Kansallinen kirjallisuuskatsaus turvallisesta lääkehoidosta vuosina 2007-2012

Opinnäytetyön tarkoituksena oli kirjallisuuskatsauksen avulla kartoittaa Suomessa viime vuosina julkaistua kotimaista hoitotyön lääkehoitoon liittyvää tutkimustietoa ja ammattilehdissä julkaistua tietoa. Tavoitteena oli saada kattava käsitys kotimaisista lääkehoidon tutkimuksista ja ammattilehtien julkaisemista artikkeleista. Opinnäytetyön tavoitteena oli tuottaa ajankohtaista tietoa tarjolla olevasta hoitotyön lääkehoidon tutkimustiedosta ja ammatillisesta tiedosta.
Opinnäytetyössä haettiin tietoa systemaattisesti Medic- ja Melinda- aineistotietokantojen avulla. Laurus- hakua käytettiin myös. Tutkimuksen aineisto käsitti 42 artikkelia. Tähän katsaukseen otettiin mukaan vain ne julkaisut, jotka selkeästi liittyvät hoitotyöhön ja lääkehoitoon, kliinisen lääketieteen piiriin kuuluvat julkaisut jäivät pois. Ikärajana julkaisuille oli viisi vuotta. Aineistoksi kelpuutettiin väitöskirjat, pro gradu -tutkielmat ja artikkelit hoitotyön ammattilehdistä.
Aineisto analysoitiin laadullisella sisällönanalyysillä ja luokiteltiin aihepiirien mukaan. Analyysirunkona käytettiin Sosiaali- ja terveysministeriön turvallisen lääkehoito-oppaan kymmentä osa-aluetta.
Tulosten mukaan Suomessa on selkeästi panostettu lääkehoidon tutkimiseen ja selvittelyyn viime vuosina. Erityisesti esille nousi huoli hoitohenkilökunnan ja alan opiskelijoiden osaamisesta ja sen varmistamisesta sekä lääkepoikkeamista ja niihin liittyvästä toiminnasta.
Yhtenä tärkeänä osana lääkehoidon ja lääkitysturvallisuuden kehittämistä voitiin pitää terveydenhuollon ammattihenkilöiden lääkehoidon osaamisen edistämistä ja riittävää päivittämistä lisä- ja täydennyskoulutuksen sekä itsenäisen opiskelun kautta.
Kirjallisuuskatsauksen perusteella vaaratapahtumia aliraportoidaan. Paljon läheltä piti -tapahtumista jää raportoimatta kokonaan ja paljon niistä raportoidaan vain harvoin. Hoitohenkilökuntaa pitäisi rohkaista raportoimaan myös läheltä piti -tilanteet, koska niitä analysoimalla voidaan arvioida mahdollisia virhekohtia ja suunnata toiminnan kehittäminen näihin heikkoihin kohtiin.
On selvää, että Sosiaali- terveysministeriön Turvallinen lääkehoito -oppaan myötä on herätty miettimään lääkehoidon prosessia kokonaisuudessaan erityisesti turvallisuuden kannalta. Lääkehoidosta on tällä hetkellä kansallisella tasolla tarjolla varsin hyvin ajankohtaista tietoa ja omaa tietämystään pystyy päivittämään niin halutessaan.

Kidneys are small in size, even though they have a major function in the human body. Kidneys’ function is to remove harmful substances from the body, control the blood pressure level, body´s water and electrolyte balance, acid-base balance, calcium and phosphorus balance, as well as to control the production of red blood cells. When the renal function is reduced waste substances and fluids accumulate in the body. When a person's kidney function is impaired partially the diagnosis is renal insufficiency. Acute and chronic renal failure is caused when the functions of the glomerular capsules are impaired. The nursing staff should manage extensively a holistic patient care. A diet can affect a kidney´s function. One of the most important duties for the nursing personnel is to give guidance and support on the appropriate diet The aim of the study is to produce an instruction card for nursing staff on the diet for renal failure patients. The aim of the thesis is to develop nurses´ knowledge about the diet for pa-tients of renal failure and support them with an instruction card which shows how the diet should be followed and what kind of information need special attention. The main tasks for the nurses are patient counseling, health promotion and the maintenance of health. This thesis is practice-based and a part of the Meilahti Triangle Hospital –project in the Hospital District of Helsinki and Uusimaa. The project aims to promote the client-driven welfare in the hospital. The thesis was carried out in collaboration with wards 6A and 6B at the Meilahti Triangle Hospital. A variety of national and international current research data was collected for the study to be utilized in the production of the instruction card. We also requested comments from the nurses who work at the ward as well as from the last-year nursing students. The instruction card can be utilized in the guidance of student and new employee. The layout was structured visually interesting and the text easy to read. In the future, the instruction card can be extended to become a more comprehensive guide for the nursing staff....

Ventrogluteal injection – students´ experiences of using the ventrogluteal site for intra-muscular injections Ventrogluteal injection is not a new injection method, as it was first introduced in 1954 by a Swiss doctor named A. von Hochstetter who suggested this area in the body to be used when giving the injection. Its use is still, however, very insignificant, since most nursing care work environments have not applied the method because it is not very common and the nursing staff lacks proper skills. Proper education in ventrogluteal injections is still unavailable for the nursing staff in many workplaces. The purpose of this thesis is to thoroughly study the experiences of using the ventrogluteal injection and learning its use in practice by the nursing and public health nurse students. The students were educated the proper way to give a ventrogluteal injection and given a questionnaire that was related to the thesis. The most important questions were designed to examine whether the students found the ventrogluteal injection as a difficult or easy method and are they going to apply this injection method in the future as well as which injection method, the dorsogluteal injection or the ventrogluteal injection do they consider easier and safer. Safe medical treatment is debated on in the theoretical section of the thesis, due to its importance in all parenteral medical treatment. The thesis discusses ventrogluteal injection and the theory of guidance. The thesis also discusses the guidance process which explains the guidance given to the students about the ventrogluteal injection. The research questions and results are considered at the end of the thesis. Nursing and public health nurse students (N=50) of the Laurea University of Applied Sciences took part in the study. Almost all the students (92%) stated ventrogluteal injection was an easy method. Many students (96%) would rather use the ventrogluteal injection instead of the dorsogluteal injection and 98% of the students believe to be able to apply the ventrogluteal injection method in the future. The aim of the thesis is also to make ventrogluteal injections more common. The results of the study can be applied when designing the training in workplaces, and the thesis may also encourage the nursing work environments to take the new and safer intramuscular injection method in use in the future....

NOPS 2010–2012 Promoting workability and functional capability among young people in vocational schools : Occupational health discussion with hairstylist students from Hyria Karankatu This thesis is part of NOPS 2010-2012-project’s occupational health discussion activity. The purpose of NOPS 2010–2012 Promoting workability and functional capability among young people in vocational schools –project was to develop a range of health promotion methods for vocational school students and develop an operations model between a university of applied sciences and the vocational schools. The objective of these methods was to improve students’ health habits, the knowledge of safe and healthy way of working during studies. The objective of this thesis was to devise, accomplish and appraise occupational health discussion. The purpose of occupational health discussions was to support the student in promoting his/her workability and functional capability. Following topics are studied as a background theory for the study: workability and functional capability, phase of life, health and health habits of hairstylist students together with burdening factors of hairdressing. Health promotion aspect, occupational health discussion, supporting the process of changing health behavior, the role of motivation in the process of change and the impact of occupational health discussion were studied to promote young peoples’ workability and functional capability. Occupational health discussion was accomplished as a theme interview. The themes were health, health habits, occupational health, motivation and change which were discussed as open questions together with support material. Ten last year’s hairstylist students from vocational school Hyria Karankatu participated in the study. The interviews were recorded and analyzed with the help of a frame consisting of the main themes. The results showed that occupational health discussion motivated for change. In addition to motivation students received more information about health together with encouragement for changes. The behavior of the instructor was experienced good. The instructor was described encouraging and dependable among other things. The atmosphere was natural and open. Occupational health discussion was considered good as a method of health promotion. Support material was good to be used but also needs enough time. Program of changes helps to see goals and ways to do changes more concrete. Occupational health discussion is a motivating method for vocational school students to do changes in their health habits. Discussion should already be introduced in the last class of comprehensive school so that the best workability and functional capability could be guaranteed. This is the way to notice the demands of future work at the earliest stage possible. The education related to health and functional capability should be in greater role in vocational schools. Teachers and public health nurses should cooperate promoting the workability and functional capability among young people....

Time beside a patient - Time allocated for direct nursing care in the hospital services provided by the city of Vantaa The aim of this thesis is to provide a preliminary report on the time used by the nursing personnel for direct nursing care in the hospital services provided by the city of Vantaa. The thesis was carried out as a part of the development project called Toimiva sairaala ― Living Lab between the hospital services provided by the city of Vantaa and Laurea University of Applied Sciences, Tikkurila unit, in which innovative operational models are developed in co-operation with technology companies. The aim of the thesis was to find out how many working hours are used for direct nursing care and how the working hours are allocated to different functions in direct nursing care. In the study, direct nursing care was defined as work taking place in the immediate interaction and contact with patients. Direct nursing care included nursing care provided by the side of a patient, such as feeding, hygiene care, medication, exercise, procedures, monitoring of a patient’s status, patient guidance, emotional support given to a patient and planning and assessing the treatment together with the patient. Indirect nursing care was defined as work taking place in groups of several patients and work where a nurse was not in direct contact with the patient. In order to collect data for the thesis, a work-time monitoring form was prepared. The monitoring form was pretested and accepted by the client organisation. The data for the thesis was collected from ten wards of the hospital services of Vantaa. The study was implemented by using a direct time measurement and observing systematically the nursing personnel (N=23). With the help of the applied activity-based costing, it was entered in the work-time monitoring form how the nursing personnel’s working hours were divided into various components of nursing care. Observations were mainly made during morning shifts. The data was recorded as files in Microsoft Excel format and analysed by utilising the features of the Excel format. According to the results, every third patient contact lasted less than a minute. In the pre-testing phase, it was noted that direct nursing care typically includes short visits to a patient. The shortest calculatory temporal adverb used in the study was a minute. In the analysis phase, patient contacts lasting less than a minute were left out of the total work-time calculation but they were taken into account in the content classes of direct nursing care and in the patient contact times. According to the results, the proportion of direct nursing care was 30.2% of the total work-time. Practical nurses used 33.4% of their working hours to direct nursing care, nurses 27.5% and physiotherapists 28.7%. The main emphasis of nursing care should be on direct nursing care. In order to increase the amount of direct nursing care, solutions are found inside the work communities and with the help of co-operation between the personnel and innovation companies. The needs of the parents of 7th crade parents for home-school collaboration The purpose of this Bachelor’s thesis is to describe what kind of needs the 7 th grade parents have for home-school collaboration in Koivukylä-Korso area. The objective of this study was to gather information on both needs and concerns that parents have and to clarify how those differ between schools. The electronic Webropol inquiry was answered by 108 parents from three different schools. The material was analysed with the help of the SPSS program and quantitative method. The results show that parents hoped for more activities organised by the schools, such as new events, as well as increased communication between home and school. The theme meetings arranged by the school and discussions about different themes with other parents were considered important. The results did not show many differences between schools; some differences in the sufficiency of the school’s activities and which subjects the parents wanted to discuss with each other. In the future it is important to improve home-school collaboration....

Intramuscular medication – review of technical knowledge and safe execution The purpose of this Bachelor’s thesis is to clarify intramuscular medication and the practices related to it. In addition the aim is to unify intramuscular injection practices in theory. One of the main emphases in this thesis was chosen injection which is for the present rather unknown method to give an intramuscular injection. Our goal is to raise wider awareness about ventrogluteal injections among the nursing staff. The thesis consists of a theoretical and a practical part. In the theoretical part we discuss special features of the most common intramuscular injection sites and other common practices related to injection techniques. One important section in the theoretical part is safety in medication, which includes possible risks and their minimisation, aseptic practices and injection equipment. The significance of guidance in connection with injecting is also brought up in the theoretical part. The practical part of the thesis is a poster. The theme of the poster is ventrogluteal injection. The purpose of the poster is to make this injection technique more approachable and practical. The poster was presented at Sairaanhoitajapäivät (Finland’s largest training event and exhibition for nurses) in 2013. The authors asked for face-to-face and written feedback. Intramuscular medication – review of technical knowledge and safe execution The purpose of this Bachelor’s thesis is to clarify intramuscular medication and the practices related to it. In addition the aim is to unify intramuscular injection practices in theory. One of the main emphases in this thesis was chosen injection which is for the present rather unknown method to give an intramuscular injection. Our goal is to raise wider awareness about ventrogluteal injections among the nursing staff. The thesis consists of a theoretical and a practical part. The theoretical part is based on current literature.
