TIEDOLLINEN TUKI GYNEKOLOGISEN SYÖPÄPOTILAAN SOLUNSALPAAJAHOITOJEN ALOITUKSESSA : Kyselylomake naistentautien poliklinikalle

Abstract

Opinnäytetyön tavoitteena oli tuottaa kirjallisuuskatsauksen perusteella kyselylomake Kanta-Hämeen keskussairaalan naistentautien poliklinikan yhteydessä toimivan syöpäpoliklinikan Check-list -projektille. Projektilla toteutetaan syöpäpoliklinikan kehittämistyötä, jonka tarkoituksena on parantaa ja yhtenäistää gynekologisen syöpään sairastuneiden potilaiden ohjausta ennen ensimmäisen solunsalpaajahoidon aloitusta. Lisäksi opinnäytetyön tavoitteena oli kehittää opetus- ja ohjausosaamista syöpätautien hoitotyössä, sekä vahvistaa ammatillista tutkimus- ja kehittämisosaamista.

Opinnäytetyön teoreettinen viitekehys koostui keskeisistä tiedollisen tuen teemoista gynekologista syöpää sairastavalle potilaalle annettavassa ohjauksessa rajautuen vaiheeseen ennen ensimmäisen solunsalpaajahoidon aloitusta. Näitä olivat sairauteen ja hoitojen toteuttamiseen liittyvä tieto, ammatillisen ja käytännöllisen tuen saamiseen liittyvä tieto, solunsalpaajahoitojen sivuvaikutuksiin liittyvä tieto sekä sairauden vaikutukset potilaan seksuaalisuuteen. Teoreettisessa viitekehyksessä käsiteltiin myös psyykkisen tuen merkitystä sairastuneen tukemisessa sekä tiivistetysti sairastumisen kriisiä, mikä heijastuu merkittävästi potilasohjaukseen.

Toiminnallisen opinnäytetyön tuotoksena syntyi kyselylomake, joka annettiin syöpäpoliklinikan Check-list -projektin käyttöön. Kyselylomake laadittiin yhteistyössä syöpäpoliklinikan vastuusairaanhoitajan kanssa. Lomakkeen tekemiseen käytettiin Helena Leino-Kilven (1994) HYVÄ HOITO -mittaria, joista valittiin kysymyksiä lomakkeeseen siten, että valmis lomake soveltuisi Check-list -projektin tarkoitukseen. Check-list -projekti toteuttaa kyselyn syksyllä 2017.

The aim of this thesis was to produce a questionnaire based on a literature review. The idea was to afford the completed questionnaire to the outpatient clinic for gynecological cancer which is a part of the outpatient clinic of gynecological diseases at Kanta-Häme central hospital. The outpatient clinic is working on a development project called The Checklist. The purpose of their project is to ameliorate and standardize the counselling of the patients with gynecological cancer before their first cytostatic treatment. The aims of the thesis were also to improve the competence of guidance and counselling in the nursing of cancer patients and fortify the professional competence of research and development.

The theoretical part of the thesis was composed of the essential themes of the support of knowledge in counselling patients with gynecological cancer with relation to the counselling before starting the first cytostatic treatment. The four themes were the knowledge of disease and the implementation of treatment, the knowledge of having professional and practical support, the knowledge of the side effects of treatment and the knowledge of the sexual effects of disease. The theoretical part of the thesis was concerned with the meaning of emotional support in supporting a patient. Also, the crisis of having the diagnosis of cancer has a significant effect on counselling, in other words the crisis was discussed briefly in the theory section.

The product of the practice based thesis was the questionnaire which was given to the Checklist -project. The questionnaire was prepared in co-operation with an authorized nurse of the outpatient clinic of gynecological cancer. The questionnaire was compiled by using the scale of HYVÄ HOITO of Helena Leino-Kilpi (1994). The questions were selected from HYVÄ HOITO -scale so the qualified questionnaire would be appropriate to Check -list-project. The survey is going to be carried out in the fall 2017.
