Description of the current drug treatment situation at the service centre for elderly in Lohja city This dissertation is the final project of development action made within a given framework of Safe Drug treatment 2 project. Safe Drug Treatment 2 project partner was city of Lohja and the target was to develop the drug treatment safer at the Home care units and at the service centre for elderly. The purpose of this development work was to describe the current drug treatment situation in the wards Helmi and Hermannni and Akseli and Elina at the service centre for elderly. The development action team was made up by three nurse students and from the working life two registered nurses, one practical nurse and one practical nurse in social and health. The development action project is based on the Ministry of Social Affairs and Health guide of Safe Drug Treatment whose purpose is to determine the minimum demands of the actualization of drug treatment, standardize the principles of drug treatment actualization and clarify the responsibility divisions in the drug treatment process. The development was carried out by dissemination method in qualitative research. In the dissemination method, the current situation of the drug treatment was found out by interviews. The information gathered from the interviews was edited by transcriptions and content analysis to describe the current drug treatment in a fish bone model. The research information was gathered to evaluate and it was based on the Ministry of Social Affairs and Health guide of Safe Drug Treatment. By contrasting the description of the current situation with the theoretical and experience information, were able to reveal the good practices and the development targets of the drug treatment process. In order to evaluate the development action, the work community was provided with a quality evaluation form the purpose of which was to find out the opinions of the work community about the development action’s significance for the development of the drug treatment at the units. 68 per cent of the provided evaluation forms was returned. The activity of the work community has a positive meaning to the evaluation of the development action and the development of the drug treatment. The Ministry of Social Affairs and Health recommends that every public health service unit should have a drug treatment plan. The preparation of the drug treatment plan is made easier by the description of the current drug treatment situation. Good practices and development targets have been stand revealed in the current drug treatment situation at the units and based on that the drug treatment process can be developed in the future. The drug treatment license procedures are the next target of development at the units in the Safe Drug Treatment 3 project. ...

Muistihäiriöpotilaan kuntoutus pitkäaikaishoidossa-hoitajan näkökulma

Opinnäytetyön tarkoituksena on kuvata, mistä tekijöistä muistihäiriöpotilaan kuntoutus pitkäaikaishoidossa
koostuu hoitohenkilökunnan näkökulmasta. Sekä selvittää mitkä tekijät edistävät ja estävät kuntoutuksen toteutumista.
Toteutan tämän työn kirjallisuuskatsauksena. Aineiston analysoin laadullisella sisällönanalyysilla teorialähtöisesti. Aineiston analyysia ohjaavat kuntoutuksen yläkategoriat nousivat teorian pohjalta. Opinnäytteessä on kaksi opinnäytetyökysymystä, joihin pyrin löytämään vastaukset. Aineistona tässä kirjallisuuskatsauksessa on käytetty yhdeksää kotimaista tutkimusta vuosilta 1999- 2007. Tutkimuksista seitsemän on pro-gradu tasoisia ja yksi Sosiaali- ja terveysalan tutkimus- ja kehittämiskeskuksen tuottama havaintotutkimus sekä yksi Turun kaupungin teettämä havaintotutkimus. Tuloksista kävi ilmi, että hoitohenkilökunnan näkökulmasta kuntoutus koostuu useista eri elementeistä, jotka limittyvät toisiinsa. Näitä elementtejä ovat moniammatillisuus, turvallinen hoitoympäristö, omaisten osallistuminen. Lisäksi mainittiin kuntoutusprosessiin liittyviä tekijöitä sekä edellytyksiä hoitohenkilökunnan ammatilliselle osaamiselle.Hoitohenkilökunta kuvasi kuntoutusta hyvin monipuolisesti. Kuntoutuksen yleisenä tavoitteena on kuntoutujan kokonaisvaltaisen toimintakyvyn ylläpitäminen ja sen edistäminen. Kuntoutustaedistävät tekijät voi jakaa hoitajaan itseensä liittyviin tekijöihin, työyhteisön toimivuuteen ja kuntoutusprosessiin liittyviin osa-alueisiin. Estäviä tekijöitä kuntoutukselle ovat organisaatioon liittyvät tekijät, hoitajan ammatillisten valmiuksien puute. Merkittävimpänä esteenä kuntoutukselle on se, ettei kuntoutuksen merkitystä ymmärretä.

Draamaopetus tupakasta Martinlaakson lukion terveystiedon tunnilla

Tupakkakokeilut aloitetaan yleensä 12–14 vuoden iässä. Suomessa 14-vuotiaista pojista ja tytöistä yli kolmasosa on kokeillut tupakointia. 16- 18- vuotiaista pojista ja tytöistä lähes kolmasosa tupakoi päivittäin. (Kansanterveyslaitos 2006, 19.) Terveys 2015 -kansanterveysohjelmassa tärkeimpiä nuorten terveyden edistämisen tavoitteita on 16–18-vuotiaiden tupakoinnin vähentäminen alle 15 prosenttiin vuoteen 2015 mennessä. (Stakes 2002, 70- 71.) Lukiolaisista noin kymmenen prosenttia ilmoittaa tupakoivansa päivittäin.( Kouluterveyskysely 2008)
Opiskelijaterveydenhuollon tavoitteena on koko kouluyhteisön hyvinvoinnin ja oppilaiden terveyden edistäminen sekä terveen kasvun ja kehityksen tukeminen yhteistyössä oppilaiden, oppilashuollon muun henkilöstön, opettajien ja vanhempien kanssa.
Opinnäytetyössä perehdyttiin nuorten tupakointiin toiminnallisen draamaopetuksen kautta. Työ toteutettiin Martinlaakson lukion 1- 2-vuosikurssin opiskelijoille. Työ on osa Vantaan ehkäisevän päihdetyön ja Laurean yhteistyötä. Työn tarkoituksena oli johdatella lukiolaisia itse löytämään ratkaisuja tupakoinnin aiheuttamiin terveyshaittoihin ja lisätä heidän tietämystään tupakoinnin lopettamisesta. Ajatuksena on tuoda yhdessä esiin oppilaiden asenteita ja kokemuksia tupakasta.
Opetus toteutettiin draaman kautta. Tunnin aluksi johdateltiin oppilaat aiheeseen ja esitettiin väittämiä tupakasta. Tarkoituksena oli, että päihdevalistuspäivästä tulisi toiminnallinen ja mieluisa oppimiskokemus oppilaille. Draamatehtävänä oli näytellä jonkin henkilön huono päivä. Jokaiselle ryhmälle annettiin eri tukisanat, joita tuli käyttää esityksissä. Tukisanat olivat tupakka, nikotiini, nuuska ja vierotusoireet. Lisäksi oppilaat saivat yhdessä keksiä muita avainsanoja ja käyttää niitä esityksessä.
Draamaopetus on hyvä valistamismuoto nuorille, sillä heillä on jo paljon tietoa tupakoinnista....

Substance abuse of over 65 year old patients in home care - evaluation of a questionnaire form This thesis is part of a development project of alcohol and substance abuse counseling services by Verso, The Centre of Expertise on Social Welfare of Päijät-Häme and Itä-Uusimaa region.The project began in March 2008, and was finalized during the end of project seminar held in Loviisa city in October 2009. The main object of the project was to gather information about the alcohol consumption habits and alcohol abuse of over 65 year old patients in home care in Itä-Uusimaa region. Furthermore, the object was to develop new services and protocols or dealing with elderly people suffering from alcohol abuse, and for identifying alcohol abuse at an early stage among the elderly in home care. The method for gathering this information was an alcohol abuse questionnaire form created by city of Tampere. The form was adapted for this project. The purpose of this thesis was to test how suitable the questionnaire form is for identifying alcohol abuse at an early stage. In addition, the purpose was to instill the form as a new working method when dealing with elderly people who have such problems. A guide by the Ministry of Social Affairs and Health, named “Let’s Find Out!”, was used as a tool along the questionnaire form. Three research questions were set: 1) Did the alcohol abuse questionnaire form produce desired information about the alcohol consumption among home care patients over 65 years old? 2) How did the home care patients experience the use of the questionnaire form and the “Let’s find out!” -guide? 3) How did the home care staff experience using the questionnaire form and the “Let’s find out!” -guide? The material for the alcohol abuse among elderly people project was gathered in February 2009 during home care visits. The interviews for the questionnaire were carried out by the home care staff. The material for this thesis was then gathered via questionnaire form send by mail in May 2009 to the home care staff who participated in the project. Quantitative methodologies were used in the analysis of the material, and frequencies and percentages were highlighted.The results showed that the alcohol abuse questionnaire form suits well home care as a working method, and it gave enough information about the alcohol consumption habits among home care patients who are over 65 years old. Furthermore, the home care staff were pleased with the form as a working method. The drug questionnaire form could be further developed by simplifying its content, and by adding a question about the background and reasons for drinking alcohol. In addition, some guidance or training could be organized for the staff who use the form in their work....

Yksilövastuisen hoitotyön toteutuminen terveyskeskussairaalan vuodeosastojen potilaskohtaisissa kirjauksissa

Primary nursing realization in health center hospital wards The purpose of this study is to find out how primary nursing appears in patient documentation in wards in Katriina hospital. The city health care center hospital of Vantaa, Katriina, is renewing its service structures with the goal to develop its primary nursing system and the competence of its staff. The aim of this study is to obtain information for the hospital for their work to develop nursing. The material included 447 pages of patient documentation of the Katriina hospital, printed from their patient documentation system Graphic Finstar in November 2008. The material consisted of three types of patient-specific documents from eight hospital wards. The documents were nursing plans, summaries of treatment periods and daily treatment follow ups as one-day samples. Patients were selected by random sampling by ward. The patient sample amounted to 98. The material was analyzed using both quantitative and qualitative methods that are complementary to each other and provide the most comprehensive picture of the material. Results were obtained by seeking information whether patient’s prerequisites are documented in the nursing plan, whether the part of primary nurse is showing in the documentation and whether the participation of the patient or patient’s relatives during hospital treatment period is shown in the documentation. Results showed that in the health care center hospital’s documentation the appearance of primary nursing varies widely and there are major differences in the documentation between wards. Prerequisites in the nursing plan were documented fairly extensively but the primary nurse’s involvement in creating the documentation does not fulfill the criteria of primary nursing. In the documentation of the patient’s involvement in the treatment most entries were found in context to general consideration. The lowest amounts of entries were related to nutrition. In the documentation, the involvement of relatives was mostly shown in matters regarding follow-up care. Overall, the results show that primary nursing carries out moderately in patient-specific entries. Development of the primary nurse role must continue and the documentation between hospital wards must become more congruent....
