Palvelutarpeen tunnistaminen ensihoidossa : toimintakyvyn arviointityökalu ensihoidon päätöksenteon tueksi

Abstract

Palvelutarpeen arviointi tehdään sosiaali- ja terveydenhuollon palveluiden ja tukitoimien saamiseksi. Palvelutarpeen arvioinnin tekee sosiaalialan ammattihenkilö, mutta palvelutarpeen tunnistaminen kuuluu jokaisen sosiaali- ja terveysalan ammattihenkilön velvollisuuksiin. Ohjaamalla sosiaalihuollon palveluista hyötyvät henkilöt oikean avun piiriin pystytään ehkäisemään terveyspalveluiden kuormittumista, katkaisemaan jo mahdollisesti alkaneita terveyspalveluiden kuormituskierteitä sekä tukemaan vanhenevan väestön kotona asumista.

Ensihoitajilla on mahdollisuus havaita potilaiden kotona selviytymiseen liittyviä huolenaiheita, jotka muuten saattaisivat jäädä huomiotta. Varhaisella ensihoidon toimesta tehdyllä palvelutarpeen tunnistamisella voidaan lisätä ajoissa tehtyjä yhteydenottoja sosiaalitoimeen ja ennaltaehkäistä potilaan selviytymiskykyä vaarantavien ongelmien kasaantumista.

Toimintakyvyn arviointi on keskeinen osa palveluntarpeen arviointiprosessia. Palvelutarpeen arvioinnissa tarkastellaan asiakkaan arjesta selviytymistä, johon vaikuttavat toimintakyvyn fyysiset, psyykkiset, kognitiiviset ja sosiaaliset ominaisuudet sekä niiden suhde ympäristöön.

Opinnäytetyö oli osa kehitteillä olevaa MyEHPAKE-hanketta ja työn tilaajana toimi Varsinais-Suomen sairaanhoitopiirin ensihoidon vastuulääkäri Timo Iirola. Opinnäytetyössä tuotettiin Varsinais-Suomen sairaanhoitopiirille lomake, jonka avulla ensihoito pystyy tunnistamaan potilaan palvelutarpeen arvioimalla selviytymistä, toimintakykyä sekä ympäristön riskitekijöitä. Lomake ohjaa tarvittaessa ensihoitajaa tekemään ilmoituksen sosiaalihuoltoon.

Opinnäytetyö rakentui kirjallisuuskatsauksesta ja tuotoksena syntyneestä lomakkeesta. Lomakkeen on tarkoitus tulla käyttöön Varsinais-Suomen alueella, mutta sitä voidaan hyödyntää myös valtakunnallisesti.

Service needs evaluation is a requirement for a person to receive social and healthcare's supportive services. The service needs evaluation is made by a social worker, but recognizing the needs for such services is part of the duties of every social- and healthcare worker. Prevention of overcrowding in healthcare services is possible if the patients who would benefit from social services are directed within the scope of the right assistance. This also supports the elderly to live in their own homes longer and possibly can interrupt a person's habit of constantly using healthcare services.

Paramedics have an opportunity to detect issues with patients' ability to live at home that would otherwise go undetected. An early recognition of service needs by the paramedics can increase contacts to social services made early and thus prevent issues that might jeopardize the patients' survivability.

Evaluating the ability to function is a central part of evaluating a persons' need for services. Service needs evaluation takes into consideration the patients' survivability in day to day life. Survivability is influenced by physical, psychological, and social attributes and their relation to the environment.

This bachelors' thesis is part of an under-development project called MyEHPAKE. This thesis is ordered by the Hospital District of Southwest Finland's chief medical officer of emergency medical services, Timo Iirola. A form, with which emergency medical services can recognize the patients' service need by evaluating survivability at home, ability to function and the environmental risk factors, was produced as part of this thesis for the Hospital District of Southwest Finland. The form instructs the paramedic using it to contact social services when necessary.

This bachelors' thesis is made up of literature review and the produced form. The form is meant to be used specifically in Southwest Finland, but it can also be used nation-wide.
