Ohjeistus tyypin 2 diabeteksen seurantakäynnin toteutumisen tueksi

Abstract

Diabetes on yksi kansantaudeistamme, jota sairastaa arviolta yli 500 000 suomalaista. Tässä opinnäytetyössä käsiteltävä tyypin 2 diabetes on yleisin kaikista diabetesmuodoista. Diabeteksen hoidossa korostuu sairastuneen voimavarat ja kyky toteuttaa omahoitoa, minkä vuoksi hoidonohjauksella ja tuen järjestämisellä on suuri merkitys hoitotasapainoon.

Opinnäytetyön tarkoituksena oli kehittää Siilinjärven terveysaseman omahoitajan vastaanotolla tapahtuvan tyypin 2 diabetesta sairastavan seurantakäynntiä. Tavoitteena oli laatia omahoitajan tueksi ohjeistus seurantakäynnin toteuttamiseksi sekä kirjaamiseksi. Työssä nostettiin esiin rakenteellisen kirjaamisen pohjalta laadittavan hoitosuunnitelman etua osana laadukasta hoidonohjausta.

Laadittiin teoriatietoon pohjautuva ohjeistus, jossa käydään vaiheittain läpi seurantakäynnillä huomioon otettavat sekä hoitosuunnitelmaan kirjattavat asiat. Onnistunut seurantakäynti mahdollistaa tyypin 2 diabetesta sairastavien mahdollisimman yhtenäisen, suunnitelmallisen ja kokonaisvaltaisen hoidon.

In Finland diabetes is a relatively common disease, with over 500 000 people being diagnosed with it, thus making it one of the great public health problems. This thesis focuses on the treatment of type 2 diabetes, which is the most common form of diabetes. The resources and ability to carry out the self-care plan are important in the treatment of type 2 diabetes. Because of this referral to treatment and support system have a great importance in the therapeutic equilibrium of those affected with the disease.

The purpose of this thesis was to develop the content and procedure of the diabetic’s follow-up visit with the nurse at the Siilinjärvi health care center. The aim was to create guidelines for the nurse’s follow-up meeting. The method of structural documentation was used. Structural documentation is a good tool when planning a care plan as part of high quality patient education.

The guidelines were developed based on theoretical knowledge. These guidelines give step by step instructions for the nurses on what to take into consideration and what to record to the care plan at the follow-up visit. The benefit of a successful follow-up visit is to have well planned, uniform and holistic treatment for the sufferers of type 2 diabetes.
