LONKKALI'USTA JALKEILLE : Asiakkaiden kokemuksia Kotkan kotihoidoin kotiutustiimin toiminnasta

Abstract

Ikäihmisten määrä on nopeasti lisääntymässä. Tavoitteena on, että myös tulevaisuudessa ikäihmiset voisivat asua itsenäisesti omissa kodeissaan mahdollisimman pitkään. Tämä asettaa vaateita sosiaali- ja terveydenhuollon järjestelmille. Lonkkamurtumien riski kasvaa väestön ikääntyessä. Lonkkaliukumäki - toimintamalli on kehitetty tehostamaan kuntoutumista, vähen-tämään laitostumista ja tuomaan kustannussäästöjä. Opinnäytetyön tavoitteena oli selvittää kotkalaisten lonkkamurtumapotilaiden kokemuksia siitä, kuinka Kotkan kotihoidon kotiutustiimin toiminta tuki heidän toimijuuttaan kuntoutuslaitoksesta kotiutumisen jälkeen. Opinnäytetyön tarkoituksena oli kerätä asiakkaiden kokemuksiin perustu-vaa tietoa kotihoidon hyödynnettäväksi palveluita suunniteltaessa ja kehitettäessä. Työssä tuo-daan esille myös asiakkaiden toimijuutta vahvistavia ja heikentäviä tekijöitä. Opinnäytetyö toteutettiin laadullisena tutkimuksena. Aineisto kerättiin teemahaastattelun avulla haastattelemalla Kotkan kotihoidon kotiutustiimin entisiä asiakkaita. Opinnäytetyöhön haasta-teltiin seitsemää ihmistä. Haastattelujen analysoinnissa käytettiin sisällönanalyysia. Tulokset osoittivat asiakkaiden olevan tyytyväisiä saamiinsa Kotkan kotihoidon kotiutustiimin palveluihin. Henkilöstön käytös koettiin ammattimaisena ja ystävällisenä. Kotihoidon palvelut tukivat hyvin fyysisen toimintakyvyn osa-aluetta mutta psyykkisen toimintakyvyn osa-alueen tukemisessa oli puutteita. Tulosten pohjalta haittapuolena nousi esille vähäinen mielen hyvin-voinnin tukeminen lonkkamurtuman jälkeen sekä toimijuuden merkityksen vähäinen huomiointi. Tulosten perusteella johtopäätöksenä on, että kotihoidon on tarpeen kehittää ikäihmisten mie-len hyvinvoinnin tukemista lonkkamurtuman jälkeen. Jatkotutkimuksena voitaisiin selvittää läheisten näkemyksiä kotihoidon palveluista ja tutkia, mitä kehittämisehdotuksia heidän näkökulmastaan tulisi esille. Lisäksi voitaisiin tutkia kotihoidon asiakkaiden toiveita psyykkisen hyvinvoinnin tukemiseen sekä kotihoidon työntekijöiden näke-myksiä siitä, miten asiakkaiden mielen hyvinvoinnin tarpeisiin voitaisiin vastata.

In western societies, the number of older generations is vastly increasing due to high living standards and the technological development of medical care. The goal has been set to sup-port the elderly people's home living as long as possible. This leads to the problem that a grow-ing number of the elderly will need support of the social and health services provided by the state. The purpose of this thesis was to investigate opinions of elderly home care clients regarding the home care team services provided for the patients after hip fracture in Kotka. The aim was to find out what factors in care may promote or inhibit patients agency after hip fracture. Agen-cy refers to activities and is determined according to who defines it and in what context. Finally, the aim of this thesis was to gather reliable information that could be used in the process of planning, implementing and developing home care services in Kotka. The method of this study was a qualitative approach. The material for this study was collected by interviewing seven home care team clients. The interviews were executed by using the fo-cused interview method and the material was analyzed by using inductive content analysis. The results show that the clients of the home care team unit of Kotka are generally content with the services they have received. The staff in home care team was seen professional and friendly by the clients. However, according to the results, the main drawback was lack of psy-chological support after the hip fracture. Also, the agency should be taken into account much more actively. The home care team provides good services as per the physical aspect of the care, but the support in mental care is lacking behind. The results of this study lead to the con-clusion that there is need for development of elderly people's mental health care in the home care unit after hip fracture. The suggestion for future research includes opinions of the patient's relatives regarding the home care services.
