Ikääntyneen lonkkamurtumapotilaan kotikokemus lonkkaleikkauksen jälkeen

Abstract

Tämän opinnäytetyön tarkoituksena oli kuvata lonkkamurtumapotilaan kotikokemusta sairaalasta kotiutumisen jälkeen. Tavoitteena oli selvittää, mitkä kokemukset olivat tukeneet tai heikentäneet kotona asumista. Opinnäytetyön tavoitteena oli myös selvittää, miten koti ja kotona asumisen merkitys sekä elämänlaatu olivat muuttuneet kotiutumisen jälkeen. Opinnäytetyö oli osa Katriinan sairaalan osasto 2B:n ja Laurea ammattikorkeakoulun kehittämisyhteistyötä, jonka tavoitteena oli tuottaa uutta tietoa, joka tukee lonkkaleikkauspotilaan hoidon kehittämistyötä.

Opinnäytetyössä käytettiin tutkimusmenetelmänä kvalitatiivisiin tutkimusmenetelmiin kuuluvaa haastattelututkimusta. Tutkimusaineisto kerättiin haastattelemalla kolmea lonkkamurtuman kokenutta ikäihmistä elo-syyskuussa 2017. Haastateltavat valittiin ajatuksella, että osastolla oltiin epävarmoja kotiutuisen onnistumisesta. Tutkimusmenetelmäksi valittiin haastattelututkimus, sillä lonkkaleikkauspotilaiden oma ääni haluttiin saada kuuluviin. Haastattelun pääteemat käsittelivät kotikokemusta, elämänlaatua, ympäristöä ja selviytymisen tukemista. Haastatteluaineisto analysoitiin induktiivisella sisällönanalyysillä.

Tulosten mukaan haastateltavien kotikokemukset sairaalasta kotiutumisen jälkeen olivat pääosin myönteisiä. Kodin merkitys oli korostunut. Sopeutumisessa uuteen tilanteeseen auttoivat kodin tuttuus ympäristöineen. Elämänlaatu koettiin pääosin hyväksi. Kotona pärjäämisen apuvälineet, muutostyöt sekä omaiset ja/tai kotihoidon käynnit olivat edellytyksenä kotona asumiselle. Myös haastateltavien omat ominaisuudet sekä elämänkatsomus olivat auttaneet haastavasta tilanteesta selviytymisessä.

Tulosten perusteella kehittämiskohteiksi nousivat kotiutumisen turvallisuus, tiedonsaannin helppous esimerkiksi kirjallisena ”kotiutumiskansio”–muodossa, sekä jälkihoito. Opinnäytetyön tulosten avulla osasto sai uutta tietoa osaston epävarmojen kotiutujien kotikokemuksista. Tuloksia henkilökunta voi käyttää hoidon kehittämiseen tarkoituksenmukaisemmaksi ja potilaslähtöisemmäksi. Yhteiskunnallisesti ajatellen opinnäytetyön tuloksista voisi olla apua palvelujärjestelmien kehittämistyössä.

The purpose of this thesis was to describe the elderly hip arthroplasty patients’ home experience after they were discharged from hospital. The first aim of the thesis was to find out which experiences had supported or weakened the patient’s life at home. The second aim of this thesis was to find out how the significance of their home and the quality of life had changed after discharge. This thesis is part of a development project which is a collaboration project between Vantaa city hospital Katriina ward 2B and Laurea UAS. The aim is also to generate new knowledge to support the development of hip arthroplasty patient nursing.

The approach in this thesis was qualitative. The research data was collected through interviews in August- September 2017. The interviewees were chosen from a group of elderly patients whose discharge was uncertain. The research method was chosen because the aim was to have the arthroplasty patients’ own voice heard. The main themes of the interviews concerned home experience, quality of life, environment and support of coping at home.

Based on the results, the interviewees’ home experiences after discharge were mainly positive. The significance of home was emphasized. The home with its familiar environment helped the patients to adapt to the change. The interviewees experienced the quality of life basically good. The aid tools, changes done in the home environment, help from family members or other close relatives and/or home care were the requirements for coping at home. Also, the personal features and the view of life had helped them to cope with the challenging situation.

Based on the results the things to be developed would be the security of the patients when they returned home, proper information e.g. through “a homecoming file”, and some kind of after-care e.g. a check-up phone call or a visit at the patients’ home. With the help of the results of the thesis, the ward could receive new information about the home experiences of their patients. The personnel can use the results of this thesis to develop their care to become more appropriate and more patient focused. From the point of view of the society, the results of the thesis could help to develop the service structure.
