Säde- ja solunsalpaajahoidon aiheuttamat suuhun kohdistuvat haittavaikutukset sekä niiden ehkäisy- ja hoitomenetelmät : opas hoitohenkilökunnalle

Abstract

Tilastojen mukaan syöpäsairaiden määrä on kasvussa. Samaan aikaan syövästä selviytyneiden suhteellinen osuus on luonnollisesti kasvussa. Tänä päivänä syöpää voidaan hoitaa eri menetelmin. Pään ja kaulan alueelle annettu sädehoito, erityisesti suun, leukojen tai nielun alueelle kohdistettu sekä systeemisesti annettava solunsalpaajahoito voivat muiden haittavaikutuksien ohella aiheuttaa suuhun kivuliaita haittavaikutuksia.

Tämä toiminnallinen opinnäytetyö toteutettiin Metropolia Ammattikorkeakoulun suun terveyden koulutusohjelman opinnäytetyönä. Työn tarkoituksena on tuottaa opas hoitohenkilökunnalle. Opas sisältää tiedon säde- ja solunsalpaajahoidon aiheuttamista suuhun kohdistuvista haittavaikutuksista sekä niiden ehkäisy- ja hoitomenetelmistä. Opinnäytetyön tavoitteena on kasvattaa hoitohenkilökunnan tietämystä säde- ja solunsalpaajahoidon aiheuttamista haittavaikutuksista, joita voi esiintyä suussa. Heidän välityksellä pyritään vaikuttamaan myös syöpäpotilaiden tietämykseen suun terveyden ylläpidosta raskaiden hoitojen aikana.

Toiminnalliseen opinnäytetyöhön kerättiin materiaalia hoitoalan tietokannoista, kuten Medic, PubMed ja EbscoHost. Tietokannoista kerättyjen tutkimusten ja artikkeleiden lisäksi työssä on käytetty tietoperustana ajankohtaisia Käypä hoito -suosituksia, hammaslääketieteen perusteosta Therapia Odontologicaa ja lääketieteen Syöpätaudit -käsikirjaa. Tietoa käsitellystä aiheesta on kerätty myös Duodecimin ylläpitämästä portaalipalvelusta, Terveysportista, joka on suunnattu terveydenhuoltohenkilöstölle. Tärkeänä osana työn tietoperustaa olivat myös eri asiantuntijainformaatikoiden kanssa käydyt keskustelut.

Opinnäytetyö koostuu kahdesta pääkategoriasta. Ensimmäinen kategoria kertoo haittavaikutuksista suussa. Millaisia haittavaikutuksia on olemassa ja miten ne ilmenevät suussa sekä millaisia oireita ne aiheuttavat. Toinen kategoria taas kertoo haittavaikutuksien ehkäisy sekä hoitomenetelmistä ja käy läpi omahoidon oikeaoppisen toteuttamisen.

Opinnäytteen tuotoksena on opas, jota voi hyödyntää päivittäin tapaamisissa syöpäpotilaiden kanssa. Hoitoalan sekä suun terveydenhuollon opiskelijat voivat hyödyntää opinnäytetyötä kokonaisuudessaan tiedonlähteenä. Opas sopii myös tiedonlähteeksi itse syöpäpotilaille.

According to statistics, the number of cancer patients is increasing. At the same time, the proportion of survivors is increasing alongside. Nowadays cancer can be treated with several different methods. Radiotherapy to the head and neck area, especially to oral, jaw or pharynx area, and systemic cytostatic treatment can affect oral health negatively and cause painful side effects.

This was conducted as a thesis for Metropolia University of Applied Sciences. The purpose of this thesis is to produce a guidebook for health care personnel. The guidebook provides a summary of information on the side effects of radiotherapy and cytostatic treatments in oral cavity. It also includes information in preventing and treating these side effects. The aim of the thesis is to increase knowledge of health care personnel on the side effects of radiotherapy and cytostatic treatments in oral cavity. Increasing the knowledge of these side effects allows for the personnel to prevent and treat the side effects in oral cavity.

The thesis was carried out as a functional work. Material for the thesis was collected from nursing science databases. Used databases were Medic, PubMed and EbscoHost. Recommendations of Käypä hoito, Therapia Odontologica and medical Syöpätaudit -manual were also used in the thesis. Important source of knowledge for the thesis were conversations with professional informatics. Language choices for the thesis were English and Finnish. Material was accepted if it was from 21st century until present.

There are two main categories in the thesis. The first encompasses the side effects of radiotherapy and cytostatic treatment on oral cavity; which types of side effects there are, how they emerge in the oral cavity and the kind of symptoms they cause. The second category is on preventing and treating the side effects and how self-care in oral cavity is properly done.

The result of the thesis is the guidebook, which can be used daily as part of meeting with cancer patients. Nursing and dental hygiene students could also take advantage of the guidebook and thesis. Additionally, the guidebook could be handed to cancer patients themselves as a source of information on side effects in oral cavity. Information in the guidebook has been gathered from the thesis and it includes pictures of self-care.
