Otsonin käyttö karieksen pysäytyshoidossa

Abstract

Tässä opinnäytetyössä selvitettiin kirjallisuuskatsauksen avulla otsonihoidon vaikutuksia hammaskarieksen pysäytyshoidossa sekä potilaiden asenteita otsonihoitoa kohtaan. Tavoitteena oli luoda tiivis yhteenveto, josta sekä suun terveydenhuollon ammattilaiset että potilaat voivat saada perustietoa otsonihoidosta ja sen tehokkuudesta karieksen pysäytyshoidossa.

Opinnäytetyön teoreettisissa lähtökohdissa tarkastellaan karieksen etiologiaa ja siihen liittyviä käsitteitä sekä esitellään karieksen perinteiset hoitomuodot, joiden rinnalla voidaan käyttää otsonihoitoa. Osiossa esitetään myös karieksen hallinnan kokonaisvaltaista hoitoa ohjaava malli, suuhygienistin rooli hoitoprosessissa sekä perustiedot otsonista ja otsonihoidosta. Opinnäytetyö etenee esittelemällä opinnäytetyön toteuttamisen prosessin ja päättyy otsonihoidosta tehtyjen tutkimusten ja tutkimustulosten esittelyyn sekä analyysiin.

Tutkimusten etsimiseen käytettiin sekä manuaalista hakua että sähköisiä tietokantoja, kuten CINAHL, Medic, Google Scholar, Medline ja Cochrane. Aineistolle asetettiin erilaisia kriteerejä ja niistä valittiin ainoastaan ehdot täyttävät tutkimukset. Tutkimusten hakemisessa käytettiin hakusanoina esim. seuraavia virkkeitä: Ozone therapy in dentistry ja Ozone therapy in treating caries. Kirjallisuuskatsaukseen löytyi 13 hakukriteerit täyttävää tutkimusta, joista kaikki ovat ulkomailla toteutettuja. Analyysimenetelmänä käytettiin luokittelua, jossa tulokset esitetään vastauksina tutkimuskysymyksiin.

Tulosten mukaan otsonihoito on tehokas hoitomenetelmä ja lisäapu karieksen perinteisten pysäytyshoitokeinojen rinnalle. Lähes kaikki katsauksessa mukana olleet tutkimukset osoittavat otsonihoitojen pysäyttävän tai kääntävän hampaan kovakudoksen liukenemisen takaisin ehjäksi kudokseksi. Myös potilaiden asenteet otsonihoitoja kohtaan ovat myönteisiä. Lisätutkimuksia tarvitaan kuitenkin otsonihoitojen tehokkuudesta karieksen pysäyttämisessä pidemmällä aikavälillä. Myös otsonihoitojen kustannustehokkuus perinteisten hoitomuotojen rinnalla vaatii lisäselvityksiä.

The purpose of this thesis was to assess the ozone treatment effectivity to arrest or re-verse the progression of dental caries as well to assess the patients’ attitude to the method. The aim was to produce information for all of the oral health care professionals and patients about the ozone treatment and its effictivity in arresting or reversing the progression of caries. The theoretical frame introduces the etiology of dental caries and the related concepts as well presents the traditional treating methods which can be supported with the ozone treatment. This part also contains the guiding model of arresting and reversing the caries, the role of a dental hygienist in caries management and the basics of ozone and the ozone treatment. The thesis continues with the presentation of the process of the thesis composition and finishes with the presentation of ozone treatment studies, study results and analysis. The thesis was implemented as a literature study. The included studies were looked up manually from electronic databases (CINAHL, Medic, Google Scholar, Medline and Cochrane). The exact criteria were set for all of the searches and only the studies which met the criteria were chosen. The studies were searched with using keywords, e.g. Ozone therapy in dentistry and Ozone therapy in treating caries. All together there were 13 studies which met the criteria; all of these were foreign. The material was analysed using the classification method where all of the results were presented as answers for the research questions. The results of the literature analysis prove that the ozone treatment is effective to arrest or reverse the progression of dental caries and would be a useful method alongside with the traditional caries treatment. Almost all of the analysed studies showed ozone’s ability to arrest the demineralization of tooth enamel and switch the process back to remineralization phase. According to the results the patients have also a high acceptance of this treatment regime using ozone. Still further research about long-term effects and cost-effectiveness of ozone treatment is needed.
