A process description of patient guidance in chronic obstructive pulmonary disease in the Clinic of Pulmonary Diseases The purpose of the thesis was to describe the process description of patient guidance in chronic obstructive pulmonary disease in the hospital district of Helsinki and Uusimaa, Triangle Hospital, the Clinic of Pulmonary Diseases. The intention was that with the process description the Clinic of Pulmonary Diseases can develop their patient guidance. The thesis was carried out in Triangle Hospital –project, started in year 2003 in the hospital district of Helsinki and Uusimaa. The Triangle Hospital –project is a collaboration project between the hospital district of Helsinki and Uusimaa, Helsinki University Central Hospital, Medical profit-and-loss centre and Laurea University of Applied Sciences Otaniemi unit. Qualitative method was used in implementing this thesis. The material was gathered with a theme interview, which was conducted by interviewing three nurses of the Clinic of Pulmonary Diseases in a group. The themes of the interview were the process of patient guidance, the methods of guidance, the contents of guidance and the professionals who participate in patient guidance. The interviews were recorded and the information gained from the interview was transcribed. The material was analyzed by using material-based content analysis. Based on findings in the content analysis, a process description of patient guidance in chronic obstructive pulmonary disease was created using QPR ProcessGuide –program. Based on the analysis, patient guidance in chronic obstructive pulmonary diseases divided to three stages in the process description: the definition of the necessity in patient guidance, the completion of patient guidance and confirming the continuity of treatment. Patients with chronic obstructive pulmonary disease visit the Clinic of Pulmonary Diseases on average two times, and after that their treatment is relocated to primary health care. In the Clinic of Pulmonary Diseases a doctor and a nurse participate in the patient guidance. A physiotherapist, a rehabilitation instructor, a dietician and a social worker also participate in the patient guidance when required. In the patient guidance of chronic obstructive pulmonary disease the central idea is how a patient can have an effect on disease and to confirm the continuity of treatment with one’s own actions in everyday life. One can affect chronic obstructive pulmonary disease by stopping smoking, exercising, controlling weight, using medication and controlling relapse phase. Patients with chronic obstructive pulmonary disease are guided individually based on their needs. As guidance methods nurses use individual guidance, which and the guidance contents are presented orally, in writing, and through demonstration and audiovisual methods. The findings of the thesis and the process description can be used in observing and developing the procedures in the Clinic of Pulmonary Diseases....

Diabetes Conversations -program in type 2 diabetes patient's group counseling The topic of the thesis is type 2 diabetes group counseling by using the Diabetes Conversations –program. The Diabetes Conversations –program was developed in USA by the Healthy Interactions in collaboration with the International Diabetes Federation, and is sponsored by Eli Lilly & Company. The program is a guided teamworkmodel where maps, discussion, ques-tion cards and other supplementary materials are used in the program. The thesis was carried out at Loviisa healthcare center in the autumn of 2011. In Loviisa the diabetes nurses are planning to use group counseling in the future as one form of diabetes patients’ counseling. The first diabetes group counseling was carried out in the form of the thesis as a proposed by the Loviisa healthcare center diabetes nurses. The purpose of this thesis is to study the effectiveness of the Diabetes Conversations –program and the areas of development for diabetics’ group counseling. When planning the thesis, we started with the belief that diabetes counseling is efficient if carried out in a group, not only for the individuals who participate in the group, but also for the staff. It was assumed that peer support and group counseling supports learning and handling of new things and issues, and this way improves patient’s compliance and diabetic’s selfcare. The theoretical framework consisted of type 2 diabetes, patient guiding, group counseling and adults learning. I.a. Laurus, Medic, PubMed Central and Arto were used for information gathering. The diabetes nurses of Loviisa healthcare center chose a group of diabetics to take part in the group counseling. There were in total four participants in the group. The group counseling took place in Loviisa in the autumn of 2011. Four meetings were arranged with the group and the subjects of the sessions were: Living with diabetes, the effects of diabetes in the system, nutrition and exercise and pedicure. A questionnaire was used for the thesis. It included two questions for background information and the remaining questions were openended and sought answers to the goals of the thesis. The material was analyzed using qualitative content analysis. The material was collected in autumn of 2011. The results show that taking part in the group counseling encouraged participants to think about the importance of good selfcare. By taking part in group counseling, participants experienced that they got peer support, positive encouragement and they had assimilated new information. The most important result of the thesis was that the Diabetes Conversations -program does support learning in a group in a meaningful way. The most important development proposals that arose from the thesis are, group counseling as part of the diabetics counseling and goal setting for the diabetic´s selfcare....

Lasten ylipaino ja lihavuus ― puheeksi ottaminen ja ohjaus lastenneuvoloiden terveydenhoitajien kokemana

Tämän opinnäytetyön tavoitteena oli kartoittaa lastenneuvolan terveydenhoitajien kokemuksia lasten ylipainon ja lihavuuden puheeksi ottamisesta ja ohjauksesta. Tarkoituksena oli selvittää millaisiksi neuvolan terveydenhoitajat kokevat valmiutensa kyseisissä aiheissa. Lisäksi selvitettiin tekijöitä, jotka he kokevat niitä edistäviksi ja estäviksi.
Aineisto kerättiin elektronisen kyselylomakkeen avulla Espoon, Lahden, Lohjan ja Mäntsälän alueiden lastenneuvoloiden terveydenhoitajilta. Kyselylomake koostui suurimmaksi osaksi avoimista kysymyksistä. Tutkimusaineisto rakentui 21 vastauksesta, ja se analysoitiin sisällönanalyysilla. Tutkimusmenetelmänä käytettiin laadullista tutkimusta.
Tutkimustulokset tuovat esille, että suurin osa lastenneuvolan terveydenhoitajista koki valmiutensa lapsen ylipainon tai lihavuuden puheeksi ottamiseen ja ohjaukseen hyvinä. Vastausten perusteella terveydenhoitajat kokivat, että heidän valmiuksiaan voitaisiin kehittää erityisesti aiheeseen liittyvän koulutuksen avulla. Varsinkin puheeksi ottamisen koulutus nähtiin tarpeellisena. Lapsen ylipainon tai lihavuuden puheeksi ottamista ja ohjausta edistävät ja estävät tekijät liittyivät suurimmaksi osaksi vanhempien elämäntapoihin, työntekijän työkokemukseen sekä rohkeuteen puuttua asiaan. Ympäristöstä lähtöisin olevina tekijöinä tuli esille merkittävimpänä yleisesti muuttuneet liikunta- ja ruokailutottumukset, sekä resursseihin liittyvät haasteet....

Recommended practices for the use of arteriovenous impulse system as part of traumatological adult patient care in Töölö hospital The thesis is quality development project between Töölö hospital and its traumatological intensive care unit, which belong to the HUCH Hospital Area and Laurea University of Applied Sciences between 2007 - 2012. The goal of this thesis was to develop and increase the quality of nursing care among traumatological patients and to produce evidence-based integrated recommended practices for HUCH Hospital Area and Töölö hospital of arteriovenous (A-V) impulse system in a traumatological patient’s care, concentrating on intensive care. A-V impulse system pressure elements are placed in the limbs, which send impulses to maintain blood flow in the lower and upper limbs. They mimic the natural physiological process. For A-V impulse system the primary purpose is to prevent deep vein thrombosis. In addition, they reduce the acute and chronic swelling and improve peripheral arterial microcirculation. In addition, they increase the venous flow. A-V impulse system therapy promotes the return flow of the heart as effectively as walking and thus reduce and prevent the formation of deep vein thrombosis. We have used the Elcos Medical A-V impulse system in the project. Based on information the recommended practices include the observation of contraindications of A-V impulse system before starting the treatment, the start of the treatment, the setting of pressure elements, the documentation of treatment with A-V impulse system, A-V impulse system therapy follow-up and the duration of AV-impulse system treatment. The aim was also to familiarize the Töölö hospitals medical staff and nursing students with the A-V impulse system for use with traumatological patients based on recommended care practices as well as to provide integrated medical practices as part of traumatological patient care. The project data acquisition, project plan and project writing took place from January to September 2011. The student’s work placements in the traumatological intensive care unit in Töölö hospital played a key role in the progress of the project. In addition to the nursing students the project team consisted of lecturers Hannele Moisander and Hellevi Pelto-Huikko, traumatological intensive care nursing specialists Anna-Liisa Fel and Helena Berg, medical science expert Ilkka Tulikoura and nursing clinical instructor of Töölö Hospital, Netta Pohjamies-Molander. The completed recommendations are presented in Töölö hospital 13th December 2011....

Leikki-ikäisen lapsen ruokailu päiväkodissa

Tämän opinnäytetyön tarkoituksena oli selvittää lasten ruokailutapoja päiväkodin ruokailutilanteissa, päiväkodissa esiintyviä lasten syömiseen ja ruokailuun liittyviä haasteita, sekä päiväkodin kasvattajien ratkaisukeinoja mahdollisiin ruokailutilanteissa ilmeneviin pulmiin. Tämä työ on osa Lapsen ja nuoren hyvä arki –hanketta. Hankkeen tavoitteena on ammattikorkeakoulutuksen ja työelämän yhteistyön kehittäminen. Opinnäytetyön tuottamaa tietoa voidaan hyödyntää lapsiperheen ravitsemusneuvonnassa ja päiväkodin ravitsemuskasvatuksen tukena.
Opinnäytetyön tutkimusote oli laadullinen eli kvalitatiivinen. Aineistonkeruumenetelminä käytettiin tarkkailevaa havainnointia ja kyselyä. Aineisto kerättiin Hyvinkäällä sijaitsevassa päiväkodissa havainnoimalla päiväkotiryhmän ruokailutilanteita, sekä kysymällä päiväkodissa työskenteleviltä kasvattajilta päiväkotiruokailussa esiintyvistä haasteista ja mahdollisten pulmien ratkaisukeinoista. Tiedonkeruun välineinä käytettiin tätä työtä varten laadittua havainnointirunkoa, sekä avoimia kysymyksiä sisältävää kyselylomaketta. Kyselyyn vastasi 5 kasvattajaa kahdesta eri päiväkotiryhmästä, joissa lapset ovat iältään 3-5-vuotiaita. Kerätty tutkimusaineisto analysoitiin sisällönanalyysillä.
Opinnäytetyön tuloksina todetaan, että lasten ruokatavat vaihtelivat päiväkodin ruokailutilanteissa. Hyviä ruokatapoja olivat muun muassa käsien peseminen ennen ruokailua, pöydän kattamiseen osallistuminen, asiallinen käyttäytyminen ruokapöydässä, ruokailuvälineiden käyttö omien taitojen mukaisesti, oman paikan siistiminen ja suun pyyhkiminen ruokailun jälkeen, sekä ruoasta kiittäminen oma-aloitteisesti tai kasvattajien muistuttamana. Parantamisen varaa lapsilla oli muun muassa ruokailuun rauhoittumisen, sekä pöydässä istumisen suhteen. Myös syöminen saattoi välillä unohtua, kun lapset alkoivat leikkiä ruokapöydässä. Lisäksi rauhallista ruokapöytäkeskustelua oman pöytäseurueen kanssa ei juuri esiintynyt. Päiväkodin ruokailutilanteissa esiintyi erilaisia haasteita ruokailuilmapiiriin, syömiseen, sekä hygienian toteutumiseen liittyen. Ruokailuilmapiiriin liittyviä haasteita olivat erityisesti melu, ruokarauhan puuttuminen, lasten levottomuus, sekä pitkät odotusajat. Syömiseen liittyviä haasteita puolestaan olivat valikoiva syöminen, ennakkoluulot uusia ruokia kohtaan, omatoimisuuden puuttuminen ruokaillessa, sekä aterian koostamiseen liittyvät haasteet, kuten joidenkin ruoka-aineiden niukka syöminen tai syöminen yli oman tarpeen. Tämä näkyi etenkin lasten vähäisenä salaatin syömisenä. Hygienian toteutumiseen liittyviä haasteita taas olivat puutteet käsihygieniassa, ruokaan ja ruokailuvälineisiin koskeminen sormin, lattialle pudonneen ruoan syöminen, sekä eritteiden leviäminen ruokapöydässä. Tulokset osoittivat, että päiväkodin kasvattajien ratkaisukeinoja ruokailutilanteissa ilmeneviin pulmiin olivat muun muassa aikuisen esimerkki ruokailutilanteissa, yhteiset säännöt ja rituaalit, sekä lapsen yksilöllinen ohjaus.
Leikki-ikäisten ajoittaiset syömiseen ja ruokailuun liittyvät pulmat ovat hyvin yleisiä sekä kotona että päiväkodissa. Monet lasten ruokailuun liittyvät ongelmat ratkeavat kuitenkin kodin ja päiväkodin yhteistyöllä. Aikuisen esimerkki molemmissa on avainasemassa lasten ruokatottumusten myönteiselle kehitykselle....

Student’s conceptions of bullying and its influences in Hyria vocational school of Hyvinkää in 2010-2011 This report is part of NOPS 2010-2011 – project, which aims to create for student who studies in vocational school a standard of activity which increases working capacity and -order. The aim of this report was to find out, what kind of bullying goes on in vocational school, how it effects students and how the school has been intervene. The frame of this study was based on literature on bullying. The questionnaire consists 20 structured and one open-ended question. Our focus group was students whom started in the autumn 2010 in the vocational school of Uudenmaankatu of Hyria educational Oy. We got 34 returned questionnaires. The results showed that in vocational school six percent of the students who started in autumn 2010 told they had been bullied. Students felt mostly displeasure about bullying and six percent of students had been out of school because of bullying. 35 % of the students told that adolescents did not get enough help at the moment of bullying and most of these students felt that grown-ups did not do anything. The majority of the students answered that the bullied did not get enough help because they did not want to tell anyone about bullying for fear of the situation getting worse. Our study showed that telling about bullying did not make the situation worse even if person did tell anyone about it. 65 % of the students did not know whether adolescents had enough help and only six percent responded that adolescents did have enough help. The results indicated that teachers needed to be more available in bullying situation. That way it would be much easier to intervene in the situation and the teacher would recognize the situations of bullying early enough. The result showed that there is room for a further study comprising all the students of the school and possibly the other Hyria offices as well....

Seventh Grader´s Experiences of Meaningfulness, Comprehensibility and Control in Life This thesis belongs to a project called Maintaining and Promoting Junior High School Pupils` Health and Well-being (Yläkouluikäisten nuorten terveyden ja hyvinvoinnin ylläpitäminen ja edistäminen). The project takes place between the years 2010 and 2013. The cooperation partners are the department of Social Services, Nursing and Sport of Laurea University of Applied Sciences and Kuitinmäki School in Espoo. The main aim is to increase seventh to ninth graders awareness of well-being and health issues. The aim of this thesis was to describe seventh graders’ views of their own well-being and the best information sources for health issues. The participants were seventh graders from Kuitinmäki School and a control group of seventh graders from Tapiola School. The work was based on Aaron Antonovsky`s theory of the sense of coherence. The theory includes three main points: meaningfulness, comprehensibility and control in life. A quantitative research method was used in analysing the material. The material was collected by a questionnaire. The main part of the questionnaire was the indicator of the sense of coherence created by Antonovsky. The indicator was divided into thirteen parts. The answers were analysed by SPSS statistics program. There were 196 participants in the study. The results of the study showed that only two statistically significant claims in the questionnaire differ between the two schools. The main results of the study showed that control in life is the strongest part of the seventh graders’ sense of coherence and comprehensibility in life is the weakest part. The best informants for the adolescents in health issues were parents, health education in school and school nurse. The participants experienced that friends and internet do not have a major influence on the choices concerning health issues. The intention is to conduct the same study for the same participants when they are on the ninth grade. Thus the results might show the possible changes in adolescents` knowledge, skills and attitudes....

Vuorotyön terveysvaikutukset Kotokartanosäätiön hoitohenkilökunnan kokemina

Tämän opinnäytetyön tarkoituksena oli kuvata vuorotyön terveyshaittojen nykytilannetta Kotokartanosäätiössä. Aihetta ei ole aikaisemmin tutkittu kyseisessä yhteisössä ja siksi opinnäytetyölle oli tarvetta. Opinnäytetyön tavoitteena oli kartoittaa vuorotyöstä aiheutuvia terveyshaittoja Kotokartanosäätiön hoitotyöntekijöillä sekä tuottaa uutta tietoa ja kehittämisideoita.
Opinnäytetyössä käytettiin määrällistä tutkimusmenetelmää, koska haluttiin selvittää olemassa oleva tilanne. Aineistonkeruumenetelmänä oli kyselylomake. Kyselyyn vastaaminen oli vapaaehtoista ja siihen osallistui Kotokartanosäätiön 41: stä hoitotyöntekijästä 30. Aineisto analysoitiin käyttäen SPSS- ohjelmaa.
Opinnäytetyön tulosten pohjalta voidaan todeta Kotokartanosäätiön hoitohenkilökunnalla esiintyvän etenkin ravitsemukseen liittyviä vuorotyön terveysvaikutuksia. Tulokset osoittivat myös, että hoitohenkilökunnalla on vaikeuksia ylläpitää sosiaalisia suhteita vuorotyön takia. Kuitenkin suurella osalla oli säännöllinen harrastus, joka auttaa vuorotyöstä palautumisessa. Mahdollisuudella vaikuttaa omiin työaikoihinsa, on todettu olevan positiivinen vaikutus vuorotyössä jaksamisessa. Kotokartanosäätiön hoitohenkilökunnasta suuri osa koki voivansa vaikuttaa niihin aina tai joskus riittävästi, mutta peräti kolmasosa oli sitä mieltä, että vaikutusmahdollisuudet olivat vähäiset.
Jatkotutkimusehdotuksina esitän samanlaisen tutkimuksen uusimista tietyin väliajoin. Myös hoitohenkilökunnan työhyvinvointiin kohdistuva tutkimus voitaisiin tehdä jatkossa....

Virtsarakon syöpä, toteaminen ja hoito : Opas polikliiniselle hoitajalle

Urologian poliklinikalla hoidetaan monenlaisia urologisia potilaita. Myös potilaiden oireet ovat monisyisiä ja voivat hankaloittaa elämää runsaasti. Tässä työssä käsitellään urologisia oireita ja tauteja, mutta päähuomio on keskitetty virtsarakonsyövän toteamiseen, oireisiin ja hoitoon. Virtsarakon syöpää hoidetaan pääosin polikliinisesti, joten tämä työ käsittelee virtsarakon syövän polikliinista tutkimista ja hoitoa.
Työn tarkoituksena on toimia ohjekirjasena sairaan– ja lähihoitajille. Työssä on siis käsitelty myös ohjeiden tekemistä ja potilaan ohjausta. Työn teoriaosuudessa käsitellään myös aiheeseen liittyen virtsateiden anatomiaa ja fysiologiaa, mikä edesauttaa hoitojen ja potilaan ymmärtämistä ja ohjausta. Työ perustuu kirjallisuuskatsaukseen sekä hoitotyön harjoitteluun HUS:n Meilahden kirurgisella poliklinikalla. Teoriaosuus on laadittu kirjallisuuden perusteella. Toimenpiteiden suoritukset ja potilaan ohjaus toimenpiteisiin ovat oppaassa liitteenä. Liite sisältää myös urologisen sanaston.
Virtsarakon syöpä on neljänneksi yleisin syöpä Suomessa miehillä. Virtsarakon syövälle altistavat monet tekijät, kuten tupakointi, aniliinivärit sekä skistosomiaasi. Rakkosyövät ovat moninaisia ja niiden luokittelu vaikeaa. Kasvain luokitellaan joko histologisesti tai sytologisesti.
Tyypillisiä oireita ovat mm. verivirtsaisuus ja erilaiset virtsaamiseen liittyvät häiriöt. Virtsarakkoa voidaan tutkia mm. kystoskopialla, ultraäänellä tai sytologilla menetelmillä. Syöpää voidaan hoitaa joko leikkaamalla rakko pois, höyläämällä syöpäkasvaimet pois sekä antamalla syövän uusiutumista ehkäiseviä virtsarakon huuhteluita. Huuhteluissa käytetään joko sytostaatteja tai BCG- medac:a. BCG-medac sisältää heikennettyjä tuberkuloosibakteereita, joka aktivoi potilaan omat puolustusmekanismit. Potilaista 70 % on elossa viiden vuoden kuluttua diagnoosista, joten mikäli syöpä ei ole ehtinyt invasoitua rakon ulkopuolelle, on ennuste suhteellisen hyvä.
Syöpädiagnoosin saaminen on potilaalle yleensä henkisesti raskasta, joten potilaan ohjaus on erittäin tärkeää. Myös hyvä ohjaus ennen toimenpidettä, toimenpiteen aikana ja sen jälkeen saa potilaan tuntemaan olonsa turvallisemmaksi ja edesauttaa hoitojen onnistumista ja hyvään hoitotulokseen pääsyä. Toimenpiteen suorittaminen oikein parantaa niin potilaan kuin hoitajan työturvallisuutta....
