Palvelutalossa asuvan ikääntyneen suun terveydenedistäminen : Toimintamalli Vaasan kaupungin hammashuollossa

Abstract

Ikääntyneen väestön määrä lisääntyy Suomessa tulevien vuosikymmenien aikana. Suun terveydellä on suuri merkitys yleisterveydelle sekä hyvinvoinnille ja suun terveys on yhteydessä moniin yleissairauksiin. Suun terveys tukee ikääntyneen terveyttä ja hyvinvointia sekä ehkäisee aliravitsemusta. Tehostettu palveluasuminen on lisääntynyt 2000-luvun ensimmäisinä vuosikymmeninä ja se on yleistyvä palvelumuoto. Myös suun terveydestä huolehtimisen tulee kuulua perustoimenpiteisiin palveluasumisyksikössä. Mikäli iäkäs henkilö ei ole kykenevä itse huolehtimaan riittävästä suuhygieniastaan päivittäin, on hänen hoidostaan vastuussa olevan henkilön huolehdittava toteutuksesta vanhuspalvelulakiin perustuen. Opinnäytetyö toteutettiin tutkimuksellisena kehittämistyönä ja kehittämiskohteena olivat Vaasan kaupungin suuhygienistien kaupungin tehostetuissa palveluasumisyksiköissä tekemät vierailut. Tarkoituksena oli kehittää toimintamalli suuhygienistien käyttöön palveluasumisyksikössä asuvan ikääntyneen suun terveyden edistämiseksi. Opinnäytetyöstä saaduilla tutkimustuloksilla suunnattiin toimintamallin kehittämistä. Kehittämisen suuntaamisen tueksi tehtiin myös tiedonhakua palvelutalojen hoitohenkilöstöön kohdistuvista interventioista iäkkään suun terveyden parantamiseksi sekä pohjana toimi Sosiaali- ja terveysministeriön toimintamallisuositukset iäkkäiden suun terveyden parantamiseksi. Tutkimuksessa oli mukana kahdeksan Vaasan kaupungin suuhygienistiä. Aineisto kerättiin kevään ja kesän 2018 aikana teemahaastatteluin yksilöhaastatteluina. Haastattelun teemoina olivat ikääntyneiden asukkaiden suun terveyden ja suuhygienian tila, hoitohenkilöstölle annettu ohjeistus, vierailujen toteutus, kokemukset vierailuista sekä vierailut jatkossa. Teemat nousivat aiemmista tutkimuksista sekä STM:n suosituksista. Aineisto analysoitiin aineistolähtöisellä sisällönanalyysillä. Tulosten tulkinnan mukaan Vaasan kaupungin suuhygienistit kokivat, että suuhygienian ja suun terveydentilat olivat vaihtelevia, suuhygienian toteutuminen oli moninaista sekä suun tilat olivat vaihtelevia toimintaympäristön sisällä. Ohjeistus oli tai voisi olla toimintaympäristöön kohdennettua, toimintaympäristössä tapahtuvaa sekä usealle taholle annettua ohjausta. Suuhygienistien kokemuksista ilmenivät resurssien käytön suunnitelmallisuus sekä sisäinen ja ulkoinen ammatillinen ja moniammatillinen yhteistyö.

In the future population is ageing in Finland. Oral health has a big impact on general health and is associated with many general illnesses. Good oral health supports health and well-being in the elderly and it also prevents malnutrition. Form of intensive assisted living has increased in the first two decades and it is becoming more common. Oral health care should also be included in the basic routines in the assisted living facilities. Based on the law the person responsible of the care must perform daily oral hygiene routines if the elderly is not capable taking care of one’s sufficient daily oral hygiene. This thesis was executed as a constructive study and the development area was the visits dental hygienists in municipal health care in Vaasa have made in the city’s intensive assisted living facilities. The goal was to develope a model for the dental hygienists to promote the oral health of the elderly living in the assisted living facilities. The results from this thesis were used to direct the developement process of the model. To support the direction process the systematic literature search considering interventions targeted on nursing staff to promote the oral health of the elderly were made. The model was based on the recommendations of the Ministry of Social Affairs and Health. Eight dental hygienists participated the study. The data was collected during the spring and summer 2018 through the individual thematic interviews. The themes were the oral health and oral hygiene status, the instructions given to the nursing staff, execution of the visits, experiences considering the visits and the visits in the future. Themes were based on the background literature and the recommendations of the Ministry. The data was analysed using inductive qualitative content analysis. The interpretation of the results showed that dental hygienists experienced there were variation within oral health and within oral hygiene, the performance of the oral hygiene was diverse and oral status varied within the environment. Given instructions were or could be targeted on the environment, happening in the environment or given to multiple directions. Organized use of resources and internal and external professional and multi-professional collaboration occured from the dental hygienists experiences.
