Torakolumbaalisen faskian liikkeen manuaalinen mittaaminen

Abstract

Tämän opinnäytetyön aiheena on torakolumbaalisen faskian liikkeen manuaalinen mittaaminen. Työn tavoitteena oli selvittää, kuinka tarkasti torakumbaalisen faskian pinnallisen ja syvän posteriorisen kerroksen liikettä voidaan mitata palpaatiolla ultraäänikuvantamiseen verrattuna. Faskiat ovat tänä päivänä yhä puhutumpi aihe fysioterapiassa, koska niiden toimintaa ja merkitystä ihmiskehossa aletaan ymmärtää yhä paremmin. Sen johdosta onkin tärkeää selvittää, kuinka luotettavasti niiden liikettä olisi mahdollista palpoida. Aihe työhön tuli työn tilaajalta, Jouko Heiskaselta.

Opinnäytetyön tarkoituksena oli toimia pilottitutkimuksena ja lisätä fysioterapiaopiskelijoiden sekä fysioterapeuttien tietoisuutta faskian manuaalisen palpaation tarkkuudesta ja teoreettista osaamista faskiarakenteista. Opinäytetyö toteutettiin määrällisenä tutkimuksena ja siinä käytettiin kartoittavaa otetta.

Raportti koostuu teoriaosuudesta, jossa esitetään perusteet faskioiden, sekä tutkimuskohteena olevan torakolumbaalisen faskian toiminnoille ja liikkumiselle. Tutkimusosuudessa käydään läpi opinnäytetyön prosessin kulku, tutkimusprosessin kulku, tutkimuksen tekeminen, tutkimus ja aineiston analysointi menetelmät sekä tutkimustulokset.

Tutkimustuloksena saatiin, että torakolumbaalisen faskian pinnallisen posteriorisen kerroksen liikkeen suuntaa voidaan palpoida 96 %:n tarkkuudella ja syvän posteriorisen kerroksen liikkeen suuntaa 55 %:n tarkkuudella. Tarkkaa liikkeen määrää pystytään pinnallisen kerroksen osalta palpoimaan 40 %:n tarkkuudella ja syvän kerroksen 28 %:n tarkkuudella.

The subject of this thesis is the reliability of manually measuring the movement of the thoracolumbar fascia. The aim is to find out how accurately the surface and deep posterior layer movement can be measured by the means of palpation, as opposed to ultrasound imaging. Fasciae are increasingly talked about in the filed of physiotherapy because of the increased understanding of their function and significance in the human body. Thus, it’s important to find out how accurately their movement can be palpated. The topic was given to us by the client Jouko Heiskanen.

The purpose of this thesis is to increase the awareness of the accuracy of manual fascia palpation, and theoretical expertise of fascia structures among physiotherapist students. This thesis was carried out as a quantitative research with a charting approach. The report consists of a theory section, in which the basics of fasciae and the main focus of this thesis, the functions and movement of the thoracolumbar fascia, is discussed. The research section covers the process of the thesis, the process of the research study, the actual research, ways of analyzing the research and its material, as well as the conclusive research results.

The research found that the direction of the surface posterior layer movement of the thoracolumbar fascia can be palpated with 96% accuracy and that the direction of the deep posterior layer movement with accuracy of 55%. The precise amount of surface layer movement can be palpated with 40% accuracy, whereas the deep layer movement with 28%.
