LPG-hoidon hyödyntäminen osana akillesjännepotilaan kuntoutusta. : Potilaan kokema toiminnallinen hyöty

Abstract

Opinnäytetyön tarkoituksena oli kerätä uutta tietoa LPG-hoidon hyödynnettävyydestä osana akillesjännepotilaan kuntoutusta. Tavoitteena oli arvioida asiakkaan kokemaa toiminnallista hyötyä suhteessa akillesjännevamman aiheuttamaan toiminnan rajoitteeseen asiakkaan arjessa. Työn tilaaja oli Mikkelin Kuntopalvelu, joka myös mahdollisti hoitokokeilun toteuttamisen heidän yrityksessään keväällä 2017. Opinnäytetyöprosessin aikana toteutettiin hoitokokeilu LPG-hoitolaitteella asiakkaan akillesjännevaivaan. Mittareina hoitokokeilussa käytettiin potilaskohtaista toiminnallista asteikkoa (PTA) ja VAS-kipujanaa. Tutkimusjoukko koostui kolmesta asiakkaasta, joista kaksi oli akillestendiniittipotilaita ja yksi kärsi akillesjännerepeämän jälkeisistä tunnottomuudesta ja voimattomuudesta jalkaterän ja nilkan alueella. Asiakkaat saivat LPG-hoitoa neljän viikon ajan. Hoitomäärät poikkesivat toisistaan riippuen asiakkaiden mahdollisuuksista tulla hoitoon. Hoitokokeilun tulokset osoittivat, että asiakkaiden kokemat toiminnan rajoitteet heidän itse nimeämissään arjen toimissa vähenivät ja kipu VAS-kipujanalla mitattuna väheni myös. Tulokset säilyivät pääsääntöisesti positiivisina seurantamittaukseen saakka. Seurantamittaus tehtiin puhelimitse ja sähköpostitse noin kolme kuukautta hoitojen päättymisen jälkeen. Poikkeuksena positiivisesta muutoksen suunnasta oli asiakas A, joka koki akillesjännerepeämän noin 1,5 kuukautta hoitojakson päättymisen jälkeen. Hänen tilansa oli hoitokokeilun seurantajakson jälkeen parempi kuin lähtötilanne hoitoja aloitettaessa. Tulokset eivät ole yleistettävissä tutkimusjoukon ulkopuolelle. Tutkimukseen osallistui pieni ja heterogeeninen joukko, minkä vuoksi tulokset ovat tapauskohtaisia ja suuntaa-antavia.

The purpose of this thesis was to collect information about LPG-treatment and its possibilities as a part of Achilles tendon rehabilitation process. The aim was to produce new information for the subscriber Mikkelin Kutopalvelu Ltd and for the industry of physical therapy about the LPG-treatment and its possibilities to rehabilitate achillestendon injuries. The results were measured by patient’s own experience about the functional benefit of the treatment and the relief of pain. During the thesis process, a therapeutic trial was carried out, aiming to find out if LPG-treatment had an influence on patient’s experience of pain and activity limitations on ones daily life during the achillestendon healing process. The measurements used in the trial were a Patient Spesific Functional Scale (PTA) and Visual Analogue Scale (VAS). People who participated in the trial were two achillestendinosis patients and another patient who had had an achillestendon rupture that had not healed properly. The Patients received LPG-treatment for four weeks. The number and frequency of the treatments differed between patients according to their possibilities to be treated. The results of the trial showed that the activity limitations on patients’ daily life measured by the PTA-scale were reduced and the amount of pain measured by the VAS-scale was decreased. The results remained positive after the three months follow-up measurement. The follow-up measurement was done by telephone and e-mail. An exception to the positive follow-up results was one patient, though, who had suffered an achillestendon rupture one and a half months after the trial was finished. However, the patient felt that his condition was better than the initial situation when the treatment was started. The results cannot be generalized beyond the research patients. A small and heterogeneous group participated the trial, which is why the results are only case-specific.
