Ikääntyvän asiakkaan kotiutusprosessi Espoon kotihoidossa
Opinnäytetyömme tarkoituksena oli selvittää ja kuvata ikääntyvän asiakkaan kotiuttamisprosessin toteutumista Espoon kotihoidossa. Opinnäytetyömme vastasi seuraaviin kysymyksiin: Miten ikääntyvän asiakkaan kotiutusprosessi toteutui Espoon kotihoidossa hoitajien kuvaamana, millainen kotiutusprosessi oli Espoon kotihoidossa ja millaisia vaiheita se sisälsi sekä mitä puutteita ja kehitysideoita siinä ilmeni.
Haastattelimme kohderyhmänämme ollutta Espoon kotihoidon henkilökuntaa. Haastattelussa selvitettiin heidän näkökulmaansa kotiutusprosessin kulusta ja siinä ilmenneistä ongelmista sekä kehittämisideoista. Opinnäytetyössämme käytimme aineistonkeruumenetelmänä teemahaastattelua, joka suoritettiin ryhmähaastatteluna kotihoidon henkilökunnan kesken. Haastattelussa oli läsnä kymmenen hoitoalan työntekijää. Haastateltavien keski-ikä oli 48,3 vuotta ja keskimääräinen työkokemus 18,5 vuotta.
Opinnäytetyömme perustui kvalitatiiviseen lähtökohtaan ja analysoimme aineistomme laadullista menetelmää tukevalla sisällön analyysilla. Haastattelun analysoinnissa nousi esiin kotiutusprosessin kulku sekä siinä ilmenneet epäkohdat ja kehittämistarpeet. Hyödynsimme QPR Process Guide -ohjelmistotyökalua kotiutusprosessin aukaisemisessa ja sen avulla teimme myös kotiutuksesta prosessikartan. Saamamme tulokset kotiutusprosessista Espoon kotihoidossa olivat yhtäläiset aikaisempien tutkimusten kanssa ja tukivat niitä....

Youth’s health promotion with health theme day This thesis was carried out in a project called The Young, Health and Wellbeing, which is a cooperation project between Laurea University of Applied Sciences and Kuitinmäki upper comprehensive school. The project was carried out in 2006—2009 and it provides the students of Laurea University on Applied Sciences a chance to carry out innovative thesis, learn by developing and participate in developing the working life. According to the project’s name, the young, health and well-being are some of the key words of this thesis. The health of the young has been examined on the basis of the school health inquiry, which is a national research carried out yearly on the health of the young. Health education and health literacy are also surveyed because they create the foundation to young people’s health. However, promoting the health of the young is the purpose of this thesis and empowerment is a current health promotion method to encourage everyone to promote their own and their surroundings’ health. The functional goal of this thesis was to plan, organize and evaluate a health theme day for the 7th graders of Kuitinmäki upper comprehensive school. The theme day was carried out on 29 April in 2008. A same kind of a theme day was carried out also in spring 2007 and was also then a part of a thesis. The developing proposals from the first theme day were considered while organizing the theme day in 2008 and the goal was to develop the day to be more tempting and more empowering from the perspective of the pupils. The theme day was implemented by public health nursing and physiotherapy students, who arranged interventions to the 7th graders. The subjects of the interventions were nutrition, sexuality, venereal diseases, mental health, posture and sleep and rest. The subjects were based on the preinquiry which the pupils answered three months before the theme day. The meaning of the preinquiry was to make the theme day to answer to the pupils’ needs and wishes. The theme day was evaluated with a questionnaire, which the pupils and the public health nursing students answered to at in the end of the theme day. Informal observation was used to support the evaluation. It appeared that the pupils liked the theme day, because it was deviant to normal teaching and they were especially fond of the active interventions. About 60 percent of the pupils said they received new knowledge and about a half of the pupils thought that the subjects of the interventions were interesting and increased their motivation to promote their own health....

Nursing Cultures Collide: Finland and Tanzania The purpose of this thesis was to find out experiences of social and healthcare trainees and volunteers about their international exchange in Tanzania. Thesis focused to study differences between nursing cultures from an ethical point of view. The goal was to create support material for students and volunteer workers planning to go, for example, to Tanzania. The research problems studied included the differences between Tanzanian and Finnish nursing cultures, patients’ rights and nurses´ ethical duties, students´ experiences of practical training and volunteer work abroad and meaning of it by the professional growth point of view. The study was qualitative and the approach was phenomenological, in other words focus was on the experiences of the informants. The data was gathered with open-ended questions which the informants answered with essays. The questions were categorized in themes. The informants were social and healthcare students in volunteer work and clinical training abroad in Tanzania. Questionnaires were sent to the informants (n=6) and four (n=4) of them were returned. Content analysis was used to analyze the data. The results showed the variety of differences between the nursing cultures studied. The informants had thought a lot about ethical issues during their exchange. By the informants, treatment of patients, patients´ rights and wealth were the most significant ethical differences compared Tanzanian health care and Finnish health care. A major difference between nursing cultures that the informants brought up was the importance of family in Tanzanian nursing. Western healthcare is highly developed and the latest medical inventions are used. Tanzania is a development country so differences between cultures are huge. Medical equipment and methods, degree of asepsis and concept of time were experienced as remarkable differences. Experiences of practical training abroad were positive and it was considered as conductive thing of professional growth. Skills training opportunities abroad were excellent learning environment by the informants. It had a clear role students and volunteers to achieve language skills and international professional qualifications. Confidence and trust in their own skills had increased during the exchange. According to this thesis, meaning of international exchange to employment and professional identity could be studied. Multicultural confluence in healthcare presupposes open-mindedness, mutual understanding and interaction skills. Difficulties may arise, for example, misunderstandings and cultural differences. The unwritten rules of daily living become visible when somebody acts against them. Each nurse should be aware of the cultural linkage of their own conceptions and behaviour. Questions clear things and makes co-operation easier. The nursing cultures and methods are different, but both participants can benefit from each other’s perspective. What is daily to another might be inconceivable to the other. International nursing is a vision of the future that we create with open-mindedness....

A Study about a mental health rehabilitant’s chances to find a job in Hyvinkää region The purpose of this thesis was to study what kind of chances the mental health rehabilitants had to find a job in the Hyvinkää region and how to guide mental health rehabilitants to find a job and give information about organizations and peer support to help the mental health rehabilitants’ employment. In this thesis the area of Hyvinkää refers to the region around of towns of Hyvinkää and Riihimäki and the municipalities of Nurmijärvi, Hausjärvi and Loppi. The thesis was commissioned by a service home belonging to Human Care Network. The staff of the service home needed more information about applying job and how to guide the mental health rehabilitants to find work out side of the working halls. The study was conducted between autumn 2008 and autumn 2009. The research material was gathered by interviewing the educational manager of Finnish Central Association for Mental Health Mika Vuorela and five mental health rehabilitants from one service center for mental health rehabilitants. The thesis was qualitative and phenomenological. The material of this thesis consisted of qualitative research material, gathered by an open individual interview and group theme interview. We used a phenomenological perspective in our thesis. We wanted to use a phenomenological perspective because in this perspective the research material is analysed without prejudices. The qualitative material was analysed by phenomenological method. The results showed that finding a job depended on the mental health rehabilitants’ personal skills and the current employment situation. The interviewing showed that there were not separate jobs for the mental health rehabilitants. However, there were employers who were especially favourable for the mental health rehabilitants. In spring 2009 the mental health rehabilitants had been able to find jobs in janitorial service, service branch or a seasonal job in the South of Finland. The mental health rehabilitants had had little support for employment and they knew little about different kind of supportive organizations. Working in the working halls didn’t help the rehabilitant to find a job. Help in flexible arrangements and working instructions are needed....

The Empowerment of Mental Health Clients with Group Action - Ways to Control Health to Promote Well-being and Carrying Health Response The thesis was part of the project “In the Limit of Work” of Laurea University of Applied Sciences including the project “Mind’s movement – keys of well-being” of the Mental Health organization of Espoo EMY. The goal of the project is to create a model of empowerment work. The goal of “Mind’s movement - keys of well-being” is to promote psychic, physical and social well-being of mental health clients. The purpose of this thesis was to promote mental health clients’ ways to control health and the goal was the empowerment of mental health clients with group action to promote well-being and carrying health response. This thesis was actional including ten group meetings promoting well-being every other week 9.10.2009 - 26.2.2009. The two hour group meetings included for example motion, thinking of nutritional factors, exercises of relaxation, music and a visit to a museum. The eight members of the group included seven women and one man. Their average age was 51 years. In the beginning of the group action there were twelve members in the group from which four interrupted their participation in the group. Qualitative method was used in this thesis. Participating observation, a scale to measure empowerment and a paper to set goals were used as methods to collect material. Material content analyse was used to analyse the material. The things that came out in discussions with the group were exactly the same as observations. The participants filled in scales to measure empowerment in the beginning and in the end of the group action showing added to other material that empowerment had happened. In the end of the group action the members evaluated that the goals they set in the beginning of the group action were achieved well but they also found something to develop. Based on the results of this thesis purpose and goal of the thesis were achieved. In the future it might be possible to promote national health by the ways found working to lead group action in this thesis to promote well-being and carrying health response. Because there was qualitative method in question and the members of the group were already interested in health factors as joining the group the results can’t be generalized straight....
