Päivystävän laboratoriohoitajan ammattiosaamisen ylläpito

Abstract

Opinnäytetyö käsittelee päivystävän laboratoriohoitajan ammattiosaamisen ylläpitoa. Tämän työelämälähtöisen opinnäytetyön tarkoituksena oli selvittää kehittämistarpeita ja parannusehdotuksia päivystävän laboratoriohoitajan ammatillisen osaamisen ylläpitoon. Laboratoriohoitajan ammattiosaamisen lähtökohtana on laboratoriotutkimusprosessin osaaminen, joka sisältää preanalyyttisen-, analyyttisen- ja postanalyyttisen vaiheen. Päivystävällä laboratoriohoitajalla on oltava riittävä ammattitaito, jotta hän pystyy hallitsemaan laajoja kokonaisuuksia toimiessaan itsenäisesti päivystysaikana. Päivystävä laboratoriohoitaja on vastuussa omasta taitotasostaan ja hän pyrkii kehittämään jatkuvasti omaa ammattiosaamistaan. Osaamisen ylläpidolla tässä opinnäytetyössä tarkoitetaan jo opitun tiedon ja taidon hallintaa sekä uuden opettelua.

Opinnäytetyö toteutettiin keskussairaalatasoisessa kliinisessä laboratoriossa ja siihen osallistui 30 päivystävää laboratoriohoitajaa. Opinnäytetyön aineisto kerättiin ryhmähaastatteluilla ja kirjallisesti Kaizen-taulun avulla. Aineisto analysoitiin teemoittelulla ja sisällönanalyysillä. Aineiston sisällönanalyysin avulla saatiin kolme yläluokkaa, jotka olivat: henkilöstön työpistesijoittelu, osaamisen ylläpito vuorotyössä ja työssä oppiminen.

Päivystävän laboratoriohoitajan osaamisen ylläpitoon vaikuttavat työn toistot ja työpisteessä vietetty aika. Päivystysajan osaaminen hankitaan työskentelemällä arkivuoroissa. Osaamisen ylläpidon kannalta vuorotyö aiheutti tiedonkulun ongelmia. Nopeasti päivittyvät ohjeet ja niiden sisäistämiseen käytettävä aika nousivat esille kehittämistarpeina.

Osaamisen ylläpidon välineiksi nousi kolme kategoriaa: osallistavat ja nopeat välineet, pitkäntähtäimen välineet sekä tulevaisuuden osaamisen ylläpidon välineet. Osaamisen ylläpidon välineistä osallistavat ja nopeat ovat helposti käytettävissä päivystysluonteisessa työympäristössä. Opinnäytetyön tuloksia voidaan hyödyntää päivystävässä laboratoriossa. Päivystävän laboratoriohoitajan osaamisen ylläpidon välineet ovat vuorovaikutuksellisia, työssä oppimisen välineitä, jotka mahdollistavat hiljaisen tiedon jakamisen ja jalkauttamisen

The subject of this thesis is the maintenance of professional competence for a biomedical laboratory scientist on call. The objective of this working lifebased study was to examine the needs for development and proposals for improvements in the maintenance of the professional competence of a biomedical laboratory scientist on call. The starting point for the professional competence is the knowhow of the laboratory research process, including the preanalytical, analytical and postanalytical phases. The biomedical laboratory scientist on call must be professionally competent in order to manage large aggregates when working independently during the out of office hours period. The biomedical laboratory scientist on call is responsible for his or her level of skills and developing their expertise continuously. In this thesis, maintenance of the professional competence refers to the management of already learned know-how and also the learning of new skills.

The study was conducted in the clinical laboratory of a central hospital. 30 laboratory biomedical scientist on call took part in the study. The material for this thesis was collected through group interviews and in writing, using the Kaizen board. The material was analyzed, using thematic analysis and content analysis. The content analysis of the material resulted in the following three main categories: positioning of the personnel’s workpoints, mainte-nance of the competence in shift work and learning in work.

The maintenance of the competence for the biomedical laboratory scientist on call is influenced by the repetitiveness of the work and the time spent at the workpoint. Expertise for the out of office hours period is acquired by working during the regular working hours. For the maintenance of the competence, the shift work caused problems in terms of the flow of information. Frequently updated instructions and time spent internalizing these instructions were brought out as needs for development. Three categories were created as tools for the maintenance of the competence: participatory / fast tools, systematic / long-term tools and future tools. The participatory/fast tools for the maintenance of competence are easily available in the on call work environment. The results of this study can be utilized in the laboratory on call. Tools for the maintenance of the professional competence for a biomedical laboratory scientist on call are interactive instruments for learning in work. They enable sharing and implementation of the tacit knowledge
