Implementation of the exercise events in vocational college, NOPS 2010 -2012, Promotion of work ability and functional capability among young people in vocational colleges This bachelor’s thesis is part of the NOPS project (Promotion of work ability and functional capability among young people in vocational colleges) which has been carried out between 2010-2012. This project is helping to developed methods of promoting health among students within their chosen vocation. NOPS- project’s main organizer is Laurea University of Applied Sciences. The project partners are Hyria and Keuda College in Helsinki, Järvenpää, Nurmijärvi and Mäntsälä. The target group of the project consists of students at the vocational colleges. This project is funded by Ministry of Social Affairs and Health and Laurea Hyvinkää. The purpose of this thesis was to find out that, how NOPS project Laurea’s students organized sports events have affected the vocational college students’ health. The thesis examined also, how health events practical implementation works. The theoretical basis of the thesis formed by: information, motivation and skill - Health Promotion (IBM) model, customer-based guidance and health exercise. NOPS’S project’s questioners were used to obtain and collect data at the beginning and the end of each sports events. The initial questioner was designed to obtain information on the student’s current knowledge on exercise performance, physical activity, health effect and the effects of exercise to the public health/ diseases. The second questioner looked at how the event changed student’s knowledge about physical activity-related information, as well as, how they perceived the health care skills and their motivation after the event. It also explored the effectiveness of the implementation of the sports events. There were seven events which were attended by a total of 70 students’ questioners (Total of 140 questioners’ inc. initial and the final questioners). All the answers were analyzed with SPSS- statistic program and the results were described as frequencies and as a percentage. During the health events, student’s information of sports and exercise were increased. The greatest increase was about information of breast cancer diseases (percentage change +23,4 %), and the information of smoking and physical commonality hazards (percentage change +16,5 %) as well as the information of physical hard work health effects (percentage change +13,5 %). During the events student’s ability chose healther options increased 67,5 %. 70,0 % of students took something positive from the events. 77,1 % of students involved in the experiment were of the opinion that, the events carried out were clearly understood and correct. 63,0 % of students were of the opinion that, the events were sufficiently operational. There were several challenges for the development of the events. Which are; there should be better methods for promoting health and how there can affect the student’s in the college and help them increase their exercise. There must be also some developments in supporting students to take care of ergonomics in their workplace. Students should be educated in the public health/ diseases and their prevention during their studies. Säännölliseen verenluovutukseen liittyviä motivaatiotekijöitä

Tämän opinnäytetyön tavoitteena oli selvittää säännöllisten verenluovuttajien luovutusmotivaatioon vaikuttavia tekijöitä ja näin auttaa Suomen Punaisen Ristin, Veripalvelua kehittämään verenluovutustapahtumaa. Opinnäytetyössä tarkastellaan Veripalvelutoimistojen asiakkaita ja se on toteutettu yhteistyössä Suomen Punaisen Ristin, Veripalvelun kanssa. Verenluovuttajista on aikaisemmin tehty vähän suomalaisia tutkimuksia.
Veripalvelu teki verkkokyselylomakkeen, valitsi kyselyn osallistujajoukon ja suoritti aineiston keruun. Opinnäytetyön tekijänä sain aineiston raakadatana Excel-taulukkoon sijoitettuna. Opinnäytetyön aineisto saatiin tekemällä verkkokysely 21.3–27.3.2011 käyneille verenluovuttajille. Kysely lähetettiin 2510 verenluovuttajalle joista 914 ilmoitti käyneensä luovuttamassa verta Veripalvelun toimistossa. 803 koki olevansa säännöllinen verenluovuttaja. Opinnäytetyöni tutkimusmenetelmä on kvantitatiivinen. Vastaukset on ryhmitelty sukupuolen ja ikäryhmän mukaan.
Vastausten perusteella verenluovuttajat ovat varsin yhtenäinen ryhmä kun tarkastellaan heidän motivaatiotekijöitään luovuttaa verta säännöllisesti. Sukupuolten välillä ei ollut havaittavissa suuria eroja. Ikäryhmiä tarkastellessa oli nähtävissä hajontaa nuorempien ja vanhempien ikäryhmien välillä. Nuoremmat verenluovuttajat olivat vanhempia enemmän kriittisempiä verenluovutustapahtuman suhteen ja he olivat enemmän hakeneet tai saaneet tietoa verenluovutuksesta tiedotusvälineiden kautta. Säännölliseen verenluovutukseen verenluovuttajia motivoi ennen kaikkea auttamisen ilo ja halu tehdä hyvää.
Jatkotutkimusaiheiksi nousi tutkia ammatin vaikutuksia verenluovutusmotivaatioon. Ovatko sosiaali- ja terveysalalla työskentelevät aktiivisempia verenluovuttajia kuin muihin ammattiryhmiin kuuluvat? Myös eri ikäryhmien tarkemmasta vertailusta verenluovutusmotivaatiotekijöiden kanssa Veripalvelu saisi hyödyllistä tietoa joka auttaisi kehittämään verenluovutustapahtumaa....

Promoting the health of hip surgery patients in special healthcare This thesis is part of a development project called LbD, made by assignment of the Peijas Hospital Surgical Unit. The purpose of the project was to improve the quality of working life and self-learning of the writers of this thesis. At Laurea, teaching and learning take place in compliance with an LbD model (Learning by Developing), in which students participate in actual working life development projects and assignments. The original purpose with this thesis was to review general limited to patients coming in for planned hip surgery, because such patients are a large group among hip endoprosthesis surgery patients. The thesis was implemented as a literary review with the purpose of compiling information about the subject matter and finding answers to the following research questions: 1) What does promoting health mean in a special healthcare setting? 2) Who carries the responsibility for promotion of health in special healthcare? and 3) Under what circumstances does the promotion of health take place? Databases used include Laurus, Medic, Volter, Josku, Linda and EBSCO Cinahl. The search keywords were determined based on the research objectives. All the obtained materials were entered into a research table. The materials were analysed based on the research questions and the objectives set for the thesis. A decision to implement a new search was made after the results of the first search had been studied. This time, the target group was patients coming in for planned hip surgery. According to the studies found, in special healthcare the term promotion of health refers to counselling on health that aims at supporting the patient in making choices that will improve their health and prevent illnesses. Key issues in promoting health are offering information about habits that have a negative impact on the patient's health and their potential consequences. In the studies, promotion of health was considered a general level objective that is promoted by special healthcare professionals by addressing health risks, providing instructions on treatment, guiding the patients, supporting their own resources and identifying their health risks. The answer found to the research questions of who offers guidance and under what kind of circumstances was that the person in charge of the special healthcare of a hip surgery patient is the physician or another healthcare professional who has sufficient information about the illness and how to manage the patient. Preoperative guidance and support are the most important when promoting the health of hip surgery patients. The studies state that special challenges in promotion of health in special healthcare include lack of support from the management and lack of management commitment. Another challenging issue is addressing health problems that are not the actual reason why the patient came in for surgery. Major challenges occurring in the care of hip surgery patients include optimally taking into account the patient's special characteristics, particularly when planning postoperative care and support, because the advice given to patients varies a great deal. Promotion of the health of hip surgery patients in special healthcare could be improved in the future by researching patients in groups divided by their need of surgery....

Alle 3-vuotiaiden lasten kotitapaturmien ennaltaehkäisy

Suomessa lapsille sattuu vuosittain arviolta 100 000 lääkärikäyntiä vaativaa tapaturmaa, joista noin 10 % vaatii sairaalahoitoa. Tapaturmatilastojen ja teoriatiedon mukaan alle 3-vuotiaiden lasten yleisimpiä kotitapaturman aiheuttajia lapsesta ja hänen kehitystasostaan riippuen ovat putoamiset ja kaatumiset, tukehtumiset ja myrkytykset, hukkumiset ja palotapaturmat sekä sähköiskut. Kotitapaturmia voidaan parhaiten ehkäistä riittävällä valvonnalla ja turvallisen kasvuympäristön luomisella.
Tämän toiminnallisen opinnäytetyön tarkoituksena oli tuottaa valokuvadiasarja Hyvinkään kaupungin verkkopalvelu Nettineuvola Nelliin koskien alle 3-vuotiaiden lasten kotitapaturmien ennaltaehkäisyä. Opinnäytetyön aihe saatiin Hakalan asuinalueen neuvolasta. Opinnäytetyön viitekehyksessä käsitellään neuvolatoimintaa ja nettineuvolaa, perheen ja vanhemmuuden merkitystä, lapsen fyysisistä ja neurologista kehitystä, tyypillisiä kotitapaturmia ja niiden preventiomalleja sekä audiovisuaalisen materiaalin työstämistä. Valokuvadiasarjan käsikirjoitus ja sisältö perustuvat viitekehyksessä olevaan tietoon.
Valokuvadiasarjan tavoitteena on havahduttaa pienten lasten vanhemmat sekä terveydenhuoltoalalla työskentelevät huomaamaan tapaturmien ennaltaehkäisyn merkitys lapsen turvallisuuden ja normaalin kehityksen takaamiseksi ja antaa konkreettisia esimerkkejä tapaturmien syytekijöistä kotona. Tavoitteenamme on lisäksi, että materiaalia voitaisiin käyttää yhtenä ohjausmenetelmänä neuvolassa....

Tämän opinnäytetyön tarkoitus oli kehittää ohjaustyötä erässä eteläsuomalaisessa lastensuojelun erityisyksikössä. Työ toteutettiin yhteistyössä yksikön henkilökunnan kanssa juurruttamisen menetelmää käyttäen. Kehitettäviksi asioiksi yksikössä todettiin seuraavia tarpeita: rajojen asettaminen entistä selkeämmiksi, ohjaustapojen yhtenäistäminen sekä tiiviimpi yhteistyö ohjaajien kesken.
Ohjaustyön kehittämiseen osallistui yksikön henkilökunnasta puolet, koska tällä menettelyllä työn toteutus oli vuorotyötä tekevässä yksikössä helpoin ja toimivin järjestää. Työn tuotos otettiin kuitenkin koko yksikössä koekäyttöön.
Työn tuotoksena tehtiin muutoksia ohjaustyöhön yksikössä. Muutoksilla keskityttiin yhtenäistämään ohjauskäytänteitä sekä sovittiin muutamista perussäännöistä, joissa ei ole joustovaraa. Kaikenlaiseen ei toivottuun käyttäytymiseen päätettiin puuttua aiempaa tiukemmin ja yhtenäisemmin. Väkivaltaan ja kiroiluun puuttumisessa sovittiin yhtenäiset säännöt. Kaikki työntekijät sitoutuivat kehittämistoimintaan ja työtä arvioitiin ja kehitettiin työyhteisöpalavereissa kolmen viikon välein.
Tehtyjen muutosten tavoitteena oli saada yksiköstä viihtyisä, turvallinen ja hyväsyvä asu-misympäristö siellä hoidossa oleville lapsille ja nuorille. Paikka jossa jokainen voi olla oma itsensä ja tulla hyväksytyksi omana itsenään. Työntekijän näkökulmasta työskentelyn mielekkyys lisääntyi ja kuormittavuus väheni.
Muutoksen seurauksena yksikössä rajoitustoimenpiteet lisääntyivät, koska lasten ja nuorten oli vaikea sopeutua kiristettyihin sääntöihin. Ajan kuluessa odotetaan arjen osastolla rauhoittuvan ja rajoitustarpeiden vähenevän.
Kehittämisprojekti tiivisti ohjaajien yhteistyötä ja sitoutti noudattamaan yhteisesti sovittuja toimintatapoja. Yhteishenki yksikössä lisääntyi ja työmotivaatio kasvoi kehittämisen ja yksilöllisen vaikuttamisen myötä.
Jatkotutkimuksessa voisi myöhemmin perehtyä siihen, onko juurruttamisen menetelmä juurtunut yksikössä pysyväksi menetelmäksi tuottaa uutta tietoa ja kehittää yhteisön työmenetelmiä. Toinen jatkotutkimusehdotus voisi olla kuvin tuetun viikko-ohjelman toteuttaminen. Ohjelma tukisi nyt kehitettyä toimintaa ja selkeyttäisi arjen struktuuria.

Lost Seniors Assistance Project: How senior cafes may affect an elderly person's well-being This thesis has been carried out in Lohja, Finland for Apuomena ry (Registered Association). Six senior cafeterias were established to Lohja area the help of Apuomena Lost Senior Assistance Project (2011-2013). These cafes are a low-threshold meeting places, which increase the sense of communality, elderly mobility and social interaction. Coffee shops are intended to affect the functional capacity of older people, maintaining physical mobility, and to increase activity. The starting point of work has been to examine the effects of senior cafes on the well-being among older people in the Lohja area. With the help of a structured questionnaire survey, the thesis studied how visiting senior cafes affect elderly person's functional ability, loneliness and communality. In addition, to find out what kinds of things or experiences cafe has brought to the respondents’ life and what kind of things would they like to provide in the future. The results were analyzed by PASW 18.2 for Windows, by using both frequency and cross tabulation methods. According to the findings, by the well-being of elderly people has increased by visiting these cafes when well-being is considered as a multidimensional entity. The respondents’ inclination to meet other people had increased the more time they spent at the cafes. The respondents were generally satisfied with the current social personal relationships compared to personal relationships prior to participating in the senior cafe activity. The respondents indicated the regular meeting with friends helped to alleviate their loneliness. Senior cafes have increased the respondents’ sense of communality by bringing discussion groups, music and physical activity to their life. The results show that participants find communality as an important issue. Café has become meeting point for the participants and for this reason continuation of café was seen an important. Respondents wished to have different experts offer various lectures on health and aging-related topics. Senior coffee shops have positively affected the well-being of the elderly. Examining the results, it is noted that the survey respondents´ are more satisfied with their social relationships compared to those times when the coffee shop activity was not a part of their life. As a further research topic, it was suggested that re-using the questionnaire would increase the measure of reliability. The questionnaire could include an additional question about how long the respondents have been visiting the senior coffee shops....

Motivational interviewing among abortion patients – a literature review This thesis is a part of the quality improvement project between Laurea UAS and the fields of gynaecology and obstetrics of the Hospital District of Helsinki and Uusimaa (HUS) and Helsinki University Central Hospital. The project was started in the year 2007 and it is an evidence-based development project. This project aims to develop professional qualifications, the quality of nursing and learning in a development project. The goal of this thesis was to write a literary review about motivational interviewing as a method to the gynaecology outpatient clinic of Kätilöopisto Maternity Hospital. This thesis was assigned by the gynaecology outpatient clinic of Kätilöopisto Maternity Hospital, who wanted to know more about motivational interviewing and its suitability in treating abortion patients. The literature review was started in May 2012. The presentation of the thesis plan happened in May 2012 at Women’s Hospital. The thesis was worked on during Spring 2012 and especially in Autumn. The database search was started in the Autumn 2012 after a data retrieval workshop where a discussion about starting data retrieval and keywords were had. The search was made by utilizing national and international databases. The keywords used were defined together by the members of the thesis group. Laurus, Medic, Aleksi and Linda were used out of national databases. Out of international databases eBSCO and Ovid were used. The data was analyzed among the group and the latest evidence-based sources were chosen based on how well they answered the research questions. This literature review consists of information about motivational interviewing as a method by using the latest national and international evidence-based sources. This thesis contains information about the principles, techniques and ethical considerations in motivational interviewing as well as its application in different situations. Motivational interviewing is based on equality and cooperation between the patient and the instructor where the patient plays a major role. The amount of research on motivational interviewing in nursing has increased in the past few decades and the results have been encouraging. However, desired results have not been achieved in all patient groups. The results show that motivational interviewing can be used in various different ways in patient-nurse counselling. Abortions present state is discussed by using studies and charts as reference. According to the charts the amount of abortions done in Finland has stayed about the same, but the number of intentional recurring abortions among patients have increased. The goal of this thesis is to examine if motivational interviewing could be used as a tool to decrease the number of these intentional recurring abortions. Motivational interviewing could be beneficial in gynaecology outpatient clinic of Kätilöopisto Maternity Hospital in short one- to- four interventions. The situations where the patient has no desire to change his ways are however challenging....
