Näön ja kuulon merkitys sekä niiden arviointi ja tukeminen : Laitoshoidossa olevien ikääntyneiden kokemuksia

Abstract

Opinnäytetyön tarkoituksena oli selvittää laitoshoidossa olevilta ikääntyneiltä näön ja kuulon merkitystä toimintakykyyn. Lisäksi tarkoitus oli kerätä tietoa siitä, miten hoitohenkilökunta tukee ja arvioi heidän näkö- ja kuulokykyään. Tavoitteena oli, että tutkimuksesta saadun tiedon avulla voidaan jatkossa kehittää näön ja kuulon tukemisen toimintakäytäntöjä ja sen myötä edistää ikääntyneiden elämänlaatua. Opinnäytetyö toteutettiin kvalitatiivisena. Tutkimusaineisto kerättiin teemahaastatteluilla. Kohderyhmä oli harkinnanvarainen ja sen muodosti kuusi vanhainkodin asukasta Raumalta. Haastattelumateriaali litteroitiin ja analysoitiin teorialähtöisellä sisällönanalyysillä. Opinnäytetyön tutkimustulosten mukaan näkö- ja kuulokyky olivat yhteydessä ikääntyneiden fyysiseen, psyykkiseen ja sosiaaliseen toimintakykyyn ja vaikuttivat merkittävästi heidän elämänlaatuunsa. Tutkimuksessa selvisi, että ikääntyneet kokivat kuitenkin pärjäävänsä fyysisesti, hoitajilta saamansa avun turvin. Aistiongelmat vaikuttivat merkittävästi mielialaan, rajoittivat ikääntyneiden sosiaalista kanssakäymistä sekä altistivat eristäytymiseen. Saatujen tulosten mukaan näkemistä ja kuulemista tukevia apuvälineitä oli heikosti saatavilla tai sitten niitä ei ollut esitelty haastateltaville riittävästi. Myös silmälasien hoitoon ja riittävyyteen kiinnitettiin heikosti huomiota. Tutkimuksesta selvisi, että vanhainkodin tilojen valaistukseen ja väreihin oli kiinnitetty riittävästi huomiota ja ne tukivat hyvin ikääntyneiden näkemistä. Tulosten mukaan ikääntyneiden heikentynyttä kuuloa tuettiin puhumalla riittävän kovalla äänellä ja autettiin heitä tarpeen mukaan pyydettäessä. Tutkimuksen mukaan näön ja kuulon tutkimukset eivät ole säännöllisiä, eikä niitä ole järjestetty riittävästi. Hoitajien suorittama arviointi ei ollut myöskään riittävää. Saatujen tulosten mukaan ikääntyneet eivät kuitenkaan itse kaipaa säännöllisempiä näön ja kuulon tutkimuksia. Vastaisuudessa voitaisiin selvittää näön ja kuulon tukemisen toimintakäytäntöjä ikääntyneiden palveluasuntoloissa. On tärkeää, että esiin tuleviin ongelmakohtiin puututtaisiin mahdollisimman aikaisessa vaiheessa. Tämä tutkimus voitaisiin myös uusia muutaman vuoden kuluttua, jolloin kuultaisiin, minkälaisia parannuksia on tehty ja miten niiden vaikutukset ovat nähtävissä.

The purpose of this thesis was to determine the importance of sight and hearing of the elderly people in institutional care when it comes to their ability to act, and also how they think those abilities are being supported and evaluated by the nursing staff. The aim of this research was to collect information in order to further develop the procedures in supporting the use of sight and hearing, and thus promote the quality of life among the elderly. The thesis is based on a qualitative research. Thematic interviews were used to collect the research material. The target group of this research was discretionary and it consisted of six residents living in an old people’s home in Rauma. The interview material was transcribed and then analysed by using theory based content analysis. The results indicate that the visual and hearing abilities are connected with the physical, psychological and social functions of the elderly and have a significant impact on their everyday life. According to the results the elderly think they can cope with the support they get from the nursing staff physically. Sensory problems had a significant effect on the mood; they set limits to the social relations and exposed the elderly to isolation. Based on the results there were few supporting accessories available or the elderly had not been acquainted with them in an adequate manner. Also, little attention had been paid on the care and availability of the spectacles. It turned out that the lighting and the colour scheme of the premises had been paid attention to in a way that supports the sight of the old people. According to the results impaired hearing was supported by speaking in a sufficiently loud voice and the elderly were being helped if necessary. The thesis proves that the sight and hearing are not tested on a regular basis and not often enough. The evaluation performed by the nurses was not sufficient either. However, according to the results the elderly do not wish for any more frequent tests. In future, these supportive visual and auditory procedures could be investigated in the residential homes for the elderly. It is essential that the emerging problems are dealt with as early as possible. This survey could be reconducted in a few years so as to find out what kinds of improvements have been made and how the effects can be seen.
