will answer such questions as how effectively the senior clinic functions, how the state of health of over 65-year-old caregivers was monitored and their ability to function preserved in health clinic appointments, and how the appointments supported their state of health and ability to function. Senior clinics act preventively, and the types of activities offered to the elderly stem from available resources and are investigative. The prevention aspect is dictated in the law, in the subsection §12 of legal services for the elderly. It came into effect in 2013 and states that municipalities are to organize health clinic services to the elderly. Such services are to pro-mote their health, functional capacity, and autonomy. The health of the ageing population must be promoted even after retiring from work life with the preventative services. Senior health clinics’ activities include health meetings and group meetings. Health meetings include examining and supporting the client’s state of health and ability to function. The clients’ re-sources received support, and reasons for the decreasing functional capabilities were investi-gated. Group meetings were open to all, and offered health advice on certain subjects. The data for the thesis was collected in the spring of 2013 from health meetings for the care-givers and from three open-to-all group meetings when the health clinic was being piloted. The most vulnerable group, the caregivers, was chosen as the target group. In this thesis, the material consisted of forms filled by the public health nurse during the meetings (n=20) and feedback forms filled by the clients during their health appointments (n=19). The effective-ness of group counseling was evaluated with client feedback, as well (n=34). The health benefits of preventive work become visible over a long period of time. Lifestyle counseling and strengthening the resources of the clients will help support the quality of life during the later years of the ageing population. When comparing the thesis results to the framework of the thesis, the effectiveness of senior counseling and support can be deter-mined. The immediate effects on the clients were the support provided in the health meet-ings, evidence-based redirects, and redirecting the client to further tests. The results showed that the caregivers suffered from sleeping problems, high blood pressure, nutritional deficiencies, and they expressed a need to talk about their own issues. In addition, the need for tetanus vaccines was great. The caregivers considered health appointments ben-eficial and necessary. The mental and social capabilities of the caregivers were supported. When necessary, the caregivers were ushered to seek further treatment if, for example, their blood pressure turned out to be high. According to the results, the health appointments were impressive. Based on the gathered feedback, the caregivers felt they benefitted from the ap-pointments and considered them necessary. The amount of participants ranged between 17 and 20 in the group meetings. The feedback implied that the lectures provided the participants with new and useful information on group guidance. A large proportion of the participants would recommend group guidance to their friends. 6 The senior clinic has continued operating in Sipoo after the piloting period. The thesis results suggest that senior clinic services are necessary and desirable in Sipoo. We recommend that the effectiveness of the preventive service activities should also be evaluated for a longer period of time. In the future, cooperation between the senior child health clinics and other services should be clarified better. Improving patients’ knowledge about the process of sorting patients for treatment upon admission The purpose of the thesis was to create a patient brochure of the triage process for the customers of Eastern health centre of Porvoo. The purpose of the brochure was to increase customers’ knowledge on how patients are sorted for treatment upon admission at the health centre, as well as to reduce the number of patient complaints regarding the sorting process of patients. Our thesis was an extension for a communication project, which was carried out at the health centre of Porvoo Eastern in the spring of 2013. The purpose of the development project was to collect background information and to survey the customers' knowledge about the triage process via interviews. The project showed that the customers of the health centre should be informed more about the triage process because their lack of knowledge increased the amount of complaints. The communication project was carried out in collaboration with University of Diak, University of Laurea, the city of Helsinki and the city of Porvoo. Our thesis was a functional thesis. The purpose of the thesis was to increase the patients’ knowledge about the sorting and prioritising process of patients and to create a brochure that would explain the process. The thesis was carried out with Porvoo Laurea and with Eastern health centre of Porvoo. The material for the thesis was gathered from literature, studies and articles concerning the triage process, legislation, health communication and informative brochures for patients. The result of the thesis was a patient brochure about the triage process, which has been in-troduced to the Eastern health centre of Porvoo. The health centre hoped that the brochure would increase the customers’ knowledge about the triage process. Two brochures were made, and they were presented to the customers of the Eastern health centre of Porvoo to gauge the effectiveness of the brochures. The customers were encouraged to give feedback regarding the content, appearance and the intelligibility of the brochures. The customers of the health centre selected the version with a friendlier, more familiar tone. The layout of the brochure was deemed clear. The content was considered easy to read and understand. Some clarification was required for the section about legislation because many customers did not understand the legal aspects of urgent and non-urgent care. The city of Porvoo provided a template for the brochure, which also offered information on how the health centre sorts the patients upon admission. There were several proposals for further development such as evaluating the effectiveness of the brochure, adding more information about the sorting and prioritising process of patients for treatment to the web pages of the city of Porvoo, writing news articles and creating an informative poster for the health centre. Hyvän hoidon määritelmä perheiden näkökulmasta –Systemaattinen kirjallisuuskatsaus
Tämän opinnäytetyön tarkoituksena on kuvailla hyvän hoidon määritelmää lastenosastolla
perheiden näkökulmasta. Opinnäytetyön tavoitteena on tuottaa tietoa hyvän hoidon määritelmästä
perheiden näkökulmasta Lastenklinikan osastolle K1. Opinnäytetyön tuloksia voidaan
tarvittaessa hyödyntää käytännön hoitotyössä ja sen kehittämisessä. Tutkimusmenetelmäksi
valittiin laadullisista tutkimusmenetelmistä systemaattinen kirjallisuuskatsaus, joka on koottua
tietoa rajatulta alueelta ja katsaukset ovat vastauksia tutkimusongelmiin.
Tässä opinnäytetyössä systemaattisen kirjallisuuskatsauksen aineisto analysoitiin sisällönanalyysillä.
Sisällönanalyysi on systemaattinen aineiston analyysimenetelmä, jonka avulla voidaan
kuvata analyysin kohteena olevaa tekstiä ja analyysissa haetaan vastausta tutkimustehtävään.
Aineistolähtöisessä sisällönanalyysissä keskeistä oli analyysiyksikön valinta, aineistoon tutustuminen,
aineiston pelkistäminen, aineiston luokittelu, tulkinta sekä analyysin luotettavuuden
arviointi. Olennaista oli muodostaa alustavia malleja havaitusta seikoista ja koota tutkimusten
havainnot yhteen, joka huipentui tutkimusraportin laatimiseen. Aineisto haettiin sähköisistä
tietokannoista ja tietokantointa käytettiin kotimaista Nelliä ja Mediciä sekä kansainvälisistä
Pudmedia ja Cinahlia. Opinnäytetyöhön valittiin analysoitavaksi 13 tutkimusta, joissa
käsiteltiin hyvän hoidon määritelmää perheiden eli lasten ja heidän huoltajiensa näkökulmasta.
Hyvän hoidon määritelmä kokonaisuudessaan perheiden näkökulmasta sisältää monia osaalueita
aina hoitohenkilökunnan pätevyydestä ja työskentelytavoista hoidon sisältöön ja sairaalaympäristön
viihtyvyyteen. Hyvää hoitoa on päämäärätietoinen toiminta, jolla pyritään
aikaansaamaan potilaalle kokonaisvaltaista, yksilöllistä, turvallista, omatoimisuuden ja osallistumisen
sallivaa, jatkuvaa ja tarpeenmukaista hoitoa. Hyvä hoito perustuu säädöksiin, arvoihin
ja eettisiin periaatteisiin. Keskeisiä arvoja hoitotyössä ovat luottamuksellisuus, tietojen
salassapito, oikeudenmukaisuus ja tasa-arvo.
Tulokset hyvän hoidon määritelmästä perheiden näkökulmasta muodostuvat lähes kokonaan
hoitajien ammattitaidosta suhteessa hyvään hoitoon. Perheiden näkökulmasta hyvää hoitoa on
hyvä hoitaja, joka on ammattitaitoinen, luonteenpiirteiltään empaattinen, hauska, huumorintajuinen,
lasten ja vanhempien tarpeet yksilöllisesti huomioiva, informaatiota antava, hoitoon
mukaan ottava sekä muilta toimintatavoiltaan tukea antava ja perheen edut huomioiva hoitoalan
ammattilainen.
Opinnäytetyön tulokset muodostuvat vanhempien ja lasten näkökulmista hyvästä hoidosta.
Hyvä hoito lasten näkökulmasta muodostui seuraavista tekijöistä; hyvä hoitaja lasten mielestä,
hyvän hoitajan toimintatavat lasten mielestä, hyvän hoitajan toimintatavat lasten kokemusten
mukaan, tiedonsaanti hoitajalta lasten kokemusten mukaan, lasten tarpeet tiedonsaantia
ja konsultaatiota koskien, lasten mukaanotto hoitoaan koskien, lasten osallistuminen
päätöksentekoon hoitoaan koskien, sairaalaympäristön viihtyvyyden tekijät lasten mielestä,
lasten kokemukset sairaalahoidon viihtyvyyden tekijöistä, lasten odotukset vanhempien toimintatavoista
sairaalahoidossa sekä hyvän hoidon tekijät lasten mielestä.
Hyvä hoito vanhempien näkökulmasta muodostui seuraavista tekijöistä; vanhempien tyytyväisyys
hoitotyöntekijöihin, vanhempien kokemukset lastensa sairaalahoidosta, vanhempien toiveet
hoitoa koskien, vanhempien tärkeinä pitämät asiat hoitoa koskien, vuorovaikutukselliset
seikat, joita vanhemmat arvostavat hoidossa, vanhempien mielipiteet kommunikaatiosta hoidossa,
vanhempien suhtautuminen lapsen osallistumiseen päätöksentekoprosessiin, sairaalaympäristön
viihtyvyys tärkeää vanhempien kannalta sekä hyvän hoidon tekijät vanhempien

Monikkoperheet terveydenhoitajan vastaanotolla : - koulutustilaisuus monikkoperheiden haasteista terveydenhoitajaopiskelijoille

Opinnäytetyön tarkoitus oli pitää terveydenhoitajaopiskelijoille koulutustilaisuus monikkoperheiden erityishaasteista terveydenhoitajan vastaanotolla. Tilaisuudessa keskityttiin opiskelijoiden monikkotietouden lisäämiseen ja haluttiin antaa valmiuksia kohdata monikkoperheitä työelämässä. Työn tarkoitus ja tavoitteet olivat lähtöisin työn tekijöiden havaitsemista tarpeista sekä yhteistyöstä Suomen Monikkoperheet ry:n kanssa. Koulutustilaisuudesta kerättiin myös palautetta, jonka perusteella vastaavanlaista koulutusta voisi kehittää jatkossa niin opiskelijan kuin Suomen Monikkoperheet ry:n näkökulmasta.
Opinnäytetyö toteutettiin teoriapaketin kokoamisella sekä koulutustilaisuuden järjestämisellä. Koulutustilaisuuden sisältö suunniteltiin yhteistyössä Suomen Monikkoperheet ry:n vapaaehtoistyön koordinaattorin kanssa. Koulutuksen havainnollistamiskeinoina käytettiin PowerPoint-esitystä, videoita sekä Suomen Monikkoperheet ry:n tuottamaa materiaalia. Koulutustilaisuuden ohjelmarunko oli rakennettu oppimista edistävästi, ja tilaisuudelle oli asetettu selkeät tavoitteet. Jotta pystyttäisiin arvioimaan tavoitteisiin pääsyä, laadittiin palautekyselylomake. Se sisälsi neljä suljettua kysymystä ja yhden avoimen kysymyksen, jossa vastaajat saivat omin sanoin kuvailla kokemuksiaan. Kyselyn tulokset purettiin ja ne liitettiin työhön.
Koulutustilaisuuden palautekyselyn tuloksista voitiin päätellä, että tilaisuus oli terveydenhoitajaopiskelijoille tarpeellinen kokemus. Opiskelijat kokivat tilaisuuden hyödylliseksi ja antoivat sille kouluarvosanaksi keskimäärin 9,4. Koulutuksessa esitelty teoriatieto oli lähes kaikille uutta tietoa. Monella osallistujalla oli kuitenkin kokemusta monikkoperheistä ja raskaudesta sekä lapsista yleensä oman elämän tai lähipiirin kautta.
Jatkossa olisi hyödyllistä järjestää vastaavanlaista koulutusta terveydenhoitajaopiskelijoille pidemmällä seuranta-ajalla. Myös opiskelijoille sekä jo neuvolatyössä oleville terveydenhoitajille voisi järjestää yhteisen koulutustilaisuuden, joka mahdollistaisi käytännön kokemusten jakamisen. Opinnäytetyön tutkimusaiheena voisi olla myös neuvolatyössä toimivien terveydenhoitajien kokemusten kartoittaminen monikkoperheiden osalta....

According to the most recent findings, more than a million Finns suffer from insomnia. The structure of the sleeping pattern changes due to ageing, and this in itself may be the cause for insomnia. As a person passes middle-age the sleeping patterns change, decreasing the amount of deep sleep and increasing the periods of shallow sleep. During the phases of shallow sleeping, an individual is more likely to wake up. Deep sleep, on the other hand, gives a sense of rest a feeling of and a night well slept. Research that has been conducted on the topic also shows the dissatisfaction of the elderly towards the existing services provided to treat insomnia. Even though drugs are usually offered, this often remains the only option. However, insomnia can be treated using alternative methods. These other, non-pharmaceutical methods should indeed be promoted primarily as treatment on their own or in combination with some mild drugs, as drugs for treating insomnia do not induce natural sleep. This thesis deals with insomnia amongst the elderly and non-drug treatment methods. The purpose of the thesis is to increase the awareness of the importance of sleep for the elderly people’s health and to consider whether discussion sessions can be useful for the treatment of insomnia. The aim is to increase the understanding of insomnia in the elderly non-pharmacological methods of treatment, and thus affects their ability to treat insomnia. The thesis has been restricted to primary insomnia, eliminating other sources of insomnia from our study. The thesis is based on functional work with the residents in Saga Helapuisto service home. A discussion session was held for the residents. The residents talked about their sleeping problems and during the session they were given guidance and information on non-drug methods to treat insomnia. The session was attended by three residents aged 80-90 years. According to the participants, sleeping drugs have always been prescribed straight away without receiving any guidance from a doctor or the medical staff concerning their sleeping difficulty beforehand. Based on the written feedback received from the participants, they felt that the session was useful and hoped for more similar sessions in the future. Hence, based on the feedback, the discussion session was successful and achieved its goal. According to the participants, sleeping drugs have always been prescribed straight away without receiving any guidance from a doctor or the medical staff concerning their sleeping difficulty beforehand. Based on the written feedback received from the participants, they felt that the session was useful and hoped for more similar sessions in the future. Hence, based on the feedback, the discussion session was successful and achieved its goal. The thesis also discusses whether more effort should be invested in non-drug treatment methods when it comes to treating and preventing insomnia in elderly people by providing advice and guidance....

The purpose of this study was to provide a compact, easy-to-read guideline about the impacts of alcohol abuse on health for the clients and nursing staff in the Acute Rehabilitation Unit. This study is a part of the Emppu-project the aim of which is to develop the mental health and intoxicant care services in Espoo. The Acute Rehabilitation Unit is part of the mental health and intoxicant care services in Espoo. The objective of this study was to provide written information which can be utilized as a guideline tool in the rehabilitation unit and also as an independently readable material. The aim was that the clients could receive information about the impacts of alcohol on health and to pay attention to the negative effects it has on their lives. The study method was functional which allows the development of practical activities in the Acute Rehabilitation Unit. The study was conducted in co-operation with the staff in the Acute Rehabilitation Unit and the nursing staff gave feedback during the whole process. The guide covers all impacts of alcohol on mental health, sexuality, sleeping disorders, memory, blood circulation, skin problems and motoric disorders. Especially a nurse can bring up these topics to private discussions with clients and provide comprehensive substance abuse treatment during the rehabilitation....
