Sähköinen hoitotahto terveydenhuollossa

Abstract

Opinnäytetyössä tarkasteltiin sähköistä hoitotahtoa terveydenhoitoalalla. Menetelmänä käytettiin kuvailevaa kirjallisuuskatsausta. Tutkimuskysymyksissä selvitettiin, millaisia sähköisiä hoitotahtomalleja Yhdysvalloissa on tarjolla sekä miten sähköinen hoitotahto näkyy terveydenhuollossa Yhdysvalloissa.

Hoitotahto on kirjallinen tai suullinen tahdonilmaus, jolla henkilö sallii tai kieltäytyy hoidosta. Sähköinen hoitotahto on muussa kuin paperisessa muodossa. Opinnäytetyön tavoitteena oli antaa tietoa sähköisistä hoitotahtomalleista terveydenhuollossa työskenteleville tiivistetyssä muodossa. Aineistona käytettiin kuutta englanninkielistä artikkelia.

Hoitotahtomalleja on Yhdysvalloissa useita erilaisia ja ne ovat sisällöltään ja muodoltaan vaihtelevia. Suuri osa on tietokoneohjelmia tai Internet-sivuja, jotka antavat samalla tietoa sairauksista ja tilanteista, joissa hoitotahdolla voi olla merkitystä. Sähköisiin hoitotahtomalleihin liittyy myös ongelmia. Hoitotahdot näkyvät terveydenhuollossa päätöksenteossa, potilaan ohjauksessa, hoitotoiveiden dokumentoinnissa sekä hoitotahtojen saatavuudessa.

Sähköisen hoitotahdon onnistumisvaatimuksena voidaan pitää onnistunutta sisältöä ja muotoa sekä hoitotahdon saatavuutta tarpeen vaatiessa.

The subject of the thesis was electronic advance care planning in health care. The method of the thesis was a narrative literature review. The objective of this thesis was to find out what kind of electronic advance care planning there is in the United States and how electronic advance care planning appears in the health care of the United States of America. Advance care planning is a process that leads to completing a living will or an advance directive for the future care purposes. The purpose of this thesis was to afford information on advance directives and advance care planning for the personnel working in health care.

The research material consisted of six articles published in the English language and containing information about advance care planning in the United States. The articles were then analyzed using a content analysis.

The results showed that most of the electronic advance care planning tools are either Internet pages or computer programs. They also offer information about illnesses and situations where advance directives can be used. Advance care planning appears in decision making, documentation of the patient’s wishes, guidance of the patient and the availability of the advance directives or advance care planning. The contents, format and the availability of advance care planning can be considered as the key factors in creating a successful electronic advance care planning model.
