Rauman vanhuspalveluiden intervalliyksikön toiminnan kehittäminen

Abstract

Tämän kehittämistehtävän tarkoituksena oli kehittää lyhytaikaishoidon yksikön toimintatapoja nykyistä paremmiksi sekä asiakaslähtöisemmiksi, jotta kyettäisiin tukemaan iäkkään toimintakykyä ja siirtämään laitoshoidon tarvetta pidempään. Iäkkäiden määrän kasvaessa ja samalla myös palveluiden tarpeen lisääntyessä, tulee mietittäväksi toimintakyvyn tukeminen eri osa-alueilla kotona asumista tukevaksi. Lyhytaikaishoidon tarve tulee lisääntymään ja samalla tulisi huomioida ja tukea iäkkään omia voimavaroja sekä selviytymiskeinoja.

Kehittämistehtävä toteutettiin laadullisena toimintatutkimuksena. Kehittämistehtävän aineisto kerättiin yksikön kehittämisiltapäivissä, joita toteutettiin kaksi kertaa sekä henkilökunnan teemahaastattelujen avulla.

Hoitohenkilökunnan yhteisissä kehittämisiltapäivissä pyrittiin löytämään uusia asiakaslähtöisiä toimintatapoja. Ryhmähaastatteluiden kautta selvitettiin toteutuneita käytäntöjä ja miten hoitohenkilökunta kokee tehdyt muutokset ja mitä heidän mielestä toimintakyvyn tukeminen on.

Kehittämistehtävän tuotoksena syntyi lyhytaikaishoidon yksikköön uusia toimintatapoja. Käytännön työssä kehittäminen näkyi iäkkään tulovaiheessa jaksolle tultaessa tehtäviin tarkentaviin kysymyksiin. Tulovaiheessa omaiselta kysytään jatkossa vielä täydentäviä kysymyksiä, jotta osastotyössä osattaisiin kiinnittää entistä tarkemmin huomiota asiakkaan voimavaralähtöiseen hoitotyöhön. Kirjaamisen tärkeys nousi merkittävään osaan, johon kehitettiin avuksi muistilista kirjaamisen sisällöstä. Viriketuokioiden pitämiseen tehtiin erillinen kansio, josta löytyy vaihtoehtoja tuokion pitämiseen. Myös kuntoutussuunnitelma on päässyt alkuun tutkimuksen kirjoittamisen aikana aktivointisuunnitelmana, jonka käyttöönotosta on jo sovittu ja sitä kehitellään edelleen tarpeen mukaan. Hoitohenkilökunnan yhteinen päämäärä, tavoite oli kaikilla sama – asiakaslähtöinen, toimintakykyä tukeva hyvä hoitotyö.

The purpose of this assignment was to improve the working methods of the short-term care-unit and to develop a more customer oriented approach, and thus to be able to support the functional ability of the elderly and move the need for institutional care further. As the number of seniors grows and the need for service increases, it becomes necessary to consider how the different factors relating to functional ability could be utilized to best support living at home. The need for short-term care will increase in the future and at the same time, the individual resources and coping methods of the elderly should be taken into consideration and supported. The assignment was carried out in the form of qualitative action research. Materials for the thesis were collected during two development afternoon sessions at the unit, and by arranging themed interviews with the medical staff. The goal of the joint development afternoons for the medical staff was to discover new customer oriented work practices. The group interviews were utilized to determine what practices were in use and to find out how the staff felt about the recent changes and what was their view of the meaning of functional ability support. As an outcome of the assignment, the short-term care-unit adopted new policies. In practical work, the development was seen in the specifying questions asked from relatives at the time of an elderly customer´s arrival at the unit. In future, during admission, relatives are asked further supplementary questions to be able to pay more attention to customer resource-based nursing at the ward. The importance of logging patient information became a significant factor in the study, and thus a new checklist to aid the logging process was developed. A separate folder containing more options for recreational activities was made. Also, at the time of writing this research, a rehabilitation plan has been initiated as an activation plan. The implementation of the plan has already been agreed on and it will be further developed as needed. The short-term careunit staff all shared the same goal, a common purpose – a performance supporting and customer oriented approach to good nursing.
