Nutrition of preschool-age children at home : parents' and children's experiences Chilhood nutrition can influence nutrition, obesity, cardiovascular diseases and type 2 diabetes in adulthood. Eating habits should be learned as a child in order to easier adopt healty eating habits. Nutrition is not intented solely to maintain physical well-being or prevent diseases, it is intented to be part of overall human well-being. This is a gualitative study. It is part of the project "Good life for children and adolescents". The ultimate aim was to investigate the realization of nutritional recommendations for children, to descibe the children's and parents' experiences of dining situations, and to learn about the challanges encountered in such situations. The data were collected by interviewing parents and pre-schoolers and by observing pre-schoolers at dinner table. The participants were one-year-old children and a group of three to four years old children. Six families with seven children were interviewed. The data were analyzed using a content analysis method. The results indicated that the parents considered the children's eating habits important. The parents regarded regular meals and mealtimes as important. The parents had set long-term goals for children's eating behaviour. They wanted their children to be open-minded towards new food and eat home-cooked meals. The meals were prepared in accordance with the plate model. Children's plate model was unknown to the parents. The parents did not know how the children's plate model differed from the common model and what it consisted of. During the meals the children were directed to proper behaviour at the table. The children were interested in eating and the parents prepared the meals with them. The families preferred to dine together, if possible. Eating problems were related to certain situations such as refusals and trying the limits. The parents preferred positive guidance in solving such problems. The results can be used by parents and nurses. Families with children can compare the results in health and nutrition counselling....

RECOMMENDED BEST PRACTICES FOR PREVENTION OF VENTILATOR-ASSOCIATED PNEUMONIA IN MECHANICALLLY VENTILATED PATIENTS IN ICU This thesis is part of preliminary caring quality developing programme organized by the Laurea University of Applied Sciences and the Helsinki and Uusimaa Hospital District (HUS), HUCH-Hospital-operative industry and within there intensive care unit (ICU), coronary care unit (CCU) and acuteward (POS). The aim of our thesis was to develop nursing of mechanically ventilated patients and intention was to create evidence-based recommended practices to the nursing of mechanically ventilated patients and how to prevent infections in mechanically ventilated patients in ICU. Ventilator-associated pneumonia (VAP) is the most common infection within the ICU. VAP develops in some of the mechanically ventilated patients with in 48 hours after the intubation. One criteria on the diagnostics is that patient didn’t have pneumonia prior to the intubation and beginning of the mechanical ventilation. Ventilator-associated pneumonia is acquired most commonly by aspiration of nosocomial bacteria to the lower respiratory ducts or by aspiration of bacteria in the gut. The endotracheal tube can be contaminated. Risk factors to require VAP are: male sex, existing pulmonary disease and multiple organ failure. Internationally there have been established evidence-based recommended practices to prevent ventilator-associated pneumonia. The project was started in October 2010 and reached its conclusion in April 2011. Students created recommended practices to prevent ventilator-associated pneumonia by using systematic review. The recommended practices are presented in process order. In total 26 practices were developed. The recommended practices are based on evidence, tried and found methods and experience of the nursing staff. The recommendations were divided into six categories: general prevention, prevention of aspiration, oral care, suction technique, nutrition and other preventative measures for VAP. The project proceeded according to the schedule and enhanced students professional skills and development of expertise. In the future the created evidence-based recommended practices can be updated....

Ohjelehtinen unettomuuden lääkkeellisestä ja lääkkeettömästä hoidosta kotihoidon työntekijöille yhteistyössä Tapiolan apteekin kanssa

Unettomuus on erittäin yleinen vaiva erityisesti ikäihmisten keskuudessa. Aikuisista 5-12 % ja vanhuksista jopa 15 % kärsii unettomuudesta. Suomessa unilääkkeiden käyttö on varsin yleistä. Unettomuutta tulisi kuitenkin ensisijaisesti hoitaa lääkkeettömästi. Mikäli lääkehoitoa kuitenkin tarvitaan, tulisi sen olla lyhytaikaista. Pitkäaikaisesta lääkehoidosta ei ole todettu olevan hyötyä unettomuuden hoidossa.
Kotihoidon asiakkaista suurin osa on ikäihmisiä. Heidän keskuudessaan unettomuus ja unilääkkeiden käyttö on yleistä. Apteekin farmaseutit ovat kokeneet kotihoidon asiakkaiden ohjauksen haasteelliseksi. Kotihoidon työntekijät hakevat lääkkeet asiakkaille, jolloin apteekin farmaseutit eivät tapaa asiakkaitaan henkilökohtaisesti. Lääkkeitä käyttävien asiakkaiden itsehoidon edistäminen apteekkilaitoksen ja terveydenhuollon yhteistyönä –hankkeessa pyritään kehittämään alojen välistä yhteistyötä.
Opinnäytetyön tavoitteena oli tehdä työelämää palveleva ohjelehtinen unettomuuden lääkkeellisestä sekä lääkkeettömästä hoidosta kotihoidon työntekijöille. Tarkoituksena oli lisätä kotihoidon asiakkaiden saamaa ohjausta unettomuuteen liittyen. Lisäksi haluttiin korostaa unettomuuden lääkehoidollisen ohjauksen lisäksi myös lääkkeettömiä hoitokeinoja.
Ohjelehtiseen tarvittava aineisto kerättiin kirjallisuuskatsauksen avulla. Ohjelehtisen sisällön lääkehoidolliset valinnat tehtiin yhteistyössä apteekin edustajan kanssa. Ohjelehtisen onnistumista arvioi kotihoidon työntekijä puolistrukturoidussa haastattelussa.
Kotihoidon sairaanhoitaja arvioi ohjelehtisen kokonaisuutena onnistuneeksi. Hän koki voivansa hyödyntää sitä omassa työssään. Ohjelehtistä tehtäessä noudatettiin huolellisesti Terveyden edistämiskeskuksen terveysaineiston laatukriteereitä....

NOPS 2010 – 2012, Promotion of adolescents’ working and functional ability in vocational colleges : Occupational health discussion as a support in adolescents’ lifestyle changes This thesis was made as part of the project NOPS 2010 – 2012, Promotion of workability and functional capability among young people in vocational colleges. The main target of this project is to develop a model of operations where health care and social services students in university of applied sciences hook up as part of health promoting action in vocational colleges. The aim of this project is to promote adolescent’s healthy lifestyles as well as safe and healthy way of working. NOPS-project is developing health promotion methods that suit adolescents in vocational colleges. These kinds of methods are health events, health discussions and Tsemppi – action. In my thesis I took part NOPS – team’s action in Laurea Hyvinkää, where the team planned, carried out and evaluated individual occupational health discussions (n= 120) held with students in vocational colleges. The occupational health discussions for this thesis were held with students (n=14) of clothing study programme of Keuda Järvenpää. The occupational health discussions included evaluation of the state of health, reflection discussion, resource centered guidance and the making of individual health plan. Contents of the occupational health discussions were adolescent’s life situation, lifestyles, occupational health and safety, change in health behavior and the effectiveness of the discussion. The reflection discussion of the occupational health discussion proceeded with interventive questions. The reflection discussions were studied qualitatively. The discussions (n=14), which were recorded, were carried out following theme interview principles. The results of the evaluation of the state of health and health plan were written as documents. The material consisting of recordings and documents were analyzed with the principles of the classification of the theory based content. The occupational health discussions indicated that more than half of the students ate breakfast every day. Half of the adolescents took enough exercise to maintain their health. Almost every student slept 6-8 hours a night during the school week. Almost half of the adolescents smoked. Half of the adolescents did not drink alcohol at all. Most of the students told that they suffered from neck-shoulder and/or back pains. Nearly all the students brought out problems which are related to the work ergonomics. The health plans included the most things in relation to nutrition as need for change and as objectives. Occupational health and safety are themes that need to be discussed further with the students at different stages of their studies....

First aid for 9-12 year olds – guide for teachers and instructors The purpose of this thesis was to produce an instructor’s/teacher’s guide to help teaching or planning first aid lessons for 9-12 year-olds. The guide provides information of the most com-mon accidents and their first aid. 9 - 12 – year-old children are several hours at home with just their siblings or by themselves. There are lots of different tools and liquids which can cause accidents, cuts and intoxications. However, first aid knowledge is not necessarily so good that they can survive by themselves. First aid teaching in school is minimal and there is not enough teaching material available. Consequently, practice of first aid skills practice is minor. After school activities are not enough in improving first aid skills. The purpose of this guide is to provide teaching material for teaching theory and practice of first aid. In the guide book for teachers and instructors, there is basic knowledge of how to teach first aid. For children there is a slide show which has been done to help them to do first aid in practice. The idea of this first aid guide book came from a person who works with children. The guide book is aimed at everyone who works with children. It has been tested in real life situation and it has been effective. The guide book received positive feedback from the instructor and the children. Infant infectious diseases Home care instructions This thesis examines common infectious diseases and their home care among under 1-years-olds. During the first year of their life children suffer on average from 5-10 infectious disease episodes. Newly-born babies are protected by the mother’s antibodies, but antibodies will disappear before the baby is six months old and many infectious diseases can occur. The most important and largest group of children's illnesses are infectious diseases. This study is part of the project Lapsen ja nuoren hyvä arki organized by Laurea University of Applied Sciences. Funding for the project has came from the Centre for Economic Development, Transport and the Environment. The purpose of this study was to prepare a literature-based brochure for parents. The most common childhood infectious diseases and their home care instructions were included in the brochure. The brochure is intended for the City of Hyvinkää public health nurses and as support material for distribution to parents during pregnancy. The brochure also provides the families with information about infections diseases and how to treat them. Content of the brochure was prepared on the basis of the theoretical framework of the thesis. The brochure was designed in collaboration with maternity and child welfare clinic in Hyvinkää. Guidance about Infectious diseases is important in the maternity and child welfare clinics. If a child gets ill parents may be uncertain, scared and anxious. Guidance that is based on adequate and good interaction reduces these feelings. A sick child's parents often first take contact a familiar nurse for information and guidance. Good guidance is based on researched and tested information. Successful guidance requires good education, solid theoretical knowledge, good management practices and interaction skills. The data for of the brochure was gathered verbally and through e-mail from parents with children. Assessing was aimed to determine whether the text was understandable and whether the home care instructions were sufficient to parents. The brochure was handed over as an electronic version to Hyvinkää maternity and child welfare clinics. The brochure is a Word file attached to the work....
