Laadunarviointilomake verinäytteenotosta hoivakodille

Abstract

Laboratoriotutkimusprosessin preanalyyttiseen vaiheeseen kuuluvat kaikki vaiheet alkaen lääkärin tekemästä laboratoriopyynnöstä näytteen analysointiin asti. Preanalyyttisessä vaiheessa tapahtuvat virheet vaikuttavat laboratoriotulokseen ja voivat vaarantaa potilasturvallisuutta. Verinäytteiden ottaminen on osa preanalytiikkaa ja tärkeä osa potilaan hoitoprosessia. Onnistunut verinäytteenotto on myös perusta laboratoriotutkimustulosten luotettavuudelle.

Verinäytteitä otetaan kliinisissä laboratorioissa, mutta myös laboratorion ulkopuolella, kuten hoivakodeissa. Vastuu laadukkaasta verinäytteenotosta on hoivakodissa hoitajien vastuulla, jotta hoivakodissa asuvat asukkaat saavat luotettavia laboratoriotuloksia. Hoivakodissa näytteenottotilanne poikkeaa laboratoriossa tapahtuvasta näytteenotosta muun muassa siten, että näytteenotto tapahtuu asukkaan kodissa. Lisäksi hoivakodissa olevat asukkaat ovat usein muistisairaita, jolloin asiakasta tunnistettaessa hän ei voi itse kertoa nimeään tai sosiaaliturvatunnustaan, vaan tunnistus on tehtävä eri tavalla.

Opinnäytetyö on toiminnallinen opinnäytetyö, jonka tuotoksena tehtiin laadunarviointilomake verinäytteenottoon. Toimeksiantaja on Siun Soten ylläpitämä Sireenipihan hoivakoti. Toiminnallisen opinnäytetyön tarkoituksena oli tehdä laadunarviointilomake verinäytteenottotilanteen arviointiin. Tavoitteena on edistää laadukasta näytteenottoa ja taata luotettavat laboratoriotulokset. Tavoitteena on myös saada aikaiseksi Sireenipihaan tasalaatuinen näytteenottotapa riippumatta siitä, kuka näytteen ottaa. Samalla tavoitteena on vähentää mahdollisia poikkeamia näytteenotossa. Laadunarviointilomaketta varten tietoa kerättiin näytteenottoon, laadusta ja ikäihmisistä liittyvistä lähteistä. Tiettyjä asioita oli huomioitava lomaketta tehdessä, kuten kuinka näytteenottotilanne toteutuu hoivakodissa. Hoivakodissa ei ole samanlaisia resursseja käytettävissä kuin kliinisissä laboratoriossa.

Lomake tehtiin valmiiksi 2017 alussa, sitä oli testattava alussa ja sitä testattiin muutamia kertoja ennen varsinaista käyttöönottoa. Lomakkeessa on 32 kysymystä, joihin vastataan kyllä- tai ei-vastausvaihtoehdoilla. Lisäksi lomake sisältää neljä avointa kysymystä. Tilaajan toivomuksesta kysymyksissä käytettiin väittämien sijaan kysymysmuotoa. Laadunarviointilomake vastasi tilaajan toivetta. Toimeksiantajan mukaan se oli selkeä, yksinkertainen ja ei vie aikaa täyttäessä. Lomake auttaa tulevaisuudessa kiinnittämään huomiota mahdollisiin poikkeamiin näytteenottotilanteessa.

All the phases, for instance starting from the physician’s request of examination till analysing the sample, belong to laboratory examination process in the preanalytical phase. Faults that happen in the preanalytical phase have an impact on the laboratory result and can endanger patient safety. Taking blood samples is a part of the preanalytical phase and is an imporant part of the patient’s treatment process. Succesful blood sampling is also a base of labora-tory test results’ reliability.

Blood samples are taken in clinical laboratories, but also outside of the laboratory such as in nursing homes. The responsibility for good-quality blood sampling are nurses’ responsibility in nursing homes in order that the residents of nursing homes get reliable laboratory test results. The sampling in nursing home differs from a laboratory’s sampling among other things that sampling takes place in a resident’s home. In addition the residents of the nursing homes sometimes suffer from dementia, which is why the resident can’t themselves tell their name or social security number therefore the identification has to be done differently.

The thesis is a functional thesis from which a quality evaluation form was made. This was made for Sireenipiha’s nursing home maintained by Siun Sote. The purpose of the functional thesis was to make a quality evaluation form for the evaluation of blood sampling. The objective is to enhance quality sampling and ensure good-quality laboratory results. Another objective is to achieve similar blood sampling practices to Sireenipiha regardless of who takes the sample. At the same time the aim is to diminish potential anomalies in sampling. For the quality evaluation form information was gathered from articles related to sampling, quality and the elderly. Certain things had to be taken into account when making the form, for instance, how sampling is done in nursing homes. There aren’t exactly same resources to use as in the clinical laboratories.

The form was completed at the beginning of 2017 and it had to be tested in the beginning and it had a trial run a couple of times before the actual use. The form contains 32 questions, which can be answered with a yes or no option. In addition, the form contains four open-ended questions. Due to the client’s request instead of using declarative sentences the form ended up containing interrogative sentences. The quality evaluation form answered the client’s expectations. According to the client it was distinct, simple and doesn’t take much time to fill out. The form helps to take into account potential anomalies in the sampling in the future.
