Kirurgisen potilaan hoitotyö - simulaatiotilanteen rakentaminen

Opinnäytetyön tavoitteena oli Laurea-ammattikorkeakoulun Lohjan kampuksen simulaatio-oppimisympäristön kehittäminen. Opinnäytetyön tarkoituksena oli tuottaa kaksi kirurgisen potilaan simulaatiotilannetta Laurea-ammattikorkeakoulun oppimisympäristöön yhteistyössä Lohjan kampuksen hoitotyön opettajien ja sairaanhoitajaopiskelijoiden kanssa. Simulaatiotilanteiden aiheina olivat reisiluun kaulan murtumapotilaan kotiutus ja urologisen potilaan leikkauksen jälkeinen tarkkailu. Simulaatiotilanteet jäljittelivät todentuntuista ympäristöä tietynlaisen päämäärän saavuttamiseksi, joka oli tässä opinnäytetyössä sairaanhoitajaopiskelijoiden valmistautuminen työelämän potilastilanteisiin.
Tässä opinnäytetyössä teoreettisena taustana toimi kirurgisen potilaan hoitotyö. Simulaatiotilanteet rakennettiin ja suunniteltiin simulaatiopohjia apuna käyttäen. Simulaatiotilanteita suunniteltaessa otettiin huomioon kohdejoukon opiskeltu teoreettinen tieto ja koulutusaste. Näin varmistettiin simulaatiotilanteiden toimivuus.
Tuotetut simulaatiotilanteet arvioitiin käyttämällä laadullista tutkimusmenetelmää. Aineisto kerättiin teemahaastattelulla. Haastateltavina oli neljä sairaanhoitajaopiskelijaa, jotka olivat toimijoina simulaatiotilanteissa. Haastattelut nauhoitettiin ja litteroitiin. Saadun aineiston pohjalta tehtiin analyysi ja pohdinta. Aineisto analysoitiin laadullisella sisällönanalyysilla. Tuloksista haluttiin saada opiskelijoiden näkemys simulaatiotilanteiden kehittämisestä tulevaisuudessa.
Haastattelujen pohjalta ilmeni kehittämiskohteita simulaatiotilateiden esitietoihin, valmistautumiseen, lisämateriaaleihin ja lopetustavan tarkentamiseen. Simulaatiotilanteisiin toivotaan myös jatkossa enemmän opettajan tukea sekä selkeyttä siihen, miten opiskelijat toimivat simulaatiotilanteissa. Haastatteluista voitiin todeta, että simulaatiotilanteet eivät tule koskaan tuntumaan aidolta tilanteelta, mutta koulussa ne ovat yksi tärkein oppimista tukeva asia, jota tulevaisuudessa voidaan kehittää.
Tuloksista voitiin päätellä, että tässä työssä simulaatiotilanteissa käytetyt esitiedot ja materiaali koettiin riittäviksi. Opiskelijat mielsivät pienen ennakkotiedon hyväksi. Lisäksi ennakkomateriaali tuki simulaatiolanteeseen menemistä, sillä tuloksista kävi ilmi, että simulaatiotilanteet olivat toimijoille uusia tilanteita. Tuloksista selvisi myös, että ryhmätyötaidot ja kommunikaatio ovat simulaatiotilanteiden onnistumisen kannalta tärkeitä asioita.
Kehittämisehdotusten perusteella voisi tutkia, miten simulaatiotilanteet ovat muuttuneet, jos kehittämisehdotukset on otettu käytäntöön. Tutkimuksen voisi tehdä suuremmalla otoksella....

Synnynnäisesti sydänsairaan lapsen sairauden vaikutus vanhempien parisuhteeseen

Opinnäytetyön tarkoituksena oli selvittää, miten lapsen synnynnäinen sydänsairaus vaikuttaa vanhempien parisuhteeseen. Aihe rajattiin koskemaan sydänlasten vanhempien parisuhdetta. Opinnäytetyön tulosten perusteella on tarkoitus laatia synnynnäisesti sydänsairaan lasten vanhemmille esite vertaistuesta ja voimavaroista, joilla voidaan ylläpitää parisuhdetta vaikeassa tilanteessa. Tavoitteena oli saada opinnäytetyön tuloksista tukea omaan työhön tulevana terveydenhoitajana.
Työ tehtiin työelämän kehittämiseksi yhteistyössä Sydänlapset ja –aikuiset ry:n kanssa. Tutkimusaineisto kerättiin synnynnäisesti sydänsairaiden lasten vanhemmilta internetin suljetulta keskustelupalstalta. Aineistonkeruumenetelmänä toimi verkkotutkimus. Saatu tutkimusaineisto analysoitiin induktiivista sisällönanalyysiä käyttäen.
Tuloksista tuli ilmi, että suurin osa vastanneista koki parisuhteen olevan syvempi ja vahvempi kuin ennen sydänlapsen syntymää. Heistä alle puolet kokivat ajan puutteen haasteena, jonka vuoksi parisuhde jäi lapsen sairauden varjoon. Tutkimuksesta nousi esille parisuhdetta vahvistavina tekijöinä yhteisen kokemuksen tunne, yhteenkuuluvuus ja arjen voimavarat, kuten yhdessä asioista puhuminen, puolison tuki ja jaksamisen vuorottelu. Lapsen sairaudentilan vakiinnuttua vanhemmat kokivat kykenevänsä viettämään normaalia perhe-elämää. Kukaan vastanneista ei ollut päätynyt eroon.
Vanhemmille kerrotaan yleisesti paljon informaatiota lapsen sairaudesta, jonka vuoksi avoterveydenhuollossa tulisi kiinnittää entistä enemmän huomiota vanhempien parisuhteen ylläpitoon ja hoitoon. Kehittämisehdotus avoterveydenhuollon hoitohenkilökunnalle voisi olla vanhempien tuen tarpeen ja avun tarjoamisen huomioiminen nykyistä enemmän myös alkujärkytyksen jälkeen. This thesis was carried out for the Finnish Association for Mental Health, Mental Health Art Week event week. The purpose was to clarify how the participation in the event in question affected the mental health of the people, who had participated in the study. Furthermore, the thesis discusses the significance of communality for the participants' wellbeing and produces information about how the event was experienced. The objective was to produce information to the client to develop the event week. The thesis was carried out as a qualitative study and the data was collected with a questionnaire. The inquiries were made in May 2015 and there were altogether 42 interviewees, in the group there were both men and women. The interviewees' age distribution was 20-57 years. The data was analyzed using content analysis. The results show that the participation in the event increased holistic wellbeing and zest for life. The participation created positive feelings and experiences of success, as well as enabled the handling of one's own thoughts and relaxation. The participation in the event expanded the participant's social capital and the group activity was regarded as a pleasant activity in which they would like to participate. It was experienced that the group gave energy and motivated to social interaction. The results showed that the art and cultural activities promoted maintaining and increasing holistic wellbeing. The obtained results can be used in mental health rehabilitation or for example in early interference....

The topic of this study is pain management for end-of-life care patients with terminal illness. This study was performed as a literature review. Our objective was to develop pain management in hospitals for terminally ill patients. The literature review was aimed to find good pain treatment therapies, both pharmalogical and non-pharmalogical therapies, focusing on the patient’s perspective. The study was structured as follows: defining the research question, searching for the original research papers, selecting the applicable research papers for the study, analyzing and presenting the findings. The data were retrieved from health care databases MEDIC, CINAHL (EBSCO), SAGE using Laurea’s Libguides portal. The early spring 2016 the search results were analyzed with inductive content analysis. The searches for studies and publications were carried out from year 2006 to 2015. Inclusion criteria was that the study needed to involve only adult patients either hospitalized or in palliative end-of-life care. Patients that received palliative care in home like circumstances were not included. From MEDIC database 6 studies and articles were included. From Journals no studies could be identified. From CINAHL database 3 studies and articles were included into the study. All studies were carefully examined and the original English papers were translated into Finnish. Relevant points were collected from the studies based on the research questions and they were divided under appropriate headings. The central findings showed that pain management for end-of-life care patients is, at least in theory, invested in to a significant extent. Different strains of opioids are the most common pharmaceutical products used, and the single biggest patient group was those with advanced stages of cancer. Only a scant amount of research was found on non-pharmalogical methods in pain management in end-of-life care. This could be a topic for further research, especially because of the adverse effects strong medication can cause....

The purpose of this study was to designate how the equality of the elderly appears in society. The goal was to find out what current research tells about empowerment. The research questions were: What is empowerment? What does it mean to empower the elderly and in what ways does mobile technology support the empowerment of the elderly? In upcoming years, the increasing number of people over 65 creates challenges when planning welfare services. The task of public power is with the communities, to improve the equality of the elderly in society. The elderly are healthier and they want to live independently, in their own homes. Everyone affects their own health and well-being with their own way of living. To secure the functional ability of the elderly, to improve their quality of life and to narrow health differences, we do need new preventive, personalized and technological services. Dignified aging includes the right to be an equal member of society. My study is a qualitative and descriptive literature review. The material to this study was selected from the years 2010-2016. The Materials to this study was collected from the Finnish databases Nelli, Medic, Melinda, Laurea Finna, and Google Scholar, and by manually searching from the magazines of the subject of the study. 11 Finnish articles and studies were chosen as the material of this study. The topics were analysed by using content analysis. The results in this study are based on the literature review. The results designate that empowerment can be defined in many ways. Empowering a human means supporting them in their individual changes. How do we empower an elderly person? To empower elderly people they need to be given individual and specific information. The empowerment of the elderly to use mobile technology must be supported through benefits. This Study was made for Sipoo community's elderly services. The modern concept of empowerment came to the fore from the material of this study. The goal of empowering is always a qualitative change in a human's life. A merger of a day hospital and an endoscopy unit in Helsinki and Uusimaa hospital district form a base for this thesis. The execution of thesis was functional. The goal was to develop the operations of the new day hospital/endoscopy unit by creating a description of the cur-rent use of working time of the nurses. The purpose of the thesis was to map nurses’ working time at the day hospital and to focus on time used in different operations at their current work. The development project of the future day hospital/endoscopy unit used this thesis in its initial planning. The health care and nursing professions are in a constant change. According to some surveys a tendency towards outpatient care is apparent. As the set of skills needed in different operat-ing environments change, it is important to review nurses’ role in each individual environ-ment. Nurses skills and use of those skills depends in which environment they are working. The data collection method used in this thesis was observation of the working time of the nurses. This was done in January 2016. Through observation we wished to examine how the working time was divided between direct and indirect care as well as external functions. The data analysis was done by a so called coding technique and the results were transferred to a table describing the nurses’ use of time in a day hospital. The results of this thesis show that most of the nurses’ working time in a day hospital is used in external functions. However, these functions are an integral part of the care work, for ex-ample, department specific operations such as department hours and securing the order in the general facilities. The share of indirect care was nearly double in comparison to the di-rect care. The indirect care includes all care work where the patient is not present, such as preparation of the care and documenting the given care. The environment influences how the nurses divide their working time. According to some pre-vious studies the working time is divided differently in wards and polyclinic environments. The results also show that majority of the nurse’s working time will be used in updating data systems, for example documenting patient’s care work or scheduling appointments. These days computers have an essential role in care work and more and more time is used in different administrative and data processing tasks. In order to improve the time management and to increase the time used in direct care a reduction in time used in administrative work is needed. This can be done by for example increasing the functionality of the operations and by simplifying the processes used in care work. By simplifying documentation systems nurses would have more time to patients direct care, for example supporting patients.
