Keskosperhe neuvolassa : keskoskansio terveydenhoitajille

Tarve tälle opinnäytetyölle tuli työelämästä. Työn tilasi terveydenhoitaja Jaana Salminen Klaukkalan neuvolasta. Opinnäytetyön tarkoituksena oli luoda Nurmijärven kunnan neuvoloihin toimiva keskoskansio terveydenhoitajille työkaluksi keskosperheiden kohtaamiseen. Tekemämme kansion tavoitteena on yhtenäistää keskosperheiden saamaa neuvolapalvelua Nurmijärvellä. Kansio sisältää keskeisimmät asiat, jotka terveydenhoitajan tulee huomioida silloin, kun asiakkaaksi tulee terveen keskoslapsen saanut perhe. Kansion tavoite on saattaa teoriatieto käytäntöön terveydenhoitajille. Kansio on koottu laajan kirjallisuuskatsaukseen perustuvan opinnäytetyömme pohjalta. Kansio antaa terveydenhoitajille valmiuksia kohdata keskosperhe paremmin, yksilöllisesti, selkeästi ja ymmärrettävästi. Keskoskansio perustuu uusimpaan käytössä olevaan tietoon. Opinnäytetyömme on toiminnallinen.
Keskoskansioita on tehty kahdeksan kappaletta, joista viisi on neuvoloihin, kaksi opinnäytetyön tekijöille ja yksi ohjaavalle opettajalle. Kansio on pehmeäkantinen, joka sisältää muovitaskuissa olevat tekstisivut. Kansiosta löytyy myös toiminnallisia ohjeita kuvineen liittyen keskosvauvan hoitoon ja vuorovaikutuksen tukemiseen.
Kehitysideana ehdotamme, että esimerkiksi opinnäytetyönä voisi tehdä lehtisen keskosvanhemmille, joka sisältää keskosuuteen liittyvät keskeiset asiat. Sitä voisi jakaa Nurmijärven neuvoloissa keskosperheille....

From bystander to participant - Literature review of men's experiences of being a father and their needs of support as well as the role of a public health nurse supporting fatherhood The purpose of the thesis was to examine what kind of support and information needs first time fathers have during the pregnancy and the first year as a father. The purpose was also to find out how a public health nurse can support fatherhood in maternity and child health clinics. The thesis was carried out by using a literature survey based on studies that have been published during the 2000s. The material for the thesis consisted of 23 research papers, scientific articles as well as caring science publications and reports. Based on the results of the studies the author of the thesis studied the experiences that men have during the pregnancy and the first year as first-time fathers. It was also examined how a public health nurse could support men while becoming fathers and in early fatherhood. The thesis was part of a project called Vantaalaisen hyvä mieli. The organizer of the project is the city of Vantaa. The project is funded by Kaste program which is a development project of the mental health and substance abuse services. The development project is carried out in 2010-2012 and it is part of wider project called Mielen avain. Regarding this thesis the main aims of the Vantaalaisen hyvä mieli project are to develop low-threshold services, strengthen the client’s status and improve the attitudes. According to the men’s experiences mothers are perceived as primary caregivers in our society. Fatherhood is often either underrated or not seen at all. Fatherhood is often seen by motherhood in maternity, child health and family coaching clinics. Many men have experiences of being outsider and some feel that they are not welcome to maternity and child health clinics. In 2000s there have been many projects that have tried to emphasize and strengthen of the role of the father. Despite of that many men still think that they do not get enough support from maternity clinic. Men experience that their partner’s pregnancy is almost always a positive matter. Clinic service is seen as important in preparing for fatherhood and a public health nurse plays an important role in supporting fathers after the baby is born. During the pregnancy fathers need support to the growth of being a father, changes in relationship and going through the experiences. Information is wished of the growth and development of the fetus, the health and care of the baby, the progress of the delivery and the father’s role in the labour. After the birth of the first-born support in parenthood and relationship is needed. The beginning of being a father is challenging but also wonderful time for many men. Men would like to have information of the growth, development, care and handling of the child. A public health nurse can support fatherhood by giving information and emotional evaluative and practical support. It is important to notice father as an equal parent, understand their feelings and needs and make fathers feel welcome in maternity and child health clinics. Many public health nurses feel that they have inadequate ability to support fathers. The difficulty of reaching and motivating fathers is seen as a problem. In the future the studies could focus on for instance to the mothers’ views of the changing roles of parents in the family. The public health nurses’ needs of more education, attitudes and experiences of supporting fatherhood could also be explored. Also the utility of working forms and methods used in maternity and child health clinics could be explored and improved...

Opinnäytetyöprosessin vaikutus hoitotyön opiskelijoihin

Ammattikorkeakoululaisten jaksaminen arjessa on monien eri tekijöiden summa. Opiskelijan psyykkiseen jaksamiseen ja hyvinvointiin vaikuttavat muun muassa oikean opiskelualan löytäminen sekä toimeentulon vaikeudet, jotka johtavat työssä käymiseen. Ammattikorkeakoululaiset kokevat erityisesti loppuvaiheen opinnot ja opinnäytetyön tekemisen raskaina. Opiskelijaelämä on siis jatkuvan muutoksen ja kuormituksen vaikutuksen alaisina. Tämän opinnäytetyön tarkoituksena on selvittää opinnäytetyötä tekevien hoitotyön opiskelijoiden jaksamiseen vaikuttavia tekijöitä, opinnäytetyöprosessin vaikutukset jaksamiseen sekä tukimahdollisuusehdotuksia koululle opinnäytetyöprosessin kehittämiseksi.
Opinnäytetyön tutkimusmenetelmänä käytettiin sekä kvalitatiivista että kvantitatiivista lähestymistapaa. Painotus oli kuitenkin kvalitatiivisessa tutkimusmenetelmässä. Aineisto kerättiin Laurea-ammattikorkeakoulun Tikkurilan yksikön hoitotyön opiskelijoilta (N =260, n =73) puoli-strukturoidulla internet-kyselylomakkeella, jossa oli 11 monivalintakysymystä sekä 4 avointa kysymystä. Kyselyyn oli mahdollista vastata 1 viikon ajan maaliskuussa. Kysely oli luottamuksellinen ja siihen vastattiin anonyymisti. Kvalitatiivisen aineiston analysointiin sovellettiin teorialähtöistä sisällönanalyysia ja kvantitatiivinen osuus analysoitiin laskemalla aineistosta frekvenssejä ja prosenttiosuuksia Excel-ohjelmalla.
Opinnäytetyön tuloksista ilmeni, että opinnäytetyön tekeminen vaikuttaa opiskelijoiden jaksamiseen. Siihen liitettynä koulutehtävät, työssäkäynti ja vapaa-ajan vähyys sekä opinnäytetyön ohjauksen puute vaikuttavat suurelta osin opiskelijoiden hyvinvointiin ja opiskelukykyyn. Sosiaaliset suhteet, taloudellinen asema sekä koulun antama tuki mainittiin jaksamista parantavina tekijöinä. Opiskelijat kokivat opintonsa vaihtelevina, ja monet vastaajista mainitsivat opintojen vaikeutuvan loppua kohden.
Ensisijaisiksi kehittämiskohteiksi nousivat koulun resurssit sekä koulun antama tuki ja opinnäytetyöprosessin organisointi. Opinnäytetyöstä saadut tulokset sekä tukivat aiempia tutkimustuloksia että antoivat uutta tietoa terveysalan opiskelijoiden jaksamisesta.
Opinnäytetyötä tekevien terveysalanopiskelijoiden jaksamista ei ole aiemmin Suomessa tutkittu. Mielenkiintoista olisi tehdä laajempi tutkimus aiheesta sekä vertailla miesten ja naisten välisiä eroja hyvinvointia edistävien tekijöiden suhteen opiskelun aikana....

Developing the care of lower extremity venosus ulcer patient in the home care of Lohja The purpose of this thesis was to develop the wound care instructions of lower extremity venous ulcer patient for the home care of Lohja. The instructions are uniform with Ward 5 of the health center of Lohja. The thesis is a part of rehabilitative nursing project in the cooperative area of social welfare and public health service of Lohja. During the development project the student, the representative of the health center of Lohja and the representative of the home care of Lohja formed a team. The purpose of the team was to develop a new operations model to the wound care of the venous ulcer patient. Dissemination was used as the method of the development project. A wound care record was created as a result of the working team. The model of the wound care card is based on the recommendations of Käypä Hoito and model of the health center of Lohja. The wound care card contains the instructions for the planning of the venous lower extremity ulcer wound care, for the working order and for the realization of the care and information about the characteristics of the wound. The task of the card is to function as a checklist to the nurses and patients. The thesis itself also eases the new workers' orientation. In the evaluation of the new way of action a semi-structured evaluation form and open ended interview were used. On the basis of the answers, the likelihood of the introduction of the card developed by the team is small or nonexistent. On the whole, the thesis was regarded as a suitable tool to the work community. The card will be brought into use in the fall of 2012 so only then will it be seen how the card serves the work community. The development could be continued in the future by editing the wound care record to the home care into a yet more suitable form....

Breast cancer patients’ fears and anxiety during treatment The purpose of the study was to produce an informative patient brochure for operative breast cancer patients about their fears, anxiety and coping strategies they needed their treatment. The contest of the brochure was based by survey, that was conducted in this study. The thesis was made in active co- operation with contact persons of a hospital providing special health care. The theoretical part of the study was based on breast cancer`s appearance and it`s treatments, literature knowledge and patient`s experiences of fear and anxiety as well as guidance. A survey, conducted among breast cancer patients in a hospital providing special health care, was included in the planning of the informative patient brochure. The inquiry took place in august 2011 and in October and November 2011. The purpose of the survey was to examine what kind of emotions the patients had experienced during the different stages of their treatment processes and what kind of coping strategies they had needed to survive. The data were analyzed by qualitative content analysis. The mean-age of the seven respondents was 60. The survey indicated that half of the respondents had experienced anxiety after getting cancer, but they had taken it in a calm manner and they had an understanding that the illness would be cured. They had also reacted positively to the future. Almost all the respondents had emphasized the importance of self-reliance, support of the family and the operation of the professionals of nursing as empowering factors. The brochure was originally intended to the operative breast cancer patients admitted to a hospital providing special medical care for surgical patients. The brochure focuses on patient guidance regarding the negative psychological effects caused by the illness, such as fear and anxiety. The brochure encourages a patient to live by utilizing independent activities with the help of patients’ coping strategies as well as mental resources, which were regarded as positive factors in the survey. The relevance of the brochure as a guiding document for the nurses and for the self-directness of the patients is an important factor. Information about public sectors help organization and some internet- addresses for cancer patients were also included. In the final phase of the process, the brochure was directed for the nursing staff, later it can be developed further for patients by the hospital providing special health care for patients....
