Laadukas lonkkamurtumapotilaan ohjaus : potilaiden näkökulma

Abstract

Tämän opinnäytetyön tarkoituksena oli kuvata, mitä laadukas lonkkamurtumapotilaan ohjaus on potilaiden käsityksien mukaan Satakunnan sairaanhoitopiirin alueella. Tutkimuksen tarkoituksena oli selvittää, miten laadukasta lonkkamurtumapotilaan ohjausta voidaan kuvata, mitkä tekijät vaikuttavat lonkkamurtumapotilaan ohjauksen onnistumiseen, mikä on lonkkamurtumapotilaan ohjauksen tavoitteena, mikä on potilaan ja omaisen rooli laadukkaassa lonkkamurtumapotilaan ohjauksessa ja miten lonkkamurtumapotilaan ja hoitohenkilökunnan ohjaussuhdetta voidaan kuvata potilaan näkökulmasta.

Tutkimuksessa käytettiin strukturoitua kyselylomaketta, joka on kehitetty Satakunnan ammattikorkeakoulun ja Satakunnan sairaanhoitopiirin yhteisessä tutkimusprojektissa. Aineisto kerättiin potilailta strukturoidulla haastattelulla käyttäen tätä kyselylomaketta, joka sisälsi myös avoimia kysymyksiä. Tutkimukseen haastateltiin 30 päivystyksenä leikattua lonkkamurtumapotilasta. Aineisto kerättiin vuoden 2008 maaliskuun ja marraskuun välisenä aikana Satakunnan sairaanhoitopiirin erikoissairaanhoidon ja perusterveydenhuollon vuodeosastoilla. Tutkimusaineisto analysoitiin tilastollisesti.

Tutkimustulokset osoittivat, että laadukasta lonkkamurtumapotilaan ohjausta voidaan kuvata kaikkien vastaajien mukaan sellaiseksi, että siinä keskitytään potilaan hoidon kannalta keskeisiin asioihin ja potilaalla on mahdollisuus kysyä haluamistaan asioista ohjauksen aikana. Vastaajien mukaan laadukasta lonkkamurtumapotilaan ohjausta antava ohjaaja hallitsee ohjattavan asian. Lonkkamurtumapotilaan ohjauksen onnistumiseen vastaajat ilmoittivat vaikuttavan ohjaajan vuorovaikutustaitojen, ammattitaidon ja persoonallisuuden. Lonkkamurtumapotilaan ohjauksen tavoitteena on potilaiden käsityksen mukaan tukea potilaan omatoimisuuden lisääntymistä ja itsenäistä selviytymistä. Potilaan ja omaisen roolia laadukkaassa lonkkamurtumapotilaan ohjauksessa potilaat kuvasivat vastauksissaan siten, että potilaalla ja potilaan omaisilla on oikeus saada ohjausta. Lonkkamurtumapotilaan ja hoitohenkilökunnan ohjaussuhteeseen kuuluu potilaiden mukaan molemminpuolinen luottamus sekä potilaan ihmisarvon ja vakaumuksen kunnioittaminen.

Tämä opinnäytetyö on osa laajempaa tutkimusprojektia. Opinnäytetyön tuloksia voidaan hyödyntää lonkkamurtumapotilaan ohjauksen kehittämisessä Satakunnan sairaanhoitopiirin alueella.

HIGH-QUALITY GUIDANCE FOR PATIENTS WITH HIP FRACTURE - PATIENTS' VIEW

The purpose of this thesis was to determine what patients saw as high-quality guidance for hip fracture patients in the Satakunta Hospital District. The purpose of the research was to determine how high-quality guidance for hip fracture patients could be described; which factors affect the success of the guidance; what is the goal of the guidance; what are the roles of the patient and their family members in the guidance of hip fracture patients; and how the relationship between the hip fracture patient and the nursing staff could be described from the patient's point of view.

A structured questionnaire form was used in the study. The form was developed for Satakunta University of Applied Sciences and Satakunta Hospital District's joint research project. The data was collected from the patients via structured interviews, with the help of this questionnaire form which also contained open questions. 30 hip fracture patients were interviewed for the study; all of them had been operated on in the emergency room. The data was collected between March and November in 2008 in the special health care and basic health care units of the Satakunta Hospital District. The research data was analyzed statistically.

The results of the study show that all interviewees felt that high-quality guidance for hip fracture patients focuses on matters that are essential to the patient's treatment, and that the patient has the opportunity to ask questions during guidance. The patients felt that the instructor in charge of the guidance for patients with hip fracture must have expertise in the matter they guide the patients on. The patients thought that the instructor's interaction skills, professionalism and personality affected the success of the guidance. According to the patients, the goal of the guidance is to support the patients' initiative and independence. As for the role of the patient and their family members in the high-quality guidance for hip fracture patients, the patients felt they and their families had the right to receive guidance. The patients also felt that mutual trust and respect for the patient's human dignity and convictions was part of the relationship between the hip fracture patient and the nursing staff.

The results of this study can be exploited in developing the guidance of hip fracture patients in the Satakunta Hospital District, as well as a part of the larger entity that is the research project between the Satakunta Hospital District and the Satakunta Univerisity of Applied Sciences. This thesis is included in the project.
