Motivational interviewing among abortion patients – a literature review This thesis is a part of the quality improvement project between Laurea UAS and the fields of gynaecology and obstetrics of the Hospital District of Helsinki and Uusimaa (HUS) and Helsinki University Central Hospital. The project was started in the year 2007 and it is an evidence-based development project. This project aims to develop professional qualifications, the quality of nursing and learning in a development project. The goal of this thesis was to write a literary review about motivational interviewing as a method to the gynaecology outpatient clinic of Kätilöopisto Maternity Hospital. This thesis was assigned by the gynaecology outpatient clinic of Kätilöopisto Maternity Hospital, who wanted to know more about motivational interviewing and its suitability in treating abortion patients. The literature review was started in May 2012. The presentation of the thesis plan happened in May 2012 at Women’s Hospital. The thesis was worked on during Spring 2012 and especially in Autumn. The database search was started in the Autumn 2012 after a data retrieval workshop where a discussion about starting data retrieval and keywords were had. The search was made by utilizing national and international databases. The keywords used were defined together by the members of the thesis group. Laurus, Medic, Aleksi and Linda were used out of national databases. Out of international databases eBSCO and Ovid were used. The data was analyzed among the group and the latest evidence-based sources were chosen based on how well they answered the research questions. This literature review consists of information about motivational interviewing as a method by using the latest national and international evidence-based sources. This thesis contains information about the principles, techniques and ethical considerations in motivational interviewing as well as its application in different situations. Motivational interviewing is based on equality and cooperation between the patient and the instructor where the patient plays a major role. The amount of research on motivational interviewing in nursing has increased in the past few decades and the results have been encouraging. However, desired results have not been achieved in all patient groups. The results show that motivational interviewing can be used in various different ways in patient-nurse counselling. Abortions present state is discussed by using studies and charts as reference. According to the charts the amount of abortions done in Finland has stayed about the same, but the number of intentional recurring abortions among patients have increased. The goal of this thesis is to examine if motivational interviewing could be used as a tool to decrease the number of these intentional recurring abortions. Motivational interviewing could be beneficial in gynaecology outpatient clinic of Kätilöopisto Maternity Hospital in short one- to- four interventions. The situations where the patient has no desire to change his ways are however challenging....

Verenpainetaudin omahoidon onnistumista edistävät tekijät: kirjallisuuskatsaus

Tämän opinnäytetyön tarkoituksena oli kirjallisuuskatsauksen keinoin selvittää verenpainetaudin omahoidon onnistumiseen vaikuttavia tekijöitä. Tavoitteena oli löytää ja kuvata tärkeimmät tekijät, joihin puuttumalla verenpainetautia sairastavan omahoitoa voidaan tehostaa. Tämä tutkimus toteutettiin osana Vantaan kaupungin terveyspalveluiden, Remote Analysis –yhtiön ja Laurea ammattikorkeakoulun Tikkurilan yksikön yhteistyöhanketta, jossa kehitetään verenpainetautia sairastavan terveydenhuollon asiakkaan omahoidon tukemista.
Tässä kirjallisuuskatsauksessa käytettiin systemaattista lähestymistapaa tutkimusten valinnassa ja analysoinnissa, mutta alkuperäistutkimusten metodologisia valintoja ei arvioitu eikä niiden tuloksia tarkasteltu tilastollisesti. Aineisto haettiin yhdestä suomalaisesta (Medic) ja kahdesta kansainvälisestä viitetietokannasta (CINAHL ja PubMed). Analysoitavaan aineistoon kelpuutettiin kahdeksan tutkimusartikkelia, jotka on julkaistu vuosina 2007 – 2012. Kaikissa tutkimuksissa lähestymistapa oli kvantitatiivinen.
Tämän tutkimuksen perusteella verenpainetautia sairastavan potilaan voimakas pystyvyyden tunne on yhteydessä parempaan omahoidon toteutumiseen. Omahoito onnistui keskimäärin paremmin myös niiltä, jotka olivat sairastaneet verenpainetautia jo pidemmän aikaa. Myös potilaan itsensä kotioloissa suorittamat verenpaineen mittaukset näyttäisivät tehostavan omahoitoa. Lisäksi potilaan terveydentilastaan huolehtimiseen suuntaaman kiinnostuksen eli terveysaktiivisuuden määrä vaikuttaa suoraan omahoitoaktiivisuuteen. Sen sijaan potilaan masennusoireiston tunnistamatta jääminen johtaa usein omahoidon epäonnistumiseen.
Tämän kirjallisuuskatsauksen perusteella terveydenhuollon omahoitointerventio kannattaisi aloittaa intensiivisesti heti verenpainetaudin diagnosoinnin jälkeen. Näin saataisiin alulle potilaan yksilöllinen terveysaktiivisuuden kehitysprosessi ja siitä seuraava omahoitovalmiuksien kasvaminen. Omahoitovalmennus tulisi räätälöidä yksilöllisesti potilaan lähtökohdat huomioiden ja sen sisältöä tulisi päivittää potilaan saavuttaessa vähittäin uusia valmiuksia. Potilaiden itsensä suorittamat säännölliset verenpaineen kotimittaukset ja niiden tuloksista raportointi sekä ammattihenkilöltä saatava mittauksiin liittyvä palaute tulisi liittää osaksi verenpaineen omahoitoa. Omahoidon onnistumisen tukemiseksi tulisi verenpainetautia sairastavien potilaiden masennusoireistoa pyrkiä tunnistamaan varhaisessa vaiheessa, esimerkiksi seurantakäyntien yhteydessä tehtävien masennustestien avulla, ja mahdollista masennusoireistoa hoitaa.
Potilaiden pystyvyyden tunteen tai terveysaktiivisuuden tason tunteminen voisi lisätä hoitohenkilökunnan valmiuksia yksilöllisen omahoidon tuen antamiseen. Tähän tarkoitukseen olisi tärkeää kehittää mittareita, joilla potilaiden pystyvyyttä ja aktiivisuutta voitaisiin kartoittaa. Lisäksi jatkossa olisi hyödyllistä selvittää hoitohenkilökunnan valmiuksia omahoidon tukemiseen ja mahdollisen lisäkoulutuksen tarvetta....

This quantitative research concerns the importance of peer support, experienced by breast cancer patients. The aim of the research was to find out how breast cancer patients are informed by patient organizations. Furthermore, our aim was to study if breast cancer patients are well informed of peer support, and during which period of their illness patients especially need peer support. In our research, we cooperated with Espoon Järjestöjen Yhteisö ry Organisation and their Vertaisresepti project. Our aim was to inform the organization on the importance of peer support, and also on how patients are informed by the hospital staff on third-sector services. Espoon Järjestöjen Yhteisö ry Organisation can use the research results to develop their work. The target group of the study consists of breast cancer patients who have been operated for the first or second time. The research was carried out with a questionnaire at Naistenklinikka Hospital, department R31 (N=65). The research material was statistically analysed with SPSS. Most patients experienced that they were sufficiently informed on patient organisations by the hospital staff. They received most of the information from leaflets and guide books written for patients. In their opinion, peer support from Internet forums, group activities, and tutors was equally useful. Most patients expressed that they needed peer support especially during breast cancer treatments and follow-up treatment periods. The patients especially needed peers to share their feelings with. For further study, we suggest a research with the target group of follow-up breast cancer patients at a later phase of illness. Vuorovaikutuksen estyminen potilaan ja hoitajan välisessä hoitosuhteessa

Opinnäytetyöni tarkoituksena oli kuvata potilaiden ja hoitajien välisen vuorovaikutuksen estymistä erilaisissa hoitotilanteissa, ja toimivan vuorovaikutuksen merkitystä potilaan hyvän hoidon kannalta.
Opinnäytetyön aineisto kerättiin tekemällä kysely hyvinkään sairaalan kirurgian vuodeosaston 1-solun hoitajille. Lisäksi työskennellessäni osastolla tein joitakin havaintoja liittyen opinnäytetyöni aiheeseen. Aineisto jäi melko vähäiseksi hoitajien lomien ja kiireisen työtahdin takia. Aineisto analysoitiin käyttämällä sisällön analyysiä.
Saamistani tuloksista kävi ilmi, että vuorovaikutus hoitajan ja potilaan välillä estyy monissa tilanteissa. Suurimpana ongelmana toimivan vuorovaikutussuhteen luomisessa koetaan kiire osastolla. Kyselyyn vastanneet hoitajat työskentelevät päivystyssolussa, joten työtahti on melko kova ja kiireinen. Kyselyssä ilmeni myös muita vuorovaikutussuhteen toimivuuden hankaloittajia, joita teoriassakin on mainittu. Hoitajien vastauksista tuli hyvin ilmi sekin, että toimiva vuorovaikutussuhde on huomattava osa hyvää hoitoa ja hoidon laatua.
Teoriatiedonkin perusteella hyvällä vuorovaikutussuhteella on huomattava merkitys potilaan onnistuneen ja hyvän hoidon kannalta. Toimivan vuorovaikutussuhteen puuttuessa tai estyessä, hoito vaikeutuu ja saattaa pitkittyä.
Opinnäytetyölläni haluan nostaa esiin vuorovaikutuksen merkityksen hoitotyössä. Tulevaisuudessa olisi hyvä panostaa hoitajien mahdollisuuteen kehittää omia vuorovaikutustaitojaan hoitotyötä ja sen ongelmakohtia ajatellen. Toivon opinnäytetyöni herättävän keskustelua ja sitä kautta tuovan myös ratkaisuja hankaliin vuorovaikutustilanteisiin hyvinkään sairaalan kirurgian osastolla....

An audit of structured nursing documentation of a home care unit This thesis was an audit study of nursing documentation in a Finnish home care unit. The research material consisted of home care units’ clients’ (N = 30, n = 20) patient care plans and daily nursing documentation of a one week period. The audit was implemented with an assessment form created by Kaila and Kuivalainen (2008) during the HoiData project. The aim of the study was to evaluate nursing documentation with minimum criteria of the assessment form and point out potential improvements in documentation based on results. The research group was constituted of patient recordings from those long-term home care units’ clients, who were visited by home care nursing staff more than three times per week. Clients meeting the research criteria were thirty persons of which the head nurse chose twenty clients by random sample selection method. The privacy of the clients was respected during the research and those who participated in study were asked a written permission to use their patient records in documentation audit. The results showed that the home care units’ documentation was in a good level and the targets of improvement were clearly on display. The prepared headlines (need of care, aim of care and planned nursing procedures) provided by the system were often used in the patient care plans and daily documentation. However, they were usually not clarified with written text by nursing staff. Because of the already existing titles (need of care, daily documentation and planned nursing procedures), these were always documented in sections pointed to them. The documentation in these sections was excellent and clients’ perspectives had been taken into consideration. Those parts of the nursing process that had no separate segments in the documentation system (implemented nursing procedures, outcome of care and assessment of care) were found out to be documented inadequately. Generally, the patient care plans were thorough and patient-oriented. The recommendation based on results of the audit is to improve nursing documentation of the home care unit especially in patient care’s outcome and assessment documentation. Home care units should jointly create segments for outcome and assessment of care, which correlate the needs of nursing documentation. The work community should emphasise the meaning of reciprocal learning and give feedback on documentation know-how. Experts of different levels can complete and improve their abilities together. As proposals for further development, it is suggested that an audit consistent with the assessment form to be carried out regularly and nursing staff’s education of documentation to be organized based on these results....

Is it possible to breastfeed? - The significance of the first information in cleft palate baby's nutrition management This thesis is part of the quality improvement project between Laurea University of Applied Sciences as well as Helsinki and Uusimaa hospital district (HUS) HUCH operative department during the years 2007- 2012. The topic of the thesis is special characteristics concerning the cleft palate baby’s nutrition management, which includes the information on cleft palates, as well as their effects on feeding and the development of secure attachment. The thesis includes a literature review and theme interviews. The interviews define the information that is received from the literature. The purpose of this thesis is to provide more information on the first information that parents have received about cleft palates, what kind of instructions they have received of the nutrition of a cleft palate baby and how mothers have experienced the development of interaction with the baby. The goal of this thesis is to describe the experiences of the mothers who have had a cleft palate baby using the topics of our research tasks. The topics of our research tasks are the mothers’ experiences of the information and support received the mothers´ experiences of feeding the baby and the mothers’ experiences of the early interaction between the mother and the baby. The goal of this thesis is that the families with a cleft palate baby and the medical staff who work with these families could benefit from the thesis. A cleft palate always comes as a surprise, because it cannot be seen in fetal ultrasound examination. Thus the families cannot prepare for the challenges brought by the cleft palate beforehand. The theoretical part of the thesis contains information about cleft palates; how cleft palates are born and how they affect the baby's development. There is also information about the baby's nutrition, breastfeeding and about the cleft palate baby's feeding. The authors examined what is good first information and how the information affects the recipient. There is also a section concerning the development of interaction between mother and her baby, because breastfeeding has been considered to be one of the factors in the development of the interaction. The importance of the first information was emphasized in the interviews. The mothers felt that the first information was inadequate and because of that the support received was also inadequate. The mothers felt that no one in the maternity hospital knew enough so that they could have provided any guidance. The right information and support was given by The Finnish Cleft Lip and Palate Centre (HUSUKE). Before the cleft palate had been found all the mothers thought that they would breastfeed the baby. Two of the mothers were told to breastfeed even though it was not possible. The babies´ weights also dropped more than the normal weight loss because of the lack of nutrition. The mothers did not feel that the lack of breastfeeding would have affected negatively the attachment with the baby. Two mothers felt that the attachment with the baby developed even better when there was no breastfeeding or when they decided to stop breastfeeding....

The importance of the first information was emphasized in the interviews. The mothers felt that the first information was inadequate and because of that the support received was also inadequate. The mothers felt that no one in the maternity hospital knew enough so that they could have provided any guidance. The right information and support was given by The Finnish Cleft Lip and Palate Centre (HUSUKE). Before the cleft palate had been found all the mothers thought that they would breastfeed the baby. Two of the mothers were told to breastfeed even though it was not possible. The babies´ weights also dropped more than the normal weight loss because of the lack of nutrition. The mothers did not feel that the lack of breastfeeding would have affected negatively the attachment with the baby. Two mothers felt that the attachment with the baby developed even better when there was no breastfeeding or when they decided to stop breastfeeding....
