eNNI-hankkeen Lahden ensimmäinen osaprojekti : kirjaamisen kehittämishanke Lahden kaupunginsairaalan osastolla L51 ja Lahden kotihoidon Launeen tiimissä 1

Abstract

Tämä opinnäytetyö on eNNI-hankkeen Lahden ensimmäisen osaprojektin kuvaus. Valtakunnallinen eNNI-hanke on luotu juurruttamaan muuttuvia kansallisia kirjaamisvaatimuksia työyhteisöihin. Hanke toteutetaan Learning by developing – oppimismallin mukaisesti. Lahden osaprojektia ohjaa Lahden ammattikorkeakoulu, jonka yhteistyötahoina toimivat Lahden kaupunginsairaalan neurologinen kun-toutusosasto L51 ja Lahden kotihoidon Launeen tiimi 1.

Lahden osaprojektin tarkoituksena on rakenteisen kirjaamisen kansallisen mallin selkeyttäminen ja osaamisen syventäminen hankkeen toimijoille. Tavoitteena on, että työyhteisöön muodostuu toimiva malli eli uusi toimintatapa kirjaamiseen. Lahden osaprojekti toteutui seuraavien juurruttamisprosessin vaiheiden mukaisesti: työyhteisön kehittämistyöryhmän muodostaminen, työyhteisön nykytilanteen kuvaus, muutoksen merkityksen muodostuminen, uuden teoreettisen ja tutkitun tiedon hankkiminen, kehittämiskohteiden tunnistaminen ja valitseminen, uuden toimintatavan muodostuminen ja muokkautuminen, kehittämistoiminnan arviointi, kehittämistoiminnan raportointi sekä muutoksen juurruttaminen työyhteisössä.

Kehittämiskohteeksi työyhteisöt valitsivat hoitotyön tulotilanteen ja hoidon suunnittelun kuvauksen. Työyhteisöjen toiveesta tehtiin heille kehittämiskohteesta oppaat. Oppaat sisälsivät tutkittua tietoa tiivistetysti, uuden toimintatavan, esimerkki potilaan tulotilanteen kuvauksen ja hoidon suunnitelman. Lisäksi tehtiin oppaaseen työyhteisöissä käytössä olevista toimintakykymittareista taulukon, mikä helpotti hoidon tarpeen määritystä. Työyhteisöt toteuttivat uutta toimintatapaa jo hankkeen aikana. Arvioinnin tulosten mukaan työyhteisön jäsenet uskoivat toimintatavan muuttuvan.

This bachelor's thesis describes the progression of the eNNI-project the first part project in Lahti. The national eNNI-project roots new changing national recording criteria to workplaces. The project was executed by Learning by developing- method. The first part project in Lahti was directed by Lahti University of Applied Sciences. The development work was carried out in Lahti city hospital neurological rehabilitation ward L51 and in Lahti home care Laune's team one.

The purpose of the first part project in Lahti was to clear the structural recording in workplaces and to make the knowhow. The aim of the project was to create working model in nursing documentation. The first part project in Lahti advances according to rooting process: building the development group, describing the present state in workplaces, building up the meaning of the development, searching new and factual information, recognising and choosing the target of the development, modifying the new model in nursing documentation, evaluating the development work, describing the development work and rooting the change in work places.

The workplaces chose as development objectives the documentation of planning and the situation when patients first come to the ward. The nurses wanted us to make a guide book of the changes in documentation. The guide book contains compressed theory, new procedure and examples of the documentation of the nursing planning and new patients. It also contains a table of working order indicator, which helps to define the needs of nursing. The working places utilized the new procedure of documentation already during the project. The results of the evaluation showed that they believed that the procedure would change.
