Mielenterveyspotilaan hoitopolku yhteispäivystyksessä

Abstract

Tämän opinnäytetyön tarkoituksena oli selvittää mielenterveyspotilaan hoitopolkua HUS Lohjan sairaalan yhteispäivystyksessä, sen toimivuutta, yhteistyötä sekä mahdollisia kehittämiskohteita hoitohenkilökunnan näkökulmasta. Työn tavoitteena oli saada esille kyselylomakkeen avulla käytännön ratkaisuja parantamaan mielenterveyspotilaan sujuvampaa hoitoon pääsyä ja oikean hoitopolun löytämistä.

Työn teoreettisessa viitekehyksessä tarkasteltiin mielenterveyspotilaan hoitotyötä, mielenterveyshäiriöitä, päihteitä, yhteispäivystystä ja lainsäädäntöä. Opinnäytetyön teoriaosuudessa käsiteltiin yhteispäivystyksen nykytilannetta ja mielenterveyspotilaan hoitopolkua.

Opinnäytetyö toteutettiin laadullisena tutkimuksena, jossa aineistonkeruumenetelmänä toimi kyselylomake. Kyselyyn vastasi kaksikymmentäkaksi yhteispäivystyksen hoitohenkilökunnan jäsentä. Kyselyn numeraaliset taustatiedot esitettiin prosenttilukuina ja henkilökunnalle kohdistetut kyselylomakkeen vastaukset analysoitiin induktiivisen sisällönanalyysin avulla.

Opinnäytetyön tulosten mukaan psykiatrisen sairaanhoitajan työ koetaan hyväksi arkiaamuisin. Tulosten perusteella koettiin potilaan hoidon toimivan virka-aikana, mutta virka-ajan ulkopuolella potilaan hoito on epävarmaa. Matalan kynnyksen paikkojen tarpeellisuus koettiin tärkeänä. Esille tuli myös, että yhteispäivystyksessä hoidetaan ensisijaisesti somaattisia sairauksia.

Kehittämiskohteita esiintyi kyselyn vastauksista useita. Vastauksista nousi esille psykiatrisen potilaan hoitoon pääsy. Lisäksi potilaan mahdollinen jatkohoito-ohjaus koettiin olevan hankalaa. Psykiatrisen sairaanhoitajan ja psykiatrin ympärivuorokautista asiantuntemusta kaivattiin. Tällöin potilaan olisi mahdollista päästä nopeaan hoitokontaktiin ja jatkohoitopaikkaan. Lisäksi kaivattiin yhtenäistä toimintamallia.

The purpose of this thesis was to examine the course of treatment of mental patients in joint emergency services of HUS Lohja Hospital, namely, its functionality, cooperation, as well as possible development targets from the point of view of the care staff. The objective of this study was to determine, with the help of a questionnaire, practical solutions for smoother access to care and finding the appropriate course of treatment for mental patients.

The conceptual framework of this thesis examined the care of mental patients, mental disorders, recreational drugs, joint emergency services and legislation. The theoretical part of this thesis addressed the current situation of joint emergency services and the course of treatment of a mental patient.

The thesis was carried out as a qualitative study with a questionnaire as the data collection method. The questionnaire was answered by twenty-two joint emergency services care staff members. The numeric background data was expressed as percentages and the care staff's answers were subjected to an inductive content analysis.

The results of this thesis suggest that the work of psychiatric nurses is considered good on working day mornings. They indicate that patient care functions well during office hours, but is uncertain outside office hours. The need for low-threshold care places was considered important. The answers also revealed that joint emergency services treated somatic illnesses with priority.

The care staff indicated several development targets. A recurring topic in the answers was the access to care for a psychiatric patient. In addition, potential follow up-care of the patient was considered problematic. A lack of round-the clock psychiatric nurse and psychiatrist expertise was also indicated. If it were available, patients would have quick access to care contact and follow-up care. Lastly, the answers brought up the need for a common approach to care.
