Suositeltavat käytänteet CPAP- potilaan hoitotyössä

Projektiraportti on osa Laurea-ammattikorkeakoulun sekä Helsingin ja Uudenmaan sairaanhoitopiirin (HUS) HYKS kehittämishanketta vuosina 2007–2012. Laurean opinnäytetyön perustana on Learning by Developing (LbD)- menetelmä, joka on kehittämispohjaisen oppimisen toimintamalli. Projektiraportin tavoitteena on laatia suositeltavat käytänteet, jotka perustuvat tutkittuun tietoon. Uuden tutkitun tiedon avulla voidaan kehittää hoitohenkilökunnan ammattitaitoa sekä syventää heidän tietämystään CPAP- potilaan hoitotyössä. Ammattitaidon kehittämisen myötä mahdollistetaan myös turvallinen hoito potilaalle. Projektiryhmäämme kuuluvat opiskelijat Emmi Aaltonen, Tommi Halonen, Hilla Kiviniemi sekä lehtori Marja Tanskanen.
Suositeltavien käytänteiden tavoitteena on edistää näyttöön perustuvaa, korkeatasoista hoitotyötä sekä selkeyttää hoitotyön itsenäistä vastuualuetta potilaiden hoidossa. Suositusten laatimisen lähtökohtana on näyttöön perustuva tieto. Suositeltavat käytänteet antavat hoitohenkilökunnalle yhtenäiset, selkeät ohjeet, joiden avulla CPAP- potilasta hoidetaan. Sairaanhoitajan on tärkeää tunnistaa äkillinen hengitysvajaus ja osata toimia nopeasti muuttuvissa tilanteissa. Tutkitulla tiedolla ja hyvällä potilasohjauksella voidaan potilaille varmistaa hyvä, ammattitaitoinen sekä turvallinen hoito. Käytänteitä laadittiin yhteensä 13. CPAP- hoidolla tarkoitetaan kasvonaamarin avulla annettavaa jatkuvaa ylipainehengityshoitoa (continuous positive airway pressure). CPAP-hoidolla saadaan aikaan jatkuva positiivinen ilmatiepaine joka estää atelektaasien syntymistä. Hoidolla ei avusteta hengitystoimintaa, vaan sen tavoitteellisin vaikutus on hapetuksen parantaminen. CPAP- hoitoa aloitettiin käyttämään 1980-luvulla ensisijaisesti uniapnean ja kaasujenvaihtohäiriöiden hoitoon. Nykyisin CPAP-hoito on yksi tärkein äkillisen hengitysvajauksen, keuhko ödeeman sekä uniapnean hoitomuoto.
Tiedonhakua projektiraporttia varten tehtiin vuosina 2010 - 2011. Teoriatietoa haettiin manuaalisesti kirjastoista sekä sähköisesti eri tietokannoista. Tiedonhaun kautta kävi ilmi että CPAP- hoitoa on tutkittu Suomessa vähän. Tiedonhaun tuloksena voidaan todeta että tutkittuun tietoon perustuvaa lähdeaineistoa löytyi eniten uniapneasta. Äkillisestä hengitysvajauksesta on luotu Käypähoito suositukset vuonna 2006, joita hyödynnettiin projektityössä....

Recipes of Healthy Life for Mental Health Rehabilitees – Nutritional Guidance in CaringTV® This thesis was executed as a part of the Safe Home-research. The purpose of the thesis was to design, execute and evaluate three CaringTV broadcasts for mental health rehabilitees. The purpose was also to develop and produce new content to the programme of the project’s target customers. Interactive CaringTV works as a way to reach the mental health rehabilitees in two different cities. Interactive sessions were executed via CaringTV which aimed to improve mental health rehabilitees’ knowledge of nutrition and in that way to help them choose healthier foods in their diet as a part of their everyday life. The aim was also to motivate mental health rehabilitees to act in a way that promotes their own health. The broadcasts were held in Laurea Otaniemi’s CaringTV-studio in May and April 2011. After the broadcasts mental health rehabilitees received a booklet including different kinds of pictures and texts related to nutrition and a few healthy recipes. Most of the mental health rehabilitees who participated in the broadcasts felt they had received new and useful information on nutrition for the future. Many of them were so motivated already during the broadcasts, that they were ready to change their shopping and eating habits on their next grocery visit. There was a consensus among the mental health rehabilitees that switching a chocolate bar to a fruit and cooking instead of having convenience food were small but important changes on the way to healthier nutrition habits. When allowing persons to take part and make a difference on things themselves, it motivates and activates the change. The specific basis of the broadcasts was to give clients the opportunity to take part by discussing and being active in the presence. Mental health rehabilitees were extremely interested in discussing nutrition and most of them said they had not known about the healthier options. Lack of knowledge can often be a hindrance for mental health rehabilitees because they are not able to choose healthier options in their diet. Because most of the mental health rehabilitees felt they had benefited from CaringTV broadcasts, a further suggestion would be to give them more CaringTV broadcasts on nutrition. The broadcasts could be held on a longer time frame. That would give more time for discussion and motivation would be more long-term. Also the results could be more permanent if there were more than three broadcasts....

A process description of the guidance of a lung cancer patient receiving chemotherapy The thesis was carried out in the Triangle Hospital project. Triangle Hospital is situated next to Meilahti Tower Hospital on Meilahti campus. It is a new hospital for internal medicine and it was completed in November 2010. The Triangle Hospital Project is a collaboration project between the hospital district of Helsinki and Uusimaa, Department of Medicine at Helsinki University Central Hospital and Otaniemi unit of Laurea University of Applied Sciences. The objective of the thesis is to develop the guidance of a lung cancer patient receiving chemotherapy in the Clinic for Pulmonary Diseases in Meilahti Triangle Hospital. The personnel can also use the process description of the thesis not only in developing the patient guidance but also in the orientation of new employees. The purpose of the thesis was to create a process description of the guidance of a lung cancer patient receiving chemotherapy for the Clinic of Pulmonary Diseases in Meilahti Triangle Hospital. The research method used in this thesis was qualitative. The data for the thesis was gathered by using a theme interview, which was carried out as a group interview of three nurses in December 2010. The research questions were: which professional groups participate in the guidance of a lung cancer patient receiving chemotherapy and what is the guidance process of a lung cancer patient receiving chemotherapy like. The interview themes were: the preparation for the guidance of a lung cancer patient receiving chemotherapy, the implementation and contents of the guidance and the evaluation of the guidance. The material gathered from the interview was transcribed and analyzed by using inductive content analysis. Based on the analysis the process description was created with the QPR ProcessGuide –programme. The findings of the thesis show that the guidance process of a lung cancer patient receiving chemotherapy is divided into the planning of the guidance, the implementation of the guidance, the evaluation of the guidance and the ensuring of the follow-up care. The nurses from the outpatient clinic, day hospital and ward participate in the guidance of the patient as well as the physicians. According to the findings there are also other occupational groups, which participate in the guidance if necessary, like a social worker and a dietician. Due to the extensive data of the thesis there was a lot of material on the guidance, e.g. the guidance of the causes of the illness in everyday life and the guidance of the medication. Mainly the treatment path of a lung cancer patient starts from the outpatient clinic after which the patient is transferred to the day hospital to get chemotherapy, if a physician has prescribed it. After the chemotherapy treatments a physician evaluates the patient’s condition and plans the follow-up care. There are several options in the patient’s follow-up care. He can be treated at home, in specialized care, in a hospice or in primary health care....

The intention was that with the process description the Clinic of Pulmonary Diseases can develop their patient guidance. The material was gathered with a theme interview, which was conducted by interviewing three nurses of the Clinic of Pulmonary Diseases in a group. The themes of the interview were the process of patient guidance, the methods of guidance, the contents of guidance and the professionals who participate in patient guidance. The interviews were recorded and the information gained from the interview was transcribed. The material was analyzed by using material-based content analysis. Based on findings in the content analysis, a process description of patient guidance in chronic obstructive pulmonary disease was created using QPR ProcessGuide –program. Based on the analysis, patient guidance in chronic obstructive pulmonary diseases divided to three stages in the process description: the definition of the necessity in patient guidance, the completion of patient guidance and confirming the continuity of treatment. Patients with chronic obstructive pulmonary disease visit the Clinic of Pulmonary Diseases on average two times, and after that their treatment is relocated to primary health care. In the Clinic of Pulmonary Diseases a doctor and a nurse participate in the patient guidance. A physiotherapist, a rehabilitation instructor, a dietician and a social worker also participate in the patient guidance when required. In the patient guidance of chronic obstructive pulmonary disease the central idea is how a patient can have an effect on disease and to confirm the continuity of treatment with one’s own actions in everyday life. One can affect chronic obstructive pulmonary disease by stopping smoking, exercising, controlling weight, using medication and controlling relapse phase. Patients with chronic obstructive pulmonary disease are guided individually based on their needs. As guidance methods nurses use individual guidance, which and the guidance contents are presented orally, in writing, and through demonstration and audiovisual methods. The findings of the thesis and the process description can be used in observing and developing the procedures in the Clinic of Pulmonary Diseases....

NOPS 2010-2012-hanke nuoren työ- ja toimintakyvyn parantaminen ammattiopistoissa, ammattiopiston opiskelijan ravitsemukseen liittyvä terveysosaaminen
Opinnäytetyöni liittyy Laurea-ammattikorkeakoulun yhdessä useiden muiden toimijoiden kanssa 2010–2012 - toteuttamaan NOPS-hankkeeseen, jonka tavoitteena on uudistaa ammattiopistojen käytäntöjä terveyden edistämisen työ- ja toimintatavoissa. Opinnäytetyössäni pyrin selvittämään IMB-mallin pohjalta yksittäisen terveystapahtuman vaikutusta interventiona ammattiopistossa opiskelevan nuoren terveyden edistämiseksi. Tutkin myös, millaista on ammattiopistossa opiskelevien nuorten terveysosaaminen ennen ja jälkeen interventioiden. Tarkoituksena on lisäksi selvittää, minkälaisia terveystaitoja opiskelijoille muodostuu interventioiden aikana ja miten motivoituneita he olivat huolehtimaan terveydestään näiden interventioiden jälkeen. Tutkin myös terveystapahtuman käytännön toteutusta.
Opinnäytetyöni teoreettisessa viitekehyksessä käsitellään nuorten terveysosaamisen lisäksi heidän ravitsemuskäyttäytymistään, sen merkitystä ja vaikutusta nuoren terveyteen. Valtakunnallisten ravitsemussuositusten lisäksi teoriaosuudesta ilmenee asiakaslähtöisen elämäntapaohjauksen ja terveysvalistuksen tärkeä merkitys nuoren terveysosaamisen ja oikeiden ravitsemustottumusten edistämisessä. Myös lapsuuden kodin ja terveyttä suosivan ympäristön vaikutus huomioidaan.
Tutkimusmenetelmä oli kvantitatiivinen. Tutkimuksen aineisto kerättiin strukturoiduilla kyselylomakkeilla, joihin vastasi 110 ammattiopiston opiskelijaa. Kysymykset olivat kaikille samat ja ne esitettiin samassa järjestyksessä, sekä ennen että jälkeen terveystapahtuman. Aineisto analysoitiin SPSS-tilasto-ohjelmalla ja tutkimustulokset ilmoitettiin prosenttijakaumina.
Tutkimustulokset osoittivat, että ammattiopiston nuorten terveystiedot ravitsemuksesta olivat melko hyvät jo ennen terveystapahtumaa. Tapahtuma lisäsi ja paransi monien terveystietämystä motivoiden heitä myös muuttamaan terveyskäyttäytymistään terveyttä edistävämmäksi. Opiskelijat kokivat tapahtuman tärkeänä ja kiinnostavana.
