Knowledge test of consciousness monitoring to nurses This thesis is part of nursing quality developing program that started in 2013 and will continue to year 2017. The program is organized by the Helsinki and Uusimaa hospital district (HUS), HYKS Hospital operative industry and Laurea University of applied sciences. The product of this thesis was a knowledge test of consciousness monitoring which was made to quality developing needs of Töölö hospital neurosurgery clinic. The goal of this knowledge test was to develop the care of neurosurgical patients by motivating nurses to develop their own occupational skills. This thesis was made by two nurse students, supervising teacher and experts of Töölö hospital neurosurgery clinic in 2013. Neurosurgery is a field of medicine in which operations focus on patient’s brain or central nervous system. To neurosurgical patients is common that intracranial pressure (ICP) will rise because of cerebral hemorrhage or because of some another brain trauma. As the intracranial pressure rise the patient’s level of consciousness decreases and the low brain tissue perfusion threatens patient’s such vital functions as breathing, blood circulation, body temperature and cognitive functions. Thus the consciousness monitoring is emphasized on neurosurgical patients. The questions to the knowledge test are taken from this thesis theoretical frame of references. Attached separately are the questions to the knowledge test and recommended practices in consciousness monitoring of a neurosurgical patient in intensive care unit made by Patané and Rekomaa in 2009 which works as the basis of questions in the knowledge test. Theoretical frame of references includes anatomy of central nervous system, definition of consciousness, diseases that decrease level of consciousness, instructions on consciousness monitoring and Aura-model. The knowledge test questions made on this thesis are calibrated on three levels based on Aura-model, so the knowledge test can benefit just graduated nurse and more competent nurse with more years of experience. There was made 76 questions to the knowledge test. The test will show ten questions at the time to test taker so same test taker can take the test multiple times....

High blood pressure prevention and treatment - Electronic mental health portfolio This thesis is part of a common Led Project between Keski-Uudenmaan Sopimuskoti and Laurea UAS, which discusses arterial hypertension, stroke, diabetes and obesity. The project was made in co-operation with Laurea University of Applied Sciences and Keski-Uusimaa Sopimuskoti ry. The purpose was to collect an electronic portfolio, which discusses the prevention and care of the arterial hypertension for mental health patients and the nurses of Keski-Uusimaa Sopimuskoti. The aim was to increase the knowledge of the prevention and care of arterial hypertension among mental health patients of Sopimuskoti and the nurses. Another aim was to create a tool for mental health work, which can be used for mental health patients in self-care guidance. The aim was also to provide a presentation about high blood pressure to the target group at Sopimuskoti. The thesis consists of the theoretical part and the electronic portfolio that has been designed for the mental health patients and nurses. With the help of the links in the electronic portfolio the mental health patients and the nurses will find additional information about the subject and are able to update the contents of the portfolio. The authors visited Sopimuskoti at the beginning of the year 2012 to give a presentation about high blood pressure to the mental health patients and nurses. The mental health patients and nurses were allowed to participate in the questionnaire inquiry asking about their knowledge of arterial hypertension prevention and treatment. The questionnaire inquiry included an open question asking about what they want to know about arterial hypertension. The authors created an electronic portfolio from the results of the inquiry. Mental health patients and nurses can quickly and easily learn about high blood pressure from the portfolio. The knowledge of arterial hypertension among mental health patients and nurses as well as their interests directed the contents and structure of the electronic portfolio. The authors did not use any scientific concepts in the thesis, so the thesis was made simple and it is based on the requirements concerning written patient education material. The theoretical part of the thesis was limited to deal with the definition of blood pressure and arterial hypertension and the high blood pressure diagnosing, reasons, symptoms and consequences. In addition, the theoretical part discusses high blood pressure and drug-free treatment, blood pressure measurement, monitoring and self-monitoring. The medical treatment was not focused on in the thesis. Tämän opinnäytetyön tavoitteena oli tuottaa liikuntatapaturmia ennaltaehkäisevä esite. Opinnäytetyön kohderyhmänä olivat 15-18-vuotiaat ammattiopisto-opiskelija nuoret. Esite toteutettiin osana NOPS 2010-2012, Nuoren työ ja toimintakyvyn edistäminen ammattiopistoissa hanketta. Hankkeen päätavoitteena on kehittää toimintamalli, jossa ammattikorkeakoulun terveys- ja sosiaalialan opiskelijat kytketään osaksi ammattiopistojen terveyttä edistävää toimintaa. Hankkeen tavoitteena on edistää nuoren terveellisiä elämäntapoja sekä turvallista ja terveellistä työn tekemistä. Tämän opinnäytetyön teoreettisena pohjana on käytetty nuorten liikuntatapaturmiin liittyvää tietoa nuorten elintavoista, nuoren kehityksen keskeisistä piirteistä, sekä ympäristön vaikutuksista liikuntatapaturmiin.
Esite toteutettiin toimintatutkimuksellista lähestymistapaa käyttäen. Esitteen yhtenä kehittämismenetelmänä olivat kohderyhmänedustajien ja asiantuntijoiden teemahaastattelut. Asiantuntijoiden haastattelut toteutettiin yksilöhaastatteluna ja kohderyhmän edustajien haastattelu ryhmähaastatteluna. Haastatteluiden tarkoituksena oli kerätä tietoa siitä, millainen on hyvä nuorille suunnattu liikuntatapaturma esite. Haastatteluiden tulokset olivat yhdenmukaisia; esitteen sisällön on oltava helppolukuista, ulkoasun tulee olla vetoava ja tekstiä ei saa olla liikaa. Haastattelujen jälkeen esitteestä valmisteltiin luonnos, joka lähetettiin arvioitavaksi kohderyhmän edustajille. Kirjallinen arviointi saatiin sähköpostin välityksellä. Arvioinnin pohjalta tuotettiin esite valmiiksi.
Valmis esite sisältää yleistä tietoa liikuntatapaturmista sekä liikuntatapaturmien ennaltaehkäisystä. Esite on saatavissa Laurea Hyvinkään NOPS -sivustolla ja sen voi tulostaa kuka tahansa. Tarkoituksena on, että esitteestä voivat hyötyä myös muut kuin kohderyhmä. Opinnäytetyön kehittämishaasteena oli, miten saisi toteutettua mahdollisimman kattavan ja asiantuntevan, mutta samalla nuoria kiinnostavan esitteen. Nuorten liikuntatapaturmat aihealueena on laaja ja siitä saatava tieto on rajallista, joten tulevaisuudessa kehittämishaasteena on aiheesta saatavan tiedon lisääminen ja sen välittäminen tietoa tarvitseville....

Needs for parenting support - Development proposals for supporting the parents with children under school-age This thesis was done in cooperation with the project workers of the parenting support development programme of the City of Vantaa. The purpose of the thesis was to describe the needs for parenting support expressed by the parents with children under school age in Vantaa and find viewpoints on parenting support. The aim was to find out through a questionnaire inquiry about the needs for parenting support, the quality of support they received from professionals and development proposals expressed by the parents to improve the services. The thesis is based on the questionnaire inquiry done on the web pages of the city of Vantaa which was originally developed to improve the parenting services.The questionnaire inquiry was mainly focused on the parents’ opinions on matters requiring change instead of wheter or not the parents were content with the services already offered. The research data was collected on the Internet through a webropol questionnaire inquiry performed among the parents in Vantaa (714 parent answered the questionnaire). The inquiry consisted of 19 different questions, which were mainly open questions. The answers of two open questions were selected to the thesis according to the study questions. The results of the inquiry were analysed with inductive content analysis. The thesis concentrates on the parents with children under school age. The results show that only about 24% of the parents had seeked parenting support. The support had mainly been seeked at child health clinics and day care centres. Moreover, the help has been obtained from friends, from their own parents and parents-in-law and from the Internet. Among the parents who returned the questionnaire inquiry 19% thought that parenting support in Vantaa has been sufficient. Only 89 respondents answered the question whether they were dissatisfied and what they would have expected otherwise. The parents were mostly dissatisfied with the difficult availability of consulting appointments and with their short duration as well as with the number and professional skills of the staff. The definition staff implies the staff of the social services, health care and school system. The question how parents could be better supported was answered by 404 respondents. The development proposals concerned the clarity of the educational guidelines, increasing the number of staff and their appointment times, receiving assistance and support faster, information on services, prevention and early intervention, activites as well as better flexibility in working life. The information provided by the thesis can be used in the development of the services in Vantaa so that they provide better parenting support. The reason for why people do not find the existing services could be a topic for further research....

Significance of psychoeducation to the discrimination and the treatment motivation of psychosis patients as described by nurses This Bachelor’s Thesis is based on quality improvement project between Laurea UAS, the business unit of the HUS/HYKS Psychiatry and The Key to the Mind project (Mielen avain), which aims to develop the common needs of mental health nursing. This thesis is part of the project that is implemented between the years 2011 and 2015 and its purpose is to serve the needs of developing common mental health nursing practices. The aim of the thesis is to describe the discrimination that first episode psychosis patients often face as well as the motivation and lack of motivation that these patients might have. The other goal is to report how psychoeducation can be used as a tool to decrease discrimination and to increase the patients’ treatment motivation. A quality research method was used to find out the results. Five registered nurses (N=5) who work at the outpatient center of Peijas Eastern rehab and work daily with the first episode psychosis patients were interviewed. Interviews were made by using the individual theme interview and the results were analyzed by inductive content analysis. The results indicate that the patients face discrimination mostly within the health care system, their circle of friends, as well as in work, and study communities. It is possible to decrease the discrimination by giving the right kind of information about the illness and its medication to those groups, and also by supporting the patients’ daily life. Lack of motivation in patients can be seen in the denial of the illness, difficulties in committing to the treatment, averseness and the unwillingness of using medication. The motivation can be improved by giving information about the illness and medication to the patients, creating confidential care relationships between nurses and patients and setting concrete goals. The results of the thesis show also that it is applicable to say that psychoeducation can be used as a tool to increase the wellbeing of the patients and it should be used as a part of successful treatment....
