Verkkoterapia osana masennuksen hoitoa : Systemaattinen kirjallisuuskatsaus

Masennus on Suomessa yleisin mielenterveyden häiriö, jonka hoidon aiheuttamat kansantaloudelliset kustannukset ovat mittavia. Tästä syystä masennuksen ja muiden mielenterveyshäiriöiden hoito painottuu yhä enemmän avohoidon puolelle ja siihen etsitään uusia hoitomuotoja ja apuja kehitteillä olevasta verkkoterapiasta.
Helsingin ja Uudenmaan sairaanhoitopiirissä on kehitetty yliopistotasoisen erikoissairaanhoidon osaamista hyödyntäen kansalaisten käyttöön palvelusivusto www.mielenterveystalo.fi. Sen tarkoituksena on välittää tietoa ja välineitä psyykkisestä hyvinvoinnista huolehtimiseen sekä tarvittavan avun saamiseen. Nämä HUS:in tarjoamat verkkoterapiat pohjautuvat kognitiiviseen viitekehykseen, jossa hyödynnetään psykoterapioita koskevaa yleistä tietoa.
Opinnäytetyön tarkoituksena oli systemaattisen kirjallisuuskatsauksen avulla kartoittaa internetin välityksellä annettujen kognitiivisten terapioiden hyötyjä ja haasteita osana masennuksen hoitoa. Tulosten avulla kartoitettiin, miten hyödyt ja haasteet näyttäytyvät kansainvälisesti. Opinnäytetyön tavoitteena oli tuottaa ajankohtaista tietoa verkkoterapian vaikuttavuudesta sekä haasteista masennuksen hoidossa hoitoalalla työskenteleville.
Systemaattinen kirjallisuuskatsaus rakentui tutkimuskysymysten määrittämisestä, tutkimusten hausta ja valinnasta, tutkimusten laadun arvioinnista sekä analysoinnista ja tulosten esittämisestä.
Tutkimusaineiston tulokset vastasivat kysymyksiin: Millaisia ovat verkkoterapian avulla saatavat hyödyt masennuksen hoidossa sekä minkälaisia haasteita nettiterapiaan hoitomuotona liittyy? Hyödyiksi nousivat verkkoterapian hoidollinen vaikuttavuus, ohjauksen merkitys verkkoterapian osana, potilaiden tyytyväisyys verkkoterapiaan, tuen antajan merkitys verkkoterapiassa, verkkoterapian avulla saatava vaikutus hoitoresursseihin, osallistumiskynnyksen madaltuminen verkkoterapissa, uudelleen sairastumisen ehkäiseminen sekä verkkoterapia elämänlaatua ja toimintakykyä parantavana tekijänä.
Haasteista esiin tulivat verkkoterapian soveltuvuus vain osalle potilaista, itsetuhoisuuden havaitsemisen vaikeus verkkoterapiassa, verkkoterapian mahdollisesti aiheuttama negatiivinen kokemus, jos hoito ei onnistu tai hoito keskeytyy sekä tiedon puute verkkoterapiassa vaikuttavista tekijöistä.
Tutkimustulokset viittaavat siihen, että verkkoterapiassa käytettävällä kognitiivisella käyttäytymisterapialla on vaikutusta masennuksen hoidossa....

Sairaanhoitajien osaamisen kartoitus päivystysalueella : Riihimäen terveyskeskuspäivystys

Opinnäytetyön tarkoituksena oli kartoittaa sairaanhoitajien kokemusta ammatillisesta osaamisestaan ja millaisia osaamistarpeita henkilökunta kokee tarvitsevansa Riihimäen terveyskeskuspäivystyksessä. Osaamiskartoituksen tarve on lähtöisin Riihimäen terveyskeskuspäivystyksestä, jossa on tehty organisaatiomuutos. Työn teoreettisessa viitekehyksessä käsitellään sairaanhoitajan ammatillista osaamista, terveydenhuollon organisaation tärkeimpiä osaamistarpeita, henkilöstön osaamisen kehittämistä ja päivystystyön erityispiirteitä.
Opinnäytetyössä etsittiin vastausta kahteen tutkimuskysymykseen: millaista ammatillista osaamista sairaanhoitajat kokevat tarvitsevansa Riihimäen terveyskeskuspäivystyksessä ja millaisia osaamistarpeita sairaanhoitajat kokevat tarvitsevansa? Tutkimus on laadullinen tutkimus ja aineisto kerättiin teemahaastattelun avulla. Haastattelu teemoitettiin Balanced Scorecardin neljän teeman mukaisesti. Haastatteluaineisto analysoitiin induktiivisen sisällönanalyysin avulla. Sisällönanalyysissa haastatteluaineistosta nousi esille myös haastattelutilanteissa ilmenneet hoitajien yhteneväiset kokemukset osaamistarpeista ja päivystyksessä tarvittavasta ammatillisesta osaamisesta.
Tutkimuksessa ilmeni, että Riihimäen terveyskeskuspäivystyksen sairaanhoitajat kokevat tarvitsevansa työssään potilaan hoidon suunnitteluun ja toteutukseen sekä potilasohjaukseen liittyvää ammatillista osaamista. Aineistosta nousi esille sairaanhoitajien tarve työhyvinvointia edistävästä organisaation sisäisestä yhteistyöstä, kuormittavien tilanteiden hallinnasta ja oman ammatillisen osaamisen kehittämiseen liittyvästä koulutuksesta....

Nurses’ experiences of structural evaluation of imminent danger on an acute psychiatric ward This thesis examines the experiences of the staff on the acute psychiatric ward at Peijas Hospital that belongs to the Hospital District of Helsinki and Uusimaa. It aims to survey the nurses’ experiences of the regular use of a structural evaluation tool, and what factors either support or restrain the use of the evaluation tool in clinical nursing according to them. The thesis is part of the Key to the mind project of the Hospital District of Helsinki and Uusimaa, where Laurea UAS is a collaboration partner. The project aims to improve the quality of mental health services through development project-based learning. This thesis applies qualitative research methods. Data acquisition was provided by semi-structured interview and data was analysed by content analysis. Six nurses working on the acute psychiatric ward took part in the interview, and they all had used the evaluation tool of imminent danger for one month. The theoretical part discusses the topic from the point of view of preventing violence, the context itself comprising violence, aggressiveness, the connection between a psychiatric disorder and tendency to violence, emerging of violence and preventing cases of violence at the psychiatric care unit. The results represent the interviewed nurses' experiences of using the evaluation tool of imminent danger on the ward. The results show that filling the evaluation form was not too time-consuming, and that there was always enough staff present, which thus supports the use of the evaluation tool. Most nurses felt the form was simple and easy to use. Insufficient introduction to using the tool, nurses' individual attitudes and the consequential futility of the tool as well as insufficient reporting of collected results were seen as restraining factors. Most nurses felt that using the evaluation tool did not result in increased patient observation or preventing danger, and it was seen mostly as only statistically useful. However, the respondents felt that mutual interaction has improved since the introduction of the evaluation tool. Based on the results it can be concluded that the nurses found the tool easy to use and it was not too time-consuming. The majority of nurses regarded the tool as mainly statistically important but with no actual role in preventing danger on the ward. The nurses wished to receive more information on how to use the tool in their work as well as clear common practices....

Self-mutilating adolescent This study was carried out as a descriptive literature review. The purpose of this study was to describe self –mutilation among young people. The aim was to provide information for nursing professionals and healthcare students about young people´s self-mutilation. Two research questions were established: What is young people`s self-mutilation about and what are health care personnel´s thoughts about young people who self-mutilate. The data was collected in health care electronic reference databases Ebsco CINAHL and PubMed. The search terms used in Ebsco CINAHL were "self mutilation" and "self cutting” whereas the search terms used in PubMed were "self mutilation AND", "self AND cutting" and "self AND Cutting and adolescent." Database searches yielded a total of 246 search results. The final material for the study consisted of six scientific articles. A predetermined intake- and exclusion criterion was applied in the selection of the material.The data was to fulfill the set quality criteria. Good scientific practices well as the methodological principles of the literature review guided our research process. The material was analyzed using a qualitative classification method. Based on the analysis six categories were made: Factors that contribute to self-mutilation/ the triggers of self-mutilation, the meaning of self-mutilation, the information related to self-mutilation and the methods, nurses conceptions about self-mutilating adolescents and self-mutilation, nurses conceptions about the contributing factors of self – mutilation and nurses´ affections and opinions about self -mutilation. These categories constitute the main results of this study. The emerging topics based on the research results concerned adolescents´ negative emotions and coping skills,the effects of the social environment in formation of self-image and identity in adolescence as well as self-mutilation as a cry for help. Further study is needed to establish the concept and the definition of the term self-mutilation. Further research is also required to determine the factors that contribute to stop self –mutilations well as the level and the amount of education provided to nursing professionals and health care students....

Promoting sexual health on the website of Porvoo contraception clinic This practice oriented thesis was conducted in cooperation with the contraception clinic of Porvoo. The purpose of our thesis was to promote sexual health on the website of Porvoo contraception clinic. Our aim was to produce an informative and customer-oriented information package on sexual health. The new pages follow the outlook of the website of Porvoo city. The theoretical framework of our report consists of health, sexual health, sexual rights, health communication and contraception clinic. On the basis of the theoretical background we have looked for reasons why it is important to promote sexual health on the internet. We collected data from the following sources: the databases of the library at Laurea University of Applied Sciences, publications on the internet and nursing science publications. We also utilized reliable sources on the internet and our interview conducted at the contraception clinic of Porvoo. Information on health is nowadays widely searched from the internet. Therefore, it is important that there is reliable and comprehensive information available on the internet. On the website, we wrote information on the different methods of contraception and venereal diseases as well as the first visit to a contraception clinic and the doctor’s. We compiled a list of useful links concerning sexual health to the website. The personnel of the contraception clinic gathered the most commonly asked questions and their answers, which were also added on the website. The topics discussed on the website were chosen together with the contraception clinic staff. The website we produced was evaluated by the staff of the contraception clinic, students at Porvoo Campus and our supervising teachers. We asked evaluation about the website from the students of Laurea and Haaga-Helia. For the evaluation process we compiled a questionnaire on the informativeness and readability of the website. In the questionnaire there was compiled question that the students were supposed to find an answer to. We got 19 answers of which evaluation was mainly positive. From the evaluators came development suggestions that pictures and graphic should be added to the website so that the text would be more alive. The evaluators were satisfied of the contents and easy access of the website. The responses were positive and indicated that we succeeded in our project....
