Haavanhoito-opas esperi hoivakoti jokilaaksoon

Abstract

Opinnäytetyön tarkoituksena oli suunnitella, laatia ja arvioida haavanhoidon opas Esperin hoivakoti Jokilaakson henkilökunnan käyttöön. Oppaan tavoitteena oli yhtenäistää hoitajien haavanhoito käytäntöjä sekä auttaa hoitotyöntekijöitä kertaamaan perustietoja haavanhoidosta. Oppaan lisäksi luotiin kirjaamisen tarkistuslista helpottamaan kirjaamista sekä yhdistämään kirjaamisen sisältöä.

Oppaan sisältö valikoitui kartoituskyselyn myötä, kysely jaettiin 32:lle hoitajalle. Kyselyyn vastasi 20 hoitajaa. Kartoituskyselyllä selvitettiin mitä osa-alueita haavanhoidosta kirjataan, miten hyvin haavanhoito koetaan hallitsevan, mitkä osa-alueet koetaan hallitsevan huonoiten sekä mitä haavanhoidossa kehitettäisiin.

Haavanhoito koettiin osaavan keskinkertaisesti, kirjaamisessa otetaan huomioon kaikki oleellinen, Mekaaninen puhdistus ja haavanhoito tuotteiden valinta tuotti hankaluuksia sekä kehittämistä toivottiin koulutuksen ja yhteisten ohjeiden muodossa. Etenkin haavanhoitotuotteiden valinta ja mekaaninen puhdistus nousi esille kartoituksen myötä. Opas on koottu ajankohtaista kirjallisuutta sekä haavanhoidon ammattilaisten julkaisuja hyödyntäen.

Oppaassa on käyty tiiviisti läpi haavatyypit, haavan arvioiminen, haavanhoitotuotteet ja niiden hankinta, haavan puhdistaminen, haavakivun arviointi ja hoito, haavan valo-kuvaaminen sekä haavanhoidon kirjaaminen. Oppaasta pyydettiin palautetta kuudelta hoitajalta. Palautteen myötä opasta havainnoitiin kuvilla. Lisäksi kaivattiin tietoa kivun hoidosta ja puuduttamisesta haavanhoidossa. Muuten opas koettiin selkeäksi ja päivittäistä haavanhoitoa sekä kirjaamista helpottavaksi.

Opas esitellään Hoivakoti Jokilaakson henkilökunnalle viikkopalaverissa. Tilaisuudessa käydään lyhyesti läpi oppaan sisältö, kirjaamisen tarkistuslista sekä video mekaanisesta puhdistuksesta. Jatkossa vastaava sairaanhoitaja päivittää opasta tarpeen tullen. Jatkokehittämishaasteena voitaisiin tutkia, miten oppaan käyttö on hyödyttänyt hoita-jia haavanhoidossa ja kirjaamisessa, sekä onko käytännöt yhtenäistyneet

The purpose of the thesis was to design, prepare and evaluate a wound care guide for the use of Espero-nursing facility in Jokilaakso, and their nursing staff. The aim of the guide was to create cohesion in the wound care practice of the nursing staff and to help nurses to recap the basic protocol on wound care management. In addition to the guide, a checklist was created to alleviate record keeping, as well as to cohesively combine the contents of the records.

A survey questionnaire was distributed to 32 caregivers, whereof 20 completed the survey. The results of the survey were then used to outline the content of the guide. The questionnaire inquired which areas of wound care were recorded, how well the nursing staff deemed their wound care management abilities, as well as which areas of the care are considered to be the most vaguely handled and what should be further developed in the general wound care management.

The results of the survey indicated that managing wound care by the nursing staff was experienced to be mediocre. The areas recorded for wound care were the ones deemed essential and prominent. From the survey, it seemed that mechanical cleans-ing and product selection caused difficulties, and development was hoped for in the form of training and general instructions in these areas. The choice of wound care products and mechanical cleaning seemed to take a prominent attention. This wound care guide has been compiled using appropriate literature as well as the publications of wound care professionals.

The guide has intricately followed up on different wound types, the assessment of the wounds as well as the products for wound care and their procurement, wound cleansing, wound healing-and treatment, wound photography and wound record keeping. Feedback on the guide was requested from six caregivers. Taking the feed-back in to consideration, the guide was thereafter equipped with images. In addition, information about pain management and local anesthesia in wound care was includ-ed. With these additions, the wound care guide was considered coherent and clear, as well as an aid in the daily wound care procedures.

The completed wound care guide will be presented to the staff at the Nursing facility in Jokilaakso during a weekly meeting. During the presentation, the content of the guide will be briefly reviewed alongside of the checklist for record keeping. A video of mechanical wound cleansing will be presented. A nurse will be in charge of updat-ing the guide in the future accordingly. In the future it would be interesting to re-search on how the guide has affected its respective users in a positive manner. It would be interesting to explore how the use of the guide has benefitted the nurses in wound management and record keeping, and whether the procedures and protocols of wound care has become more uniform.
