Anoreksia nuoren sairautena : Potilaan kohtaaminen ja siihen liittyvät haasteet hoitajien kokemana

Abstract

Opinnäytetyön tarkoituksena oli kuvata hoitajien kokemuksia ja heidän kohtaamiaan haasteita anoreksiaa sairastavan nuoren hoitotyössä. Tavoitteena oli tuottaa anoreksiaa sairastavan nuoren hoitoon liittyvää tietoa, joka hyödyttää hoitohenkilökuntaa sekä hoitotyötä opiskelevia. Tutkimus toteutettiin Keski-Pohjanmaan sosiaali- ja terveyspalvelukuntayhtymän alueella. Tutkimusmenetelmänä käytimme kvalitatiivista tutkimusta. Aineisto kerättiin teemahaastattelulla. Haastattelimme viittä sairaanhoitajaa, jotka olivat työskennelleet anoreksiaa sairastavien nuorien kanssa. Aineiston analysoimme induktiivista sisällönanalyysia käyttäen.

Tiedonantajien mukaan anoreksiaa sairastavan nuoren kohtaamiseen vaikuttavat monet tekijät, kuten yksilöllisyys, sairauden vaihe sekä fyysinen kunto. Hoitosuhteen luominen on aikaa vievä prosessi, sillä luottamus rakentuu hitaasti. Hoitajien mukaan hoitosuhteen edistyminen ilmenee siten, että suhde muuttuu ja syventyy. Kohdatessaan nuorta hoitajat säilövät voimakkaita tunteita. He vastaanottavat nuoren ahdistusta, vastarintaa ja pelkoa, minkä vuoksi hoitajilta vaaditaan tietynlaisia ominaisuuksia ja ammattitaitoisuutta. Lisäksi nuoren sairaus vaikuttaa koko työryhmään.

Tiedonantajat kokivat haasteita nuoren sairauden vaiheen vaikutuksessa hoitoon. Varsinkin viehtymysvaihe sekä nuoren riittämätön käsitys omasta sairaudestaan luovat haasteita. Nuoren sitoutuminen hoitoon ilmenee nuoren motivaatiossa sekä luottamuksen ja uskalluksen puutteena. Perheen ja hoitavan tahon yhteistyötä korostettiin, sillä ristiriidassa olevat hoitolinjaukset hidastavat nuoren hoidon prosessia. Lisäksi perheen sitoutuminen hoitoon ja tuen puute luovat haasteita, jolloin sekä hoitosuhteen eteneminen että hoidon eteneminen koetaan haasteellisina.

The purpose of our thesis was to find out what kind of experiences nurses had when they encountered a young patient struggling with anorexia nervosa. The aim was to produce information regarding treatment of anorexia nervosa. The information will benefit health -care personnel and nursing students. The survey was conducted in Central Ostrobothnia. We used qualitative survey as a method of our thesis. The data was collected by a theme interview. We interviewed five nurses who had worked with patients dealing with anorexia nervosa. The data was analyzed using inductive content analysis.

According to the respondents many factors affect the encountering of an anorexia nervosa patient. For example, individuality, stage of the illness and physical condition affect the encountering. Establishing a relationship with an adolescent is a time-consuming process because trust between a patient and a nurse is built slowly. According to the nurses, the nurse -patient relationship changes and deepens with time. The encounter affects also the nurses. They receive adolescents’ anxiety, resistance and fear. Because of that, they are required certain types of characteristics and professional skills. The illness of the patient affects also the whole nursing staff.

The respondents felt that the stage of the illness affects the treatment. Especially when the adolescent does not consider themselves sick and has insufficient understanding of the ill ness. According to the nurses, patient comm itment to the treatment is challenging. Adolescents’ motivation, lack of both trust and courage affect the commitment. There are also difficulties between the family of the patient and nursing staff. Nurses told that the conflicts delay the treatment. Families often take a negative view to the treatment and do not support the patient in care. Nurses found that the progress ion of the nurse-patient relationship is challenging. Also, the progression of the treatment is a challenge according to the respondents.
