Hyvää hoitoa Helsingin Lastenklinikan infektio- osaston potilaille hoitajan näkökulmasta

Opinnäytetyömme käsittelee hyvän hoidon määrittelyä lastenklinikan K1-osastolle. Osastolle on tehty alustava luonnos hyvän hoidon kriteereistä. Opinnäytetyömme tarkoituksena on laatia hoitajille työkalu, jolla he pystyvät arvioimaan toteutuneen hoitotyön laatua. Osastolla oli luonnosteltu lista kriiterejä, joiden avulla hoitajat voivat määritellä onko työvuoron aikana toteutunut hyvä hoito. Halusimme osana opinnäytetyötämme tehdä osastolle helposti luettavan ja yksinkertaisen työkalun hyvän hoidon kriteereistä.
Työ on kaksiosainen. Ensimmäinen osa pohjustaa tutkimuksen teorialla. Toisena osiona on selvitys, toteutus, tulokset ja analysointi. Kysely rakennettiin SWOT -analyysin kehikkoon tulosten esittämisen ja analysoinnin selkeyttämiseksi.
Työn kirjallisuuskatsauksessa tutkittiin lastensairaanhoidon erityispiirteitä, avattiin osastolla yleisimmin esiintyvät infektiotaudit.
Kyselylomakkeet lähetettiin kuudelle hoitajalle huhtikuussa 2011. Vastauksia palautui kaksi. Kirjalliset vastaukset purettiin käyttämällä sisällön erittelyä. Kyselyn pohjalta esiin nousivat hoitajien työtä hankaloittavina asioina tilan ahtaus, henkilökuntavaje sekä aggressiiviset potilaat/ omaiset. Työyhteisön vahvuuksina nähtiin hoitajien syventyminen hoitotyön eri osa- alueisiin, yhteisöllisyys sekä halu auttaa potilaita. Vastaajien mukaan hyvä hoito näkyy osastolla tyytyväisinä, kokonaisvaltaisesti hoidettuina potilaina sekä tyytyväisinä hoitajina.
Työkalun suunnittelimme graafiseen muotoon, jossa portaita pitkin edetään kohti pylväitä, jotka kuvastavat laajemmassa skaalassa hoitotyöhön liittyviä arvoja ja toimintoja. Pylväät tukevat kattoa, joka kuvaa hyvää hoitoa. Kyselyn ja teorian kautta saadut tiedot tukevat osastolla jo aiemmin tehtyjä kriteerejä sekä opinnäytetyön tuotosta....

Art activities supporting work well-being - supervisors’ experiences of art activity days This bachelor’s thesis is related to “TaHVo” - Will, Well-being and Power! (TaHVo – Tahtoa, Hyvinvointia, Voimaa!) The project studies how art activities and work coaching support work well-being. The aim of this project was to support supervisors’ work well-being as well as creativity through art activities and work coaching. The partners were ceramic artist Johanna Rytkölä, work supervisor Kirsti Pirttijärvi, Laurea University of Applied Sciences, the nursing management team for hospital services in the city of Vantaa. The financier was the Ministry of Education and Culture of Finland. The thesis focuses on the City Vantaa nursing management team. The team consists of a nursing director and 11 head nurses. In the project there were organized two art activity days in October and November 2010, as well as work coaching the same autumn. The work coaching will continue until the end of 2011. The frame reference of the thesis consisted of supervisors’ coping at work and well-being, art activities and their impact on the work well-being, work coaching, stress and load factors. Qualitative method was applied in this thesis, which examines what kind of experience supervisors had of this project, art activity days and how stress and load factors related to supervisors’ general well-being. The study was carried out through the initial survey, activity art days and feedback as well as group interviews at the end of activity days. The initial survey was answered by ten people, two did not answer. The ages of the respondents were between 46 and 57 years. The respondents had been in the current work task from three months to twenty years. The feedback questionnaires concerning both art activity days were answered by nine people, three did not answer. The group interviews were answered by eleven people, one did not answer. The research results show that the majority of the supervisors thought that the art activity days were refreshing and enabled working together. The art activity days created transparency among supervisors, but a few found that the project did not improve the work atmosphere. Some respondents did not find ceramic arts to belong to their field. The art activity days emphasised the joy to work together. The work coaching enhanced the opinions of colleagues during the discussions. Some respondents thought that everyone could not express their thoughts enough openly. The respondents felt that work coaching mainly supported being a supervisor. To some extent there was also criticism towards the art activity days concerning how it supports working life....

A documentary guide for digital rehabilitation plan in a short period unit The purpose of this study was to compile a guide for digital documentation and to improve digital documentation and the structure to make report writing easier and make sure that the nursing documentation is connected to a patient´s own rehabilitation plan in Riihikoti, a unit specialized in short term rehabilitation of the elderly and of the clients with different kinds of dementia. The purpose was to take into account the important matters in this kind of unit in digital documentation and planning an individual rehabilitation plan for the clients. By using FinCC-classification writing data will be clear and by learning the use of FinCC –classification writing will not take so much time. The documentation is based on the FinCC –classification,which is used for instance in home care, hospitals and the other health care units. That is the reason why every unit should plan their individual care and rehabilitation plans by themselves. The documentation and the individual rehabilitation plans need to be improved. By using the classification the decision making processes in health care make a nurse´s work more visible. The rehabilitation period consists of physical activity and meeting a physiotherapis who helps to solve the problems in functional capacity. Balance exercises and exercises which improve muscle strength are important for the elderly people. In addition well-being and empowerment are considered important. Nutrition and medicines are an important part of the health care. According to the staff their use daily documentation but the indivitual rehabilitation plans are not used in daily writing. In our meeting we decided to open the main components which are defining the lower components and fluent narrative writing. Documentation improvement was a part of the quality improvement years 2009-2011 in Riihikoti. As the rate of elderly people increases, there is also growth in the number of elderly people who require social and health care. It is quite common in Finland that nearest relatives are taken care by the elderly relatives, which also mean increased demand of the health care and social services for the family. The target of health care will be in the field of the home care system, where the short health care periods are planned for the families in order to help them in their everyday life. The aim is to make living at home possible as long as it is reasonable by providing short period rehabilitation so that the nearest caring relatives may have their statutory holidays....

Patient guidelines as evaluated by clients This thesis is a research report that is made for Kustannus Oy Duodecim. The report presents a research called “Client evaluation of Terveyskirjasto.fi-service`s patient guidelines” that is carried out in collaboration with Kustannus Oy Duodecim. Kustannus Oy Duodecim is a publisher that specializes in medical science. They publish schoolbooks for health care professionals, books about health in general and electronic publications. Terveyskirjasto is a free, nationwide Internet service provided by Kustannus Oy Duodecim. The website includes health articles and patient guidelines. This research report examines patient guidelines in Terveyskirjasto by analyzing client feedback. The goal of the study was to get information on how clients experience patient guidelines in Terveyskirjasto. The purpose of this thesis was to describe clients’ experiences of patient guidelines in Terveyskirjasto. The goal of this thesis was to produce information which would help to develop the patient guidelines. The main research question was how people experience patient guidelines in Terveyskirjasto. The method for carrying out this study was quantitative. The research material was gathered during six months in three health centers in the metropolitan area by using a questionnaire. All the respondents were volunteers visiting the health centers. The material received from the questionnaires was analyzed with the spreadsheet programme called Microsoft Office Excel 2007. The result of this thesis was that 83 % of all respondents think that the patient guidelines in Terveyskirjasto are good in the form they are now. 96 % of all respondents thought that the patient guidelines are reliable and 47 % of them thought that they are very reliable. 66 % of all respondents felt that the information is useful in some way and 30 % thought that it is very useful. 96 % of all respondents experienced that they got a lot of information out of the patient guidelines. 88 % of all respondents felt that they understood everything they had read and 13 % felt that they had understood enough about what they had read. This thesis and its results can be used in developing the patient guidelines in Terveyskirjasto and charting the needs that developing requires. In the future the evaluation of the patient guidelines in Terveyskirjasto could be carried out by for example using an electronic evaluation form in Terveyskirjasto website. This kind of form could preferably contain questions that are based on scales....

Implementation of Safe Immunoglobulin Infusion for a Hematological Patient The theme of this thesis was the implementation of safe immunoglobulin infusion for a hema-tological patient. The purpose of this qualitative thesis was to describe what kind of aspects relating to medication safety nurses need to consider during the process of immunoglobulin infusion treatment. The aim of this thesis was to produce information that would improve the medication safety of hematological patients receiving immunoglobulin infusions. In this thesis a hematological patient meant patients having chronic lymphocytic leukemia, lymphoma, multiple myeloma or idiopathic thrombocytopenia (ITP). The thesis was carried out in the Hematological Clinic in HUCH where the immunoglobulin infusions were commonly used in treatment of these diseases. The data was collected by one thematic interview of a pair of nurses and one single interview. Nurses who participated in the study had at least one year of working experience and information about this theme. The Interviews were recorded and transcribed. The data was analysed using inductive content analysis. The findings of our thesis are related to a certain time, environment and situation. The findings of the thesis were divided into three main categories concerning medication safety; matters concerning safety before the immunoglobulin infusion, during the infusion and after the infusion. Matters concerning medication safety before the immunoglobulin infusion were monitoring before the infusion, patient interview, patient anamnesis, revision of the medicine, revision of the prescription, preparing for the infusion, recording of the anamnesis, patient guidance before the infusion and nurse´s skills. Matters concerning medication safety during the immunoglobulin infusion were patient monitoring during the infusion, possible adverse effects, increasing the patients´ feeling of safety, safe implementation of the infusion, recording the implementation of the infusion, patient guidance during the infusion and matters related to the personel and working conditions. Matters concerning medication safety after the infusion were patient monitoring after the infusion, limitations to other treatments and patient guidance after the infusion. A new thesis theme could be for instance patient´s experiences of guidance for immunoglobulin infusion treatment. On the other hand a new thesis theme could also be safe implementation of immunoglobulin infusion treatment for some other group of patients for instance neurological patients carried out as a comparative study of immunoglobulin infusions for different patient groups....
