Satakunnan keskussairaalan lyhytjälkihoitoisen synnytyksen näyttöön perustuvan käytännön kehittäminen ja pilotointi

Abstract

Tämän työn tarkoituksena oli suunnitella Satakunnan keskussairaalan synnytysvuodeosastolle toimiva malli lyhytjälkihoitoisen synnytystoiminnan aloittamiseksi kuvaamalla siihen liittyvät näyttöön perustuvat käytännöt ja näiden pohjalta kehittää malli toiminnalle synnytysvuodeosaston nykyisten resurssien pohjalta ja aloittaa mallin pilotointi. Tavoitteena oli lyhytjälkihoitoisen synnytystoiminnan pilotoinnin aloittaminen Satakunnan keskussairaalan synnytysvuodeosastolla vuonna 2008. Kehittämisprosessi aloitettiin huhtikuussa 2007 ja pilotoinnin oli tarkoitus päättyä vuoden 2008 lopussa. Kehittämisprosessissa käytettiin näyttöön perustuvan kehittämisprosessinmallia. Malliksi valittiin The Iowa Model, josta tässä työssä käytettiin nimeä Iowan malli. Kehittämisprosessi rajattiin tiedonhakuun, suunnitteluun ja pilotointiin. Lopullinen päätöksenteko toiminnan aloittamisesta oli tarkoitus tehdä pilotoinnin arvioinnin tulosten perusteella. Kehittämisprosessiin osallistui synnytysvuodeosastolla kehittämisryhmä ja muu henkilökunta osallistettiin kehittämiseen osastotuntien puitteissa. Lyhytjälkihoitoisen synnytyksen hoidon näyttö haettiin tutkimuksista ja asiantuntija artikkeleista, sekä kokemusnäytöstä Tampereen yliopistosairaalan toiminnasta. Näiden perusteella muodostettiin kriteerit ja toimintaohjeet pilotoinnille. Pilotointi aloitettiin 18.2.2008 ja se jatkuu edelleen. Pilotoinnin loppuvaiheeseen mennessä kertyneet kokemukset olivat hyviä. Etukäteisinformaation antamista asiakkaille tulisi mahdollisesti lisätä ja järjestää toiminnalle kiinteämpi henkilöstö, joka sitoutuisi työhön ja sen kehittämiseen kiinteämmin. Pilotoinnin aikana noin 7 % synnyttäjistä osallistui lyhytjälkihoitoiseen synnytyksenhoitoon, tämä on odotettua vähemmän ja pilotoinnin perusteella asiakkaiden tiedottamista tuleekin kehittää.

The purpose of this thesis was to plan a model for short-term hospital birth in the Central Hospital of Satakunta according to a development process model based on empirical data. The objective of this study was to plan a practical model for the maternity ward in the Central Hospital of Satakunta in order to launch a program for short-term hospital births. The study describes the practices relating to short-term hospital births based on empirical data, on the basis of which an operational model was developed using the current resources of the ward and the piloting of the model was launched. The main objective was to launch a pilot project for short-term hospital births in the maternity ward of the Central Hospital of Satakunta during the year 2008. The development process was initiated in April 2007. The pilot project is scheduled to finish at the end of 2008. In this process, a development process model of evidence based practice was used. The model chosen was the Iowa Model, referred as Iowan malli (Iowa Model) in this study. The development model consisted of references, planning and piloting. The final decisions of launching the project will be done after evaluations of the pilot results. The development process included a development team in the maternity ward. Other personnel participated in the development during working hours in the ward. The empirical data for short-term hospital birth care was gathered from research and expert reports in the field as well as from empirical experiences in the Tampere University Hospital. The criteria and instructions for the piloting were formed according to the acquired information. The pilot project was launched on 18 February 2008 and is still in practice. The experiences gained so far are excellent. The author came to the conclusion that the clients need possibly more advance information. The personnel should also be organised more fixed in order for them to commit to the activities and development of the project. Approximately 7 per cent of pregnant women agreed to have a short-term hospital birth which was less than anticipated. Thus the clients need more information about the project.
