MET- ja elvytyslomakkeiden päivitys Kymenlaakson keskussairaalalle

Abstract

Hoitotyön kirjaaminen on laissa säädeltyä toimintaa. MET- ja elvytystilanteet ovat kriit-tisiä tilanteita, jotka vaativat nopeaa reagoimista ja toimimista. Kirjaaminen näissä tilan-teissa voi olla hyvinkin haastavaa. Mitä kriittisempi ja vaativampi tilanne on kyseessä, sitä tärkeämmäksi tarkka dokumentointi muodostuu.

Tämä opinnäytetyö on tehty yhteistyössä Kymenlaakson keskussairaalan kanssa. Opinnäytetyön tarkoituksena oli kartoittaa MET- ja elvytyslomakkeiden nykytila Kymen-laakson keskussairaalan elvytysryhmälle ja päivittää MET- ja elvytyslomakkeet henkilökunnan mielipiteet, kehitysideat sekä toiveet huomioiden tuoreimpaan tutkittuun tietoon pohjautuen. Tarkoituksena oli tehdä lomake mahdollisimman helppokäyttöiseksi ja selkeälukuiseksi sekä käytännöntilanteisiin sopivaksi.

Tämä opinnäytetyö on kehittämistyö ja tavoitteisiin pääsemiseen on käytetty eri tutkimusinterventioita. Ryhmähaastattelua on käytetty lähtötilanteen kartoittamiseksi sekä henkilökunnan toiveiden ja mielipiteiden selvittämiseksi. Lomakkeiden kehittäminen pohjautuu henkilökunnan mielipiteiden lisäksi tuoreimpaan tutkittuun tietoon perustuvaan kirjallisuuskatsaukseen.

Aikaisemmin elvytysryhmällä oli käytössä kaksi erillistä lomaketta MET- ja elvytyskäyntien kirjaamiseen. Nyt aiemmin käytössä olleiden kahden lomakkeen sijaan, on tehty yksi lomake, johon kirjataan MET- sekä elvytyskäynnit. Lomake tulee koekäyttöön Kymenlaakson keskussairaalan teho- ja sydänvalvontaosaston ylläpitämälle elvytysryhmälle.

Lopputuloksena oli yhtenäinen MET- ja elvytyslomake, johon kirjataan molemmat käynnit. Uuteen lomakkeeseen sisällytettiin MET- ja elvytystilanteissa vaadittavat tärkeimmät asiat ja jätettiin pois turhia elementtejä vanhoista lomakkeista. Lomakkeet pyrittiin koostamaan johdonmukaisiksi käytettävissä oleva tila huomioiden. Lomakkeet tulevat koekäyttöön Kymenlaakson keskussairaalaan, jossa sitä tarvittaessa muokataan edelleen käyttöön sopivammaksi.

Documentation is law regulated function. The situations that require MET-team or resuscitation requires quick action. It might be challenging to document during these situations. Critical situations demand accurate documentation.

The objective of this thesis was to update MET- and resuscitation forms and survey the current state of the forms. The aim was to update forms regarding the opinions, hopes and the ideas of the staff and base the new form on the latest researched in-formation. Purpose was to make new form user-friendly and easy to use during acute situations. Before the update the resuscitation and medical emergency team used to have two separate forms for documentation. After the update team will have only one form. The form will be used among resuscitation and medical emergency team which consists of the nurses that works in ICU and CCU in Kymenlaakso’s central hospital.

This thesis is development work. There were used several different research-based interventions to achieve the objectives that were set. To survey the baseline of the forms that staff used before. Forms were analyzed and there was group interview for the staff to survey the situation. The Update of the forms is based on the opinions and hopes of the staff as well as it is based on theoretical framework. Theoretical framework was based on the latest reaserched information that was available.

The result was one new form instead of two forms. New resuscitation and MET- form is for documentation in both scenes, in Medical Emergency situations and in situations that requires resuscitation. The new form includes the most important and critical matters that needs to be noticed during these situations and all things that were considered to be unnecessary were left out. The form was strived to consist that way that it is logical to read and the space that there was to use was used advisedly. The forms will be tested among the team and if necessary it can be edited to be more appropriate to use for the staff.
