Imetyksen tuen toimintasuunnitelman kehittäminen toimintatutkimuksen avulla

Abstract

Opinnäytetyön aiheena oli imetyksen tukeminen neuvolassa. Opinnäytetyö tehtiin neuvolan terveydenhoitajan imetysohjauksen ja imetyksen tukemisen näkökulmasta. Imetysohjaus ja imetyksen tukeminen valikoitui opinnäytetyön aiheeksi, koska imetysohjauksiin tai imetyksen tukemisiin ole ollut selkeitä ja yhteneväisiä ohjeita neuvolan terveydenhoitajilla. Imetysohjausta ja imetyksen tukemista tutkittiin opinnäytetyössä perhelähtöisesti.

Opinnäytetyön tavoitteena oli kehittää imetyksen tuen toimintasuunnitelma neuvolatyöhön. Imetyksen tuen toimintasuunnitelman avulla yhtenäistettiin imetysohjausta sekä lisättiin vauva- ja perhemyönteisyyttä neuvolatyössä. Imetyksen tuen toimintasuunnitelman tarkoituksena oli ohjata terveydenhoitajan antamaa imetysohjausta perhelähtöisesti. Tavoitteena oli luoda imetyksen tuen toimintasuunnitelma, joka olisi mahdollisimman helposti siirrettävissä toisiin neuvoloihin.

Opinnäytetyö tehtiin toimintatutkimuksena. Opinnäytetyö aloitettiin kuvailevalla kirjallisuuskatsauksella, jonka avulla saatiin imetysohjauksesta ja imetystuesta kokonaiskuva aikaisempien tutkimusten perusteella. Aineisto kerätiin teemahaastattelemalla yhdeksää äitiä. Teemahaastattelujen teemat muodostettiin kirjallisuuskatsauksen analyysin perusteella. Terveydenhoitajien näkökulma saatiin esille työkonferenssin avulla. Teemahaastatteluiden ja työkonferenssin tulosten avulla tehtiin imetyksen tuen toimintasuunnitelma.

Opinnäytetyön tuloksena kehitetty imetyksen tuen toimintasuunnitelma helpottaa neuvolan terveydenhoitajia imetyksen ohjauksessa, myös imetyksen erilaisissa ongelmatilanteissa, sekä imettävän äidin ja hänen perheensä tukemisessa. Imetyksen tuen toimintasuunnitelma kehitti neuvolatyötä paikallisesti. Imetyksen tuen toimintasuunnitelman siirrettävyys toisiin neuvoloihin on mahdollista, koska imetyksen tuen toimintasuunnitelma perustuu Terveyden ja hyvinvoinnin laitoksen Neuvolan vauva- ja perhemyönteisyysohjelmaan. Imetyksen tuen toimintasuunnitelmaan tehtiin arviointisuunnitelma, jotta imetysohjaus olisi laadukasta.

The topic of the master’s thesis was the support of breastfeeding at child health clinics. It was carried out from the viewpoint of the breastfeeding guidance and support provided by a public health nurse working at the child health clinic. These two topics, breastfeeding guidance and breastfeeding support, were chosen as topics of study as there have not been clear and uniform guidelines for them provided to nurses working at child health clinics. Breastfeeding guidance and support were studied in this thesis from a family-oriented viewpoint.

The goal of the thesis was to formulate a strategy for breastfeeding support, to be utilized in child health clinics. By means of this strategy, a more baby and family friendly approach at the clinics was developed and standardized. The thesis aimed to approach breastfeeding support in a family-oriented way. The goal was to create a strategy for breastfeeding support that could easily be applied at other child health clinics.

Action research was utilized as the form of research in this thesis. The thesis begins with a descriptive section on theory and previous research done on the subject, which aid in providing a holistic view of breastfeeding guidance and support. The material was gathered by conducting focused interviews with nine mothers. The themes and contents of the interviews were formulated from the analysis done on the previous studies and research. The viewpoint of the public health nurses was studied at a work conference. The strategy for breastfeeding support was formulated based on the results of the focused interviews and the work conference.

The breastfeeding support strategy formulated in this thesis will aid the public health nurses working at the child health clinics in breastfeeding guidance and support and in handling various problematic situations that may arise. The breastfeeding support strategy evolved the work at the child health clinic at a local level. The breastfeeding support strategy can be applied at other child health clinics as it is in accordance with the program for baby and family-friendly approach at child health clinics laid out by the Finnish National Institute for Health and Welfare. To ensure that the breastfeeding support provided would be proficient, an evaluation plan was crafted to support the breastfeeding support strategy.
