Kun toivotusta tulikin menetys : Naisten kokemuksia raskaudenkeskeytyksestä sikiöindikaation perusteella

Abstract

Raskaudenkeskeytyslain myötä puutteellisissa olosuhteissa tehdyt raskaudenkeskeytykset vähenivät. Muihin Pohjoismaihin verrattuna Suomessa on tehty vähemmän raskaudenkeskeytyksiä 1980-luvulta lähtien. Sikiöindikaation eli sikiön poikkeavuushäiriön perusteella tehtyjä keskeytyksiä on 500 kaikista noin 10 000 tehdyistä keskeytyksistä.

Tutkimuksen tarkoituksena on kuvata naisten kokemuksia raskaudenkeskeytyksestä sikiöindikaation perusteella. Tutkimuksen tavoitteena on saada raskaudenkeskeytyksen läpikäyneiden naisten kokemuksia ja tiedon avulla kehittää raskaudenkeskeytysasiakkaiden hoidon laatua. Tutkimuskysymyksiä ovat: mitä tunteita raskaudenkeskeytysprosessi herätti naisissa ja miten naiset kokivat hoitohenkilökunnan tuen, kun he olivat tehneet raskaudenkeskeytyksen sikiöindikaation perusteella?

Tutkimus on toteutettu kvalitatiivista eli laadullista tutkimusmenetelmää käyttäen. Aineistonkeruumenetelmänä oli avoin haastattelu ja aineisto kerättiin Internetissä keskustelupalstoilla. Tutkimusaineisto analysoitiin induktiivisella sisällönanalyysilla. Tutkimukseen osallistui kolme naista.

Tutkimustuloksista selvisi, että naiset arvostivat ymmärrystä hoitohenkilökunnalta. Hoitohenkilökunnan empaattinen suhtautuminen koko raskaudenkeskeytysprosessiin vaikutti hyvään hoitokokemukseen. Raskaudenkeskeytyspäätös aiheutti ristiriitaisia tunteita ja päätöksenteko oli haastavaa. Lisäksi naisten tunteet heittelivät laidasta laitaan keskeytysprosessin aikana. Psyykkisen hyvinvoinnin tukeminen korostuu, kun raskaudenkeskeytys tehdään sikiöindikaation perusteella.

The introduction of the Act on Termination of Pregnancy reduced the number of terminations performed under inadequate circumstances. Since the 1980s, fewer terminations have been performed in Finland than in other Nordic countries. Terminations based on fetal indication, or fetal anomalies, comprise 500 out of the approximately 10,000 terminations performed in total.

The purpose of this study is to describe women’s experiences of termination of pregnancy based on fetal indication. The aim of the study is to gather experiences from women who have undergone a termination of pregnancy, and to use the information gathered to improve the quality of care in cases of termination. Research questions include: what emotions did the termination process awaken in the women, and how did the women feel about support from nursing personnel after a termination of pregnancy based on fetal indication?

The study was conducted using a qualitative study method. The data were gathered from online discussion forums using an open-ended interview. The study data were analyzed using inductive content analysis. The study sample consisted of three women.

Findings of the study showed that women valued understanding from the staff members caring for them. The empathetic attitude of the treating staff towards the entire termination process contributed to a positive experience of the care received. The decision to terminate the pregnancy gave rise to conflicting emotions, and making the decision was challenging. In addition, the women’s emotions went from one extreme to another during the termination process. When a termination is performed based on fetal indication, supporting the client’s emotional well-being takes centre stage.
