Soiten kirurgisten osastojen hoitohenkilökunnan kokemuksia kuntouttavasta työotteesta

Abstract

Opinnäytetyön toimintaympäristönä oli Keski-Pohjanmaan keskussairaalan Soiten kirurgiset osastot 7 ja 10. Opinnäytetyön tarkoituksena oli kuvata kirurgisten osastojen hoitohenkilökunnan kokemuksia kuntouttavasta työotteesta ja sen merkityksestä. Tarkoituksena oli myös kuvailla hoitohenkilökunnan käsityksiä omasta osaamisestaan ja löytää sekä kuntouttavaa työotetta edistäviä että heikentäviä tekijöitä. Opinnäytetyön tavoitteena oli tuottaa hoitohenkilökunnalle uutta tietoa, jolla he voivat kehittää osaston kuntouttavaa työotetta sekä parantaa olemassa olevia toimintatapoja.

Opinnäytetyö toteutettiin laadullisella tutkimusmenetelmällä. Aineistonkeruumenetelmänä käytettiin paperisia vastauslomakkeita. Kysely toteutettiin touko-kesäkuun vaihteessa 2018. Aineisto analysoitiin induktiivisella sisällönanalyysillä.

Opinnäytetyön tuloksien mukaan kuntouttava työote koettiin tärkeänä osana hoitotyötä. Kuntouttavan työotteen ansiosta potilaat toipuvat nopeammin ja sairaalassaoloaika lyhenee. Se myös vähentää hoitohenkilökunnan työmäärää. Hoitohenkilökunta oli ymmärtänyt kuntouttavan työotteen merkityksen ja koki osaavansa sen, mutta halu oppia uutta koettiin silti tärkeänä. Työvuosien mukana tuoma varmuus sekä kollegoilta saatu tuki ja neuvot lisäsivät onnistumisen tunnetta omaan työskentelyyn. Edistäviksi tekijöiksi koettiin hoitohenkilökunnan yhtenäiset työtavat, riittävä osaaminen ja tieto kuntoutuksesta sekä toimivat apuvälineet. Kuntouttavaa työotetta heikentäviä tekijöitä olivat resurssipula ja siitä johtuva kiire, sekä se, etteivät kaikki työyhteisössä noudata yhteisiä sääntöjä. Hoitohenkilökunnan mielestä kuntouttavaa työotetta voidaan kehittää riittävällä ja osaavalla miehityksellä, monipuolisilla lisäkoulutuksilla sekä moniammatillisuuden korostamisella. Myös oma potilaslähtöinen ja myönteinen asenne kuntoutukseen edistää kuntouttavaa työotetta.

Opinnäytetyön jatkotutkimuskohteena voisi olla potilaiden kokemus kuntouttavasta työotteesta Soiten kirurgisilla osastoilla. Työssä voisi selvittää, miten he kokevat kuntouttavan työotteen ja ovatko he saaneet riittävästi ohjausta ja informaatiota. Toinen vaihtoehto olisi tämän työn toteuttaminen haastatteluina ja vertailla suullisia vastauksia kirjallisiin vastauksiin.

The operational environment of this thesis was the Central Ostrobothnia Joint Municipal Authority for Social and Health Care, Soite’s, surgeries 7 and 10. The purpose of the thesis was to describe the sur-geries’ personnel’s experiences of rehabilitating methods and their significance. Another purpose was to describe the nursing personnel’s views on their own abilities and to discover some factors that ei-ther support or debilitate the methods. The main goal of this thesis was to produce new information for the nursing staff so that they would be able to further develop the rehabilitating methods in the surgeries as well as to improve the procedures already in use.

The thesis was executed with a qualitative research method. The data was gathered with printed questionnaires and the inquiry was conducted in between May and June of 2018. The data was then analyzed using an inductive content analysis method.

The results of this thesis showed that rehabilitating methods were regarded as an important part of care work. Owing to the use of the methods, the patients recovered faster and spent less time in the hospital. The methods also reduced the staff’s workload. The staff had understood the importance of rehabilitating methods and regarded themselves competent in using them. However, eagerness to learn more was also valued. Confidence gained through years of experience, as well as the support and ad-vice received from colleagues increased the feelings of success in one’s work. Unified methods, func-tioning tools, sufficient skills, and knowledge of rehabilitation were seen as supportive factors. Short-ages in resources and the resulting hurry and not abiding to fixed rules were seen as debilitating fac-tors, respectively. According to the staff, rehabilitating methods can be developed by ensuring a suffi-cient number of competent staff, diverse training sessions, and by emphasizing multi-professionalism. Also one’s own positive, patient-oriented attitude towards rehabilitation supports the methods.

Further research on the subject could be conducted on the patients’ views on rehabilitating methods in Soite’s surgeries. The research could aim to investigate how the patients experience the methods and whether or not they got enough information and guidance on them. Another option could be to modi-fy this research into a verbal interview and compare the verbal and written answers.
