The giving of blood products in the wards 1-6 at Lohja’s basic security centre - Description and evaluation of the current operations This thesis is a part of a larger developing project for Safe medical care (5), which has been executed as a co-operation between Laurea University of Applied Sciences and the basic security centre of Lost. Together Lohja, Siuntio, Inkoo and Karjalohja form the co-operation district Lost which is a basic security centre of social and public health care system. Lost was founded on January first 2009. The aim of “the safe medicinal care project” is to develop the security of medicinal care at Lost. Furthermore the purpose of the project is to create the general part of Lost’s plan for medicinal care. This will be done by following the recommendations in the “Guide for safe medicinal care” compiled by Ministry of Social Affairs and Health (2006). According to the recommendation made by the Ministry of Social Affairs and Health (MSAH) the giving of both medicinal care and blood products should be based on the plan for medicinal care which is made in the operative units. This plan for medicinal care depicts both the design and execution of the unit’s administration and giving of blood products and medicinal care as a whole. The deviations from the medicinal care and blood products are also reported and followed-up in the plan. The district of Lost lacks a plan for general medicinal care. The aim of this thesis was 1. To describe the present operations in giving blood products in the wards 1-6 at Lost, and 2. According to the research results, to design a new operations manual. 12 nurses were theme interviewed in order to have a general picture of the present state of blood transfusions. Two nurses were selected from every hospital service unit. The “Guide for safe medicinal care” compiled by MSAH and essential research material of the theme were used as a base for the questions in the interviews. The interviews were recorded, transcribed and analyzed by a content analysis. The interviewed nurses were of the opinion that lack of education and orientation was problematic. The nurses wished that there were cohesive instruction manuals for the entire blood transfusion process: Beginning with ordering the blood products and ending to the disposal of the used blood product bags and transmitters. To develop the operations and secure patient security, a process scheme of blood transfusion was formed. This was done on the basis of the description and evaluation of current blood transfusions. In addition to this a check list for giving blood transfusion was comprised. That is, the units received a “room blackboard”, which was created from the process scheme and furthermore the units now had the benefit of a check list as a hands-on tool. The new operations model will be used in the general medical care plan for blood transfusions at Lost. It can be concluded that the process of blood transfusions will be secure for patients, but by changing the working habits related to be preparations of blood products, more qualified blood transfusions will be possible. Furthermore, the interviewees estimated that education and orientation adds value to the entire blood transfusion process. According to the statement of a representative from working life this thesis as a whole can be exploited in establishing a medicinal care plan for the Lost basic medical care centre in general and specifically for the wards 1-6....

Imetyksen tukeminen ja imetysohjaus : Imetysopas ohjaustukimateriaalina lastenosastolle

Toiminnallisen opinnäytetyöni tarkoituksena oli tehdä imetysopas Porvoon sairaalan lasten osastolle imetyksen tukemisesta ja imetysohjauksesta. Toiminnallinen opinnäyteyöni koostuu imetysoppaasta ja projektiraportista. Yhteistyömme alkoi syksyllä 2009 lastenosaston osastonhoitajan Eila Kohosen sekä kahden imetysvastaavan kanssa. Tarkoituksena oli tehdä mielenkiintoinen, helposti ymmärrettävä sekä höydyllinen materiaali vanhemmille imetyksen hyödyistä ja imetysohjauksesta. Opinnäytetyötä voi myös hyödyntää vanhempien ohjauksessa. Imetysopas on tarkoitettu lasten osastolle, mutta tavoitteena on, että tulevaisuudessa kyseistä materiaalia voi myös hyödyntää äitiys- sekä lastenneuvoloissa. Materiaalia ei ole tarkoitettu työntekijöiden perehdyttämiseen tai opettamiseen.
Halusin tehdä imetysoppaasta mahdollisimman selkeän. En halunnut laittaa työhön liikaa tekstiä, jolloin visuaalinen selkeys kärsisi. Keräämäni tiedot perustuvat uusimpiin tutkimustietoihin. Halusin poimia imetysoppaaseen enemmän konkreettisia kuin yksityiskohtaisia asioita. Tällöin vanhempien on helpompi omaksua imetysoppaan sisältö. Tuotos sisältää tietoa hyvistä imetysasennoista, imetyksen terveydellisista hyödyistä sekä kuvia oikeasta imemisotteesta, jolloin asian selittäminen selkeytyy huomattavasti. Tavoitteena oli saada aikaiseksi opas, joka kannustaisi äitejä imettämään.
Oppaan sisältö perustuu opinnäyteyöni teoreettiseen viitekehykseen ja omaan työkokemukseeni lasten osastolla. Opinnäytetyön teoreettisessa viitekehyksessä käsittelen äidinmaidon koostumusta sekä miten äitejä tuetaan imetyksessä. Lisäksi perehdyin imetyksen terveydellisiin hyötyihin sekä lapsen että äidin näkökulmasta aina lapsentahtisesta imetyksestä oikeaan imetysotteeseen, imetyksen onnistumiseen vaikuttavista tekijöistä sekä antaa tukea ja ohjausta ongelmallisissa imetystilanteissa. Projektiin kuuluu myös toiminnallisen opinnäytetyön tuotoksen suunnittelu ja toteutuksen vaiheet....

Safe medication – double checking and medication error form in Virkkala and Mäntynummi home care This thesis is a part of Laurea University of Applied Sciences Lohja’s student’s report of Safe medication 3 project. The purpose of the project is to provide safe medication in co-operation with Lohja home care. The project is based on Ministry of Social Affairs and Health’s Safe medication handbook (2006). The safe medication handbook was written to support medication planning and realization in different healthcare units. The Safe medication 3 project is based on the Safe medication 2 project. The present state was charted, on the basis which the development targets were revealed. Safe medication plans will be created for Lohja home care units based on these Safe medication 3 project theses. The purpose of this thesis is to develop medication safety within Mäntynummi and Virkkala home care units. The target is to create operations models for double checking and using of medication error form and further utilization of the forms. The chosen development method was dissemination, where organization’s tacit knowledge is transferred into tangible knowledge and merged with research and theoretical knowledge into a new operations model. The research and theoretical knowledge was sought from the research on safe medication, double checking, medication errors and dissemination. The present state of double checking and medication error form were charted in Virkkala and Mäntynummi home care units. The working group created operation models based on research, theoretical knowledge and present state of affairs. Models were on trial in both home care units for two months in the autumn 2009. In the created operations models for double checking, two nurses were always checking each other’s drugs administration. This was made possible by adding more client visits and through workday planning. Operation model for medication errors and dissemination were invested anonymously on the announcer. This created distinct operational principle when filling and returning the error forms and also when describing the work of the unit directors. In the final evaluation of the thesis, members of the work community evaluated the operations models with a questionnaire and orally. The double checking was experienced as work-increasing and challenging to execute due to because of lack of resources. It was considered to be a good idea even if it did not affect the number of the medication errors in drugs administration. The medication error forms were paid more attention and completed better than before. The concept of medication errors was hard to understand because of its versatility. The medication error form is still unclear and hard to fill and there is room for improvement. The unit directors have to develop further processing of the forms. The processing of the forms failed because of scheduling problems and the challenging form. The filling of the forms will improve in the future. ...

Safety medication – Developing safe medical treatment in Vihdin Sairaankuljetus oy This thesis is a part of the project Safe Medical Treatment 4 at Laurea University of Applied Sciences in Lohja. The purpose of this project was to form a medical treatment scheme for the primary care in Lost – municipality and Karviainen region. The project was based on Safe Medical Treatment - guide book of Ministry of Social Affairs and Health. The target of this work was to develop safety in medical treatment in Vihdin Sairaankuljetus oy. The object of development was chosen to be the knowledge of medical treatment and how to maintain it in Vihdin Sairaankuljetus oy. The target of this work was to form training material and some lessons for this cause. The current condition of medical treatment in Vihdin Sairaankuljetus oy was represented by the means of theme interview. The interview was recorded and transcribed into text. With this procedure the knowledge of the current state of medical treatment and potential dilemmas in accomplishing it were learned. With the analysis of the content the development target was found and the process of medical treatment was represented with the help of Herringbone pattern. The theoretical knowledge about the safe medical treatment was searched for especially from the point of view of primary health care. The means in the action of development was rooting. With the help of rooting the silent information is turned into visible knowledge even as it is combined with the studied and the theoretical information to form a whole new pattern. The team to develop the operation consisted of the three nurse students working on the thesis and three representatives from Vihdin Sairaankuljetus oy. Vihdin Sairaankuljetus oy was represented by the CEO of the company and two nurses responsible for the medical care.As a result of the project the nurse students working on the thesis formed the pharmacotherapy plan and the training material concerning all the medicines used in the units of Vihdin Sairaankuljetus oy. The training material containing the pharmacology, dosing, side effects and the potential special notes in using a medicine was put on the Power Point – slides. The material was stored in e-form to be shared to all the ambulance service stations. In addition six training lessons were carried out based on the training material. The operation was evaluated by the personnel of Vihdin Sairaankuljetus oy with an evaluation – and feedback form. On the feedback received the personnel of Vihdin Sairaankuljetus oy has found the new pattern good. ...

Sairaanhoitajaopiskelijat yhteistyön kehittäjänä nuorisotyöpaja HOPE:n kanssa

Tämän opinnäytetyön tarkoituksena oli arvioida Hyvinkään Laurean sairaanhoitajaopiskelijoiden ja Hyvinkään kaupungin nuorisotyöpaja HOPE:n välistä yhteistyötä. Yhteistyö alkoi syksyllä 2008 hoitotyön opiskelijoiden työpajanuorille pitämillä terveyskeskusteluilla ja jatkui keväällä 2009 opiskelijoiden järjestämillä hyvinvointiseminaarilla ja virkistyspäivällä. Toteutunutta yhteistyötä esiteltiin Maailman mielenterveysliiton konferenssissa Ateenassa syksyllä 2009. Yhteistyöstä kirjoitettiin artikkeli Laurean Kehittäjä-lehteen.
Yhteistyön tavoitteena oli tarjota tietoa paikallisten terveyspalvelujen toiminnasta, yleisistä terveyteen liittyvistä osatekijöistä sekä oman hyvinvoinnin ylläpitämisestä. Yhteistyön aikana hoitotyön opiskelijoilla oli mahdollisuus toimia uudessa oppimisympäristössä sekä toteuttaa Laureassa käytössä olevaa työelämälähtöistä opiskelumallia.
Opinnäytetyön tavoitteena oli luoda yhteistyömalli, jossa kuvataan yhteistyön etenemistä syksyn terveyskeskusteluista kevään hyvinvointiseminaariin ja virkistyspäivään. Mallista ilmenevät myös hoitotyön opiskelijoiden opintojaksot, joihin yhteistyön toteutus liittyy.
Työ toteutettiin toiminnallisena opinnäytetyönä ja aineisto käsiteltiin käyttäen induktiivista sisällönanalyysiä. Keväällä 2010 toteutettiin työpajaohjaajille yhteistyötä arvioivan kysely, jota käytettiin opinnäytetyön aineistona. Yhteistyön onnistumisen arvioinnin apuna on ollut myös vuosien 2009 ja 2010 hyvinvointiseminaarien sekä vuoden 2009 virkistyspäivän jälkeen osallistujilta kerätyt kirjalliset palautteet.
Yhteistyö HOPE:n ja Laurean välillä on ollut onnistunutta ja antoisaa. Palaute on ollut positiivista. Hyvinvointiseminaareissa sekä virkistyspäivänä esiin tulleet hyvinvointiin liittyvät asiat koettiin hyödyllisiksi. Hoitotyön opiskelijat saivat onnistuneita oppimiskokemuksia ja heillä oli mahdollisuus harjoitella teoriassa opittuja tietoja käytännössä, aidossa oppimisympäristössä.
Jatkona opinnäytetyölle voisi olla tutkimus, jossa tutkitaan nuorten tietämystä terveyteen ja hyvinvointiin vaikuttavista tekijöistä sekä heidän suhtautumistaan omaan terveyteen ja hyvinvointiin. Tutkimus toteutettaisiin ennen terveyskeskusteluja, seminaareja ja virkistyspäivää sekä niiden jälkeen. Tämä kertoisi, ovatko tapahtuneet vaikuttaneet nuorten ajatteluun ja toimintaan oman terveyden edistämisessä.
