In addition, the aim was to explore the advantages and disadvantages on the use of a service voucher. This thesis was part of the Pumppu project, which is an interregional thematic project financed by European Regional Developmental Fund. The project searches for new methods in delivering welfare services, and is implemented through a cooperation project concerning four Finnish provinces. The main themes of this thesis were: voucher conditions of use, the advantages and disadvantages, and the development proposals. Qualitative content analysis method was applied in conducting this thesis. The data concerning western Uusimaa area were collected through theme interviews. The interviewees were the persons responsible for municipal services. In western Uusimaa, the introduction of the service voucher has been limited. At the moment it is used in three municipalities. In these municipalities a service voucher is used for delivering dental follow-up care, providing private daycare services for children as well as offering services, which enable offering of holidays for family caregivers. The interviews have revealed that the municipal structure changes have demanded a lot of municipal resources in western Uusimaa, and therefore have contributed to slowing down of the introduction of service vouchers in this area. In addition, the use of the introduction process was experienced as challenging. However, it was evident that the introduction and use of service vouchers was seen as a crucial ethical issue in delivering supportive welfare services for the residents. The municipalities want to respond to the needs of the residents and increase their freedom of choice. The municipal service voucher has established itself as part of the municipal service delivery system in Finland. Services delivered through vouchers increase the residents’ freedom of choice and their ability to influence their own care. The municipality, in turn, benefits from the service voucher by receiving possibilities to deliver its’ services according to different service needs, peak demands and a tightening financial situation. These demands sometimes require rather rapid solutions. ”I too want a home of my own" A survey of the housing needs of disabled people in Lohja area The purpose of this thesis is to evaluate the needs and wishes of the disabled people aged 16 or more, who are living in their childhood homes in Lohja. At the same time the hopes of their families considering the needs of their close ones were gathered. The City of Lohja aimed to start at the highest level a strategic planning of the housing services available for the disabled people. This thesis is made to help and support the planning. This thesis was made as a part of the Pumppu project. The Pumppu project aims to develop the welfare services into a meaningful entity and to enter into i.a developing the services of disabled children and young people in the western Uusimaa region. The research method of the study was quantitative. The basic research of the survey is based on the references of former studies and the conclusions of them. The method of sampling material was discretionary, because the questionnaires were sent to carefully controlled set of groups. The study material was obtained by sending questionnaires to both disabled people and their families. The number of sent questionnaires was a 104. Most of the disabled people who answered the questionnaire hoped in the future to live in a residential home, where there would be staff available 24h/day or their own housing. The residence was hoped to be near services. Hopes of the families were similar. Every one of the disabled people wished for their own room and own peace. Considered important were also accessibility, home-like atmosphere, support and getting help, plus a reliable staff, thing that in particular rose up from the families. As far as services in the living environment were concerned, there was a wish that the basic services, such as shops, banking, job or a place to study and recreational opportunities would be as close to home as possible within good public transportation. The hopes of the disabled people and their families were similar concerning services. The challenges related to future housing were considered to be loneliness, homesickness and a fear that one would not be understood. The matter arose from answers from families of severely disabled people, and also there was a fear that no one else but a close family member can interpret the sign marks of a close one. A concern of the possible lack of professional skills and adequacy of the staff also emerged from the answers of the relatives. The majority of the survey respondents with disabilities are in about 5 years moving out of their childhood home within to a dorm or to own apartment. People with less severe development disabilities wished everyday help from close assistants and people with difficult disabilities have a need for basic care and attention round the clock....

Health care entrepreneurship - starting a health care business and the consulting services My interest in starting a business in the health care field as well as the results of the customer survey which was conducted by the Laurea Enterprise Center hosted by Laurea UAS influenced the choice of the topic for this thesis. The purpose of this qualitative thesis was to describe the entrepreneurship in the health care field and how a young entrepreneur can take advantage of the consulting services. The aim was also to find out what a young entrepreneur needs when starting a health care business as a sole trader, where the entrepreneur can find information when starting a business and what kind of information a young entrepreneur needs. The research was carried out during the spring 2010 and the spring 2013. The thesis includes a business plan that focuses on how to start a business. In the customer survey of the Laurea Enterprise Center in 2010 the entrepreneurs were asked what kind of consulting services they want and which consulting services they have used. The data was gathered through theme interviews with 56 entrepreneurs by telephone. The theme interviews were analysed by using inductive data-oriented content analysis method. The theoretical framework consisted of the current literature on health care, entrepreneurship and consulting as well as the trade journals, statistics in different fields, reports and legislation in the field of health care. The results indicate that there are plenty of consulting services for the health care field. There are for example services for starting, financing and developing a business. For the young entrepreneur the most important contacts are the municipal business development advisors, regional business services, centres for economic development, transport and environment as well as employment and economic development offices. The company operations starting from the business plan are regulated by the law. The study will help a young entrepreneur with the legal matters of the business as well as the consulting services available. In addition to that it will give a comprehensive impression of the care field and starting a business. For the author this study was an introduction to the demands of the branch and the basic information for starting a business. Hopefully the students of the health care field will find the study helpful when planning their own business....

Treatment of pain : guide for nursing staff and patient The aim of the thesis was to provide nursing staff and patients with scientifically studied information on various methods in the treatment of pain. As a review on published material, the thesis concentrated on the scientific publications, articles, research, dissertations, laws, and guides. The thesis deals with origin of pain, various types of pain, pharmacological, but mostly non-pharmacological pain treatment, pain evaluation, follow up and documentation. It was considered to find the best ways to guide patients in the treatment of pain. Based on review of previously published material, I wrote this guide for the nursing staff and patients about treatment of pain at the primary health service. The guide was written from a patient´s perspective. The idea of this guide is to support and to guide the independent work of the nurses and patients about pain treatment. The guide is not just for one group of patients, because it is meant to help everybody who is interested about this item. Based on earlier researches it is known that nurses have an important role in the pain treatment, evaluation, follow up and documentation of pain. They have to help and support the patients in pain treatment and consequently to promote their wellbeing. Based on previously published material, a good treatment accelerates the healing process and rehabilitation. Because good pain treatment is every patient´s fundamental right, a good treatment entails an efficient and correct pain medication and positive attitude towards efficient treatment of pain....

Laurea University of Applied Sciences Abstract Laurea Lohja Health Care Nursing Hantunen Ramona, Silta-aho Minna Experiences of services of people with heart disease in Vihti and Karkkila area Year 2013 Pages 59 This thesis is part of Pumppu project which is a project funded by the European Regional De-velopment Fund. The project’s purpose is to research welfare services during 2011-2014. Themes of the project are motivating, empowerment and seamless wellbeing path. Laurea University of Applied Sciences has a part in the Pumppu project. The purpose of this thesis was to provide information about the experiences of services by people with heart disease in Vihti and Karkkila area. The aim was to provide information of how people with the heart disease experience those services in the area that support their life control. The aim was also to find out how such services support self-management of people with a heart disease as well as their coping on clinical pathway. This thesis was in form of a dissertation and qualitative research. Persons over 45 years old were interviewed (N = 13) for the study and each of them lived in Vihti and Karkkila area and had one or more of the following diseases or conditions: high blood pressure, coronary disease, atrial fibrillation, heart failure or myocardial infarction. Interviewees were introduced to the survey via Vihti and Karkkila heart associations and via a Pumppu project discussion that was arranged in Vihti. The results of the thesis prove that people were not aware of all services. Public health care services, heart associations and private health care services were known well. Development was expected from the public health care services. There was mainly contentment towards the private and third sector services. Heart association was considered as an important supporter. In terms of empowerment of a person with a heart disease, peer support and physical exercise were considered important. The effects of heart disease that were mentioned were emotional life becoming more sensitive and physical activity calming down. However, heart disease did not affect the life of all. The results show that greatest development areas were in the public health care. Improvement of after-treatment and alliance with third sector and municipalities were considered most important matters. Additionally, there was a need for heart disease-specialized nurse in the area. In order to secure seamless wellbeing path for a person with a heart disease, target for de-velopment should be that health centers of the areas could create a job description for a nurse that is specialized with heart diseases. In terms of health promotion of people with heart disease, sustaining and supporting of physical exercise possibilities is important. Collaboration between municipalities and the heart associations is important for the benefit of people with heart disease. The neurosurgical intensive care patient – assessing and updating recommendations for monitoring consciousness This thesis is part of the quality development project of Laurea University of Applied Sciences, which aims to improve operative procedures at Hospital District of Helsinki and Uusimaa. The project began in 2007 and ended in 2012, and it aims to improve standards of care and establish common care procedures for health care personnel. This thesis examines care procedures at the neurosurgical intensive care unit (ICU) at Töölö Hospital. In 2009, another thesis project made recommendations for improved procedural standards for monitoring patient consciousness at the ICU, which were subsequently adopted as operative goals at the unit. This thesis examines the implementation of these procedural recommendations at the ICU while recommending updated procedures for monitoring the consciousness of neurosurgical patients at the ICU. In addition, this thesis aims to set procedural standards for the management of pain and elevated intracranial pressure for the nurses at the ICU. The procedural recommendations consider the specific needs of the ICU nurses and respond to the results of a questionnaire inquiry submitted to the ICU nurses as part of this thesis. Above all, the common and consistent care standards recommended by this thesis aim to ensure patient safety and high-quality care. The recommendations of this thesis may also be employed in training students and new employees at hospitals. The neurosurgical intensive care unit has a care capacity for 16 patients, who require either neurosurgery or nonsurgical care for serious cases of brain injuries or brain hemorrhage. The age of the patients as well as the complexity and length of their care varies greatly at the ICU. The care of neurosurgical patients emphasises the monitoring of patient consciousness and sedation as well as the management of the pain and intracranial pressure. The thesis project was carried out throughout the year 2012. The project involved collecting data, writing a questionnaire inquiry, and analysing its results. The questionnaire assessed the actualisation of 23 care recommendations and highlighted six areas wherein procedural recommendations were not followed by the quiry respondents or such recommendations were considered unfeasible. These areas are illustrated in data tables. Within this framework, SPSS software was utilised to examine whether any neurosurgical employment experience may have influenced the inquiry respondents in areas of particular incongruence. Only four procedural recommendations were unanimously followed and considered feasible by all inquiry respondents. This thesis focuses on problematic recommendations. Future challenges include implementing updated procedural recommendations at the ICU as well as customising such recommendations for use by other hospital wards. Furthermore, the recommendations would benefit from regular assessments and updates....

Retail Clinic – An Integrative Review Retail clinics are a relatively new health care delivery model in Finland. There is a limited number of Finnish studies on retail clinics, although the subject has been extensively studied abroad. Due to a rapid expansion of the retail clinic model, further information is needed on the working processes and organization of these clinics. The purpose of this study is to outline the different models of operation and services offered at retail clinics by means of an integrative review. The method of this study is integrative review, the most comprehensive type of literature reviews. The integrative review allows the inclusion of data with diverse methodologies. The literature search was carried out in both Finnish and in English. A total of 32 articles, 29 in English and 3 in Finnish, were extracted. The articles were analyzed using deductive data analysis. Retail clinic is an easy-access clinic staffed by nurses or nurse practitioners. The majority of retail clinics are located at retail sites. Easy accessibility and location at a public site are crucial factors for the success of the retail clinic concept. In the United States and the Great Britain, retail clinics are focused on the treatment of acute, minor illnesses. The common ailments treated there are respiratory, throat, eye and ear infections, flus and vaccinations. The emphasis of services offered at retail clinics in Finland has been on health promotion. The majority of clients visit retail clinics for health counselling, tests, screenings or vaccinations. In Finland and the Great Britain retail clinics are a part of the public health care system, whereas in the United States clinics are privately owned businesses. The majority of retail clinics in the US are owned by retail chain stores. The professional role of the nurses working at retail clinics is unlike the traditional nursing role; working at the retail clinics requires independence and diagnostic skills. The work is rewarding but also stressful at times. The core competences needed for retail clinic nursing are still unclear. The role of retail clinics as a part of the health care system is still evolving: The goals and emphases of retail clinics need further consideration. Making retail clinics more accessible for the populations in need of their services should be developed in the future. Stronger cooperation between retail clinics and health and social care operators should be promoted. In spite of these challenges, the retail clinic concept could prove to be a model and inspiration for new cost-effective health care services....

Babies with good sleep patterns – Group guidance and a guidebook of how to take care and prevent sleep problems in children under six months of age The subject of the thesis was the sleep patterns in children under six months of age and problems related to them. The thesis also discusses sleep guidance to parents given by a public health nurse. The purpose was to organise an event as part of family training to support first time parents. We also compiled a baby sleep guide booklet to be handed out as a support for guidance during the event. The aim was to inform the parents about babies’ sleep patterns, sleeping and to aid babies fall asleep and to develop sleeping routines. The aim was also to provide tools for public health nurses to facilitate guidance in sleep issues. The group guidance was organized at Family Center Maahinen in Järvenpää on 22 April 2013. The group guidance was advertised to parents with children under six months of age. The guidance included the practice of swaddling and baby massage with the parents’ own baby. A guide booklet and guides to swaddling were handed out to all group members. The feedback from group guidance was collected with a feedback form. According to the feedback, the guidance was seen as successful and useful. It was possible to receive peer support from the group. Some members wished to have more concrete ways to solve sleeping problems. This was however not possible because of the strict time limit. The guide booklet also received positive feedback. Sleeping guidance is needed more for families both for preventing causes and when sleeping problems occur. Public health nurses should familiarise themselves more with the subject. They should also give better guidance and support to parents to help babies with their sleep patterns. Using the guide booklet in public health nurse’s work would benefit them and give them better tools in their work. It would be interesting to know how members experienced the guide booklet in practice, after the group guidance. The sufficiency of the current sleep guidance would also be interesting to know. The subject of additional research might be how the sleeping guidance provided by the nurse is actually carried out. Also, what kind of additional support the parents would need....

ESBL- ja MRSA–potilaiden arjessa selviytyminen ja hoito vuodeosastolla : sairaanhoitajien kokemuksia

Opinnäytetyön tarkoituksena oli kuvata sairaanhoitajien kokemuksia ESBL- ja MRSA-kantajuuksista sekä sairaanhoitajien tiedontarvetta niistä. Lisäksi tarkoituksena oli kartoittaa ESBL- ja MRSA-potilaiden arjen tuomia haasteita sairaanhoitajien näkökulmasta. Opinnäytetyön kohderyhmänä olivat Hämeenlinnan Vanajaveden sairaalan osasto 3:n sairaanhoitajat. Työn tavoitteena oli kartoittaa mahdolliset tarpeet sairaanhoitajien lisäkoulutukselle ESBL- ja MRSA-kantajuuteen liittyen sekä tarpeet hygieniaohjeistuksen päivittämiselle.
Opinnäytetyön tutkimuksellisessa osuudessa selvitettiin vuodeosaston sairaanhoitajien kokemuksia siitä, miten potilaan ESBL- ja MRSA-kantajuus vaikuttaa sairaanhoitajien päivittäiseen hoitotyöhön sekä millaisia haasteita sairaanhoitajat kokevat kantajuuden tuovan vuodeosaston ESBL- tai MRSA-potilaiden arkeen. Lisäksi oli tarkoitus selvittää, millaista tietoa sairaanhoitajilla on kantajuuksista sekä millaiseksi he kokevat potilaiden ohjaamisen kantajuuden suhteen. Tutkimus toteutettiin haastattelemalla Vanajaveden sairaalan osaston 3 kuutta sairaanhoitajaa. Haastatteluteemoissa käsiteltiin taustatietoja koulutuksesta ja hygieniaohjeistuksen sisällöstä, sairaanhoitajien tietoutta ESBL:stä ja MRSA:sta, kantajuuden tuomia haasteita arjessa sekä kokemuksia ESBL- ja MRSA-kantajan ohjaamisesta.
Haastattelut litteroitiin ja aineiston analyysimenetelmänä oli sisällönanalyysi. Tutkimustuloksista ilmeni, että Vanajaveden sairaalan osaston 3 sairaanhoitajat osaavat ohjata ESBL- ja MRSA-potilaita ja kohtaavat osastolla erilaisia haasteita eristyspotilaiden hoitotyössä. Sairaanhoitajilla on mielestään liian vähän koulutusta ESBL- ja MRSA-kantajuuksiin liittyen, mikä näkyy myös osan tietämyksessä. Suojapukeutumista sairaanhoitajat toteuttavat eri tavoin. Esiin nousi, että suojavaatteiden käytössä on epäkohtia.
Opinnäytetyön tuloksena syntyi käsihygieniaan liittyvät ohjeet käsienpesusta, desinfioinnista ja suojakäsineiden riisumisesta. Ohjeissa havainnollistettiin kuvin käsienpesun, desinfioinnin ja suojakäsineiden riisumisen eri vaiheet....
